Job Description

OC Health Wellness Group is seeking a dedicated and patient-focused Nurse Practitioner to provide comprehensive primary care services in an outpatient setting. The NP will be responsible for delivering preventive care, diagnosing and treating acute and chronic medical conditions, managing pain, promoting wellness, and coordinating patient care to improve overall health outcomes.

Essential Duties and Responsibilities

  • Perform comprehensive patient assessments, including medical histories, physical examinations, and health screenings.
  • Diagnose and treat acute and chronic illnesses across the lifespan.
  • Provide preventive healthcare services, annual wellness exams, routine screenings, immunizations, and health education.
  • Manage chronic conditions including hypertension, diabetes, obesity, hyperlipidemia, thyroid disorders, and other primary care conditions.
  • Evaluate and manage patients with acute and chronic pain conditions utilizing evidence-based treatment plans.
  • Order, interpret, and review laboratory tests, imaging studies, and diagnostic procedures.
  • Prescribe medications and treatments in accordance with California regulations and clinical protocols.
  • Develop individualized treatment plans and monitor patient progress.
  • Educate patients on disease prevention, medication management, nutrition, lifestyle modifications, and overall wellness.
  • Coordinate care with physicians, specialists, hospitals, home health agencies, and other healthcare providers.
  • Maintain accurate, complete, and timely documentation in the Electronic Health Record (EHR).
  • Participate in quality improvement initiatives and compliance activities.
  • Support wellness, weight management, and preventive health programs offered by the practice.
  • Perform office-based procedures within scope of practice and competency.


Qualifications

  • Current California Nurse Practitioner License in good standing.
  • National Board Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
  • Current DEA Registration and Furnishing Number.
  • Current BLS Certification.
  • Minimum one (1) year of primary care experience preferred.
  • Experience in family medicine, internal medicine, pain management, wellness, or urgent care preferred.
  • Strong clinical judgment, communication, and patient education skills.
  • Ability to work independently and collaboratively within a multidisciplinary healthcare team.

Employment Type: Full-Time / Part-Time

Compensation: Competitive salary or productivity-based compensation commensurate with experience.
