Position Overview

The Home Care - Care Manager is responsible for coordinating and overseeing in-home care services for clients, ensuring they receive safe, high-quality, and individualized support. This role involves conducting assessments, developing care plans, monitoring client progress, and collaborating with healthcare providers, caregivers, and families to achieve positive health outcomes. The Home Care - Care Manager values creating partnerships, set and lead in-service, and creating referral partnerships.

Responsibilities

Care Coordination: Provide comprehensive care management services to individuals with complex healthcare needs, including assessment, planning, coordination, monitoring, and evaluation of care plans.

Client Advocacy: Serve as an advocate for clients ensuring their needs and preference are met and promoting their rights and self-determination thorough the care process.

Multidisciplinary Collaboration: Collaborate with healthcare providers, community resources, and other stakeholders to facilitate a seamless continuum of care and promote integration of services

Assessments and Documentation: Conduct thorough assessments of client needs, develop individualized care plans, and maintain accurate and up to date documentation of all interactions and interventions.

Education and Support: Provide education and support to clients, and their families regarding their health conditions, treatment options, and self-management strategies.

Quality Improvement: Participate in quality improvement and analyzing data, identifying areas for improvement and implementing evidence-based practices to enhance care outcomes.

Measurable Expectations: Develop and achieve measurable objectives within the first six months if being promoted, such as:

· Create an audit tool and lead monthly chart audits with the CACS staff,

· Lead or co-lead training once a month specific to working with older adults

· Lead weekly case review with CACS team

· Sustain a minimum of 20 billable hours per week

· Collaborate with Director of Operations to address and create policies and procedures to improve processes and enhance client experience.

· Establish partnerships with at least three community-based organizations.

· Maintain client confidentiality and ensure necessary releases of information are in the right place.

· This position may require providing client with transportation services.

· Other duties as assigned.

Qualifications

Education

· Bachelor’s degree in social work or related field. MSW or Certification in Care Management preferred.

Experience

· Experience: Minimum of two years’ experience in care management or related field, preferably working with older adults.

· Experience working with Jewish families and familiarity with Jewish culture and values is strongly preferred.

Competencies

· Understanding of social and healthcare systems, care coordination principles, and psychosocial factors that impact older adults.

· Excellent verbal and written communication skills.

· Strong interpersonal skills with the ability to build rapport, collaborate with multidisciplinary teams, and engage clients in their care.

· Exceptional organization skills with the ability to prioritize tasks, manage competing responsibilities, and meet project deadlines.

· Proficiency in using electronic health record systems, Microsoft Office Suite, and other relevant software applications.

· This position requires the ability to lift 25 pounds, sitting / standing for 30 minutes or more at a time.

· Reliable transportation, insurance, and California Driver’s License required.

Saraly and Benefits:

Salary Range: $75,000-$85,000 annually. Full-Time, Monday to Friday. This role may include occasional weekends and on-call duties, with compensation per agency policy.

This position is full-time, 37.5 hours/week. The salary is commensurate with the selected candidate's qualifications and experience.
