Position Overview
The Nursing Supervisor is pivotal in upholding the highest quality of patient care, ensuring alignment with the Agency’s objectives, standards, and policies. This role provides direct oversight of clinical staff, actively participates in home health service delivery when needed, and adeptly balances field-based responsibilities with administrative supervision to drive compliance, patient satisfaction, and staff development. Additionally, this position serves as a rotating on-call Manager on Duty (MOD), ensuring seamless patient care and staffing support during off-hours, weekends, and holidays.
Essential Duties And Responsibilities

Assist the RN Administrator in policy and procedure development, maintenance, and auditing.

Conduct patient intake assessments, including medical history review, physical assessment, and development of initial care plans.

Provide supervision, direction, and support to nursing staff and aides, ensuring clinical standards are maintained.

Conduct telehealth visits and provide remote patient monitoring as needed.

Conduct staff performance evaluations (probationary, annual, and as needed) with input from the RN Administrator

Review patient records for clinical accuracy, compliance, and appropriate care planning.

Direct staff in the use of community resources to support patients’ overall care plans.

Facilitate staff development through teaching, coaching, and counseling.
Ensure proper documentation of recertifications, discharges, supervisory visits, and rehospitalization forms. -
Support and assist with new employee orientation and field training.

Serve as a resource for conflict resolution, problem-solving, and guidance.
Field Responsibilities
-
Conduct supervisory visits and direct patient care visits when necessary.

Provide case management, coordinating with interdisciplinary teams to ensure comprehensive patient care and resource utilization.
Assess, evaluate, and monitor patients’ clinical needs during admissions, recertifications, and routine visits. -
Collaborate with physicians, patients, and families to ensure optimal care delivery.

Participate in interdisciplinary team meetings, quality assurance reviews, and utilization reviews.

Represent the agency in community functions and outreach activities.

Provide crisis intervention and clinical support in the field when required

On-Call MOD Responsibilities

Serve as the on-call Manager on Duty on a scheduled rotation, including evenings, weekends, and holidays.

Respond promptly to urgent patient needs, staffing issues, and clinical concerns after hours.

Provide direction to on-call staff, ensuring continuity of care and adherence to agency policies.

Escalate critical issues to the RN Administrator or Director of Patient Care Services as necessary.
Job Conditions

Qualifications
-
Current Registered Nurse (RN) license in the state of practice.
-
Minimum of 1 year nursing experience within the last 5 years, with at least 1 year supervisory experience in a home health agency, clinic, or healthcare facility.

Strong clinical decision-making skills and ability to manage diverse patient populations.
-
Knowledge and experience with Quality Assurance and Performance Improvement (QAPI) processes.

Current CPR certification.
-
Must pass a criminal background check.

Excellent leadership, organizational, and communication skills.
-
Willingness and ability to serve as On-Call Manager on Duty (MOD) on a rotating schedule, including evenings, weekends, and holidays.

Must demonstrate flexibility to work full-time, balancing field and supervisory responsibilities.
