Job Overview
Providence Healthcare of Thomaston is seeking a compassionate and dedicated Occupational Therapist (OT) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Occupational Therapists to provide high-quality occupational therapy services in our skilled nursing and subacute rehabilitation setting.
As an Occupational Therapist, you will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions to improve independence, safety, and functional performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs). You will play a key role in helping residents regain meaningful participation in daily life following illness, injury, surgery, or hospitalization.
You will collaborate with an interdisciplinary team committed to delivering resident-centered care and achieving optimal functional outcomes.
Essential Duties and Responsibilities

Perform comprehensive occupational therapy evaluations and develop individualized treatment plans based on clinical findings, physician orders, and resident goals

Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, strength, coordination, balance, cognition, and fine motor skills

Address cognitive, perceptual, and functional deficits impacting independence and safety
-
Train residents in self-care tasks, adaptive techniques, energy conservation, and compensatory strategies
-
Recommend, fabricate, and train residents in the use of adaptive equipment, splints, and assistive devices as appropriate

Monitor resident progress, reassess outcomes, and modify treatment plans to maximize functional independence

Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelines

Collaborate with physicians, nursing staff, physical therapists, speech-language pathologists, dietitians, and other interdisciplinary team members to coordinate resident care

Educate residents, families, and caregivers on safety techniques, home programs, fall prevention, and adaptive strategies

Participate in interdisciplinary care planning meetings, family conferences, and discharge planning
-
Maintain compliance with all professional, regulatory, and facility policies and standards
-
Promote a positive, resident-centered rehabilitation environment focused on quality outcomes and clinical excellence
QualificationsRequired
-
Graduate of an accredited Occupational Therapy program
-
Current Georgia Occupational Therapist (OT) license in good standing
-
Current CPR certification or willingness to obtain upon hire

Strong clinical assessment, communication, and documentation skills

Ability to work effectively within an interdisciplinary healthcare team
Preferred
-
Experience in skilled nursing, subacute rehabilitation, long-term care, or inpatient rehabilitation

Familiarity with Medicare documentation and reimbursement guidelines

Experience working with geriatric and medically complex populations

Knowledge of evidence-based occupational therapy practices, adaptive equipment, and cognitive rehabilitation
