Job Title: RN – Adult Care / Home Care
Location: Bronx, NY
Schedule:
• 5 Days/Week
• 7.5 Hours/Day
• 37.5 Hours/Week
• Every Other Weekend Required
Employment Type: Contract / Full-Time Opportunity
Position Overview:
We are seeking an experienced and compassionate Registered Nurse (RN) for an Adult Care/Home Care opportunity serving patients throughout the Bronx Borough. The RN will provide high-quality in-home nursing care, assess patient needs, administer treatments, and collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes.
Responsibilities:
• Assess, plan, implement, and evaluate individualized patient care plans
• Provide direct bedside nursing care to pre- and post-surgical patients
• Administer prescribed medications and treatments accurately
• Perform wound care, infection control, catheter insertion, IV starts, and dressing changes
• Monitor patient vital signs and report adverse reactions promptly
• Operate and adjust specialized medical equipment as needed
• Educate patients and families regarding procedures and post-operative care
• Collaborate with healthcare teams on discharge planning and continuity of care
• Maintain accurate patient documentation and medical records
• Follow established nursing standards and protocols
Requirements:
• Associate Degree in Nursing from an accredited institution required
• Active New York State RN License required
• AHA BLS Certification required
• Minimum 2 years of recent Med/Surg experience required
• Acute care hospital or post-acute/home care experience required
• Most recent experience must be within the past year
• Prior home care experience required
• Must be local to the Bronx/NYC area
