Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A critical role is available for a Clinical Pharmacist specializing in Home Infusion, dedicated to delivering expert care to patients undergoing complex infusion therapies. This position involves comprehensive chart and lab result reviews, ensuring precise medication management, and collaborating closely with physicians, nurses, and healthcare professionals to optimize patient outcomes.
Location:
- Lenexa, KS
Schedule:
- Monday to Friday, 8:00 AM 5:30 PM
- On-call every third weekend
Key Responsibilities:
- Evaluate patient charts for acceptance, product and device selection, and supply management.
- Analyze physician medication orders for therapeutic appropriateness, safety, accuracy, and cost-effectiveness.
- Monitor patient progress and communicate clinical data with physicians and the healthcare team; patient interaction primarily via phone with occasional visits to the infusion suite.
- Perform sterile compounding in the IV room as necessary.
Qualifications:
- Bachelor of Science or PharmD from an accredited Pharmacy School.
- Active Kansas Pharmacist License; additional state licenses may be required.
- Minimum of 1 year of recent clinical pharmacy experience, preferably in Home Infusion or hospital settings.
- Experience with sterile compounding is highly preferred.
- Willingness to participate in on-call rotation, approximately one weekend every three weeks.
