Schedule: Part-time, 16–20 hours per week (Monday–Friday, between 8:00 a.m.–4:00 p.m.)
Position Summary
The Occupational Therapist (OT) provides skilled therapeutic services to participants of the Adult Day Health Care (ADHC) program in accordance with physician orders, participant care plans, and state/federal regulations. The OT evaluates, plans, and implements individualized treatment programs designed to improve or maintain participants’ ability to perform activities of daily living (ADLs), promote independence, and enhance quality of life. The OT works as part of an interdisciplinary team to ensure holistic, person-centered care in a community-based setting.
Essential Duties & Responsibilities

Assessment & Evaluation
-
Conduct comprehensive occupational therapy evaluations of participants referred to the ADHC program.

Assess participants’ functional status, cognitive ability, motor skills, safety awareness, and ability to perform ADLs and instrumental ADLs.

Document findings and provide recommendations to the interdisciplinary team.

Treatment Planning & Intervention
-
Develop and implement individualized therapy plans in collaboration with the care team, focusing on rehabilitation, maintenance, or compensatory strategies.

Provide direct therapeutic interventions, including ADL retraining, upper extremity strengthening, fine motor coordination, cognitive-perceptual retraining, energy conservation, and adaptive equipment training.

Train participants and caregivers in the safe use of adaptive devices and strategies to improve independence.

Collaboration & Interdisciplinary Support

Participate in interdisciplinary team meetings to contribute to care planning and ongoing progress reviews.

Collaborate with nursing, social work, dietary, and activity staff to ensure therapy goals are integrated into daily care and programming.

Provide caregiver education and training to support carryover of therapy techniques in the home and community.

Documentation & Compliance

Maintain accurate, timely, and complete documentation in accordance with Medi-Cal/Medicare, Title 22 regulations, and ADHC program standards.

Ensure therapy services are delivered and billed appropriately per treatment authorization requests (TARs) and health plan requirements.

Follow all safety and infection control policies and procedures.

Professional Development & Leadership

Serve as a resource to staff regarding participant functional abilities, adaptations, and safety measures.

Stay current with best practices and regulatory requirements related to occupational therapy and adult day health services.

Qualifications
-
Education: Bachelor's or Master’s degree in Occupational Therapy from an accredited program.

Licensure/Certification:
-
Current California Occupational Therapy License.

Current CPR/First Aid certification.

Experience:
-
Minimum of one (1) year of clinical experience preferred, ideally with older adults or individuals with chronic illness, dementia, or disabilities.

Experience in a community-based or adult day health care setting preferred.

Skills:

Strong assessment, planning, and documentation skills.

Ability to work collaboratively with an interdisciplinary team.

Excellent communication and interpersonal skills.

Knowledge of adaptive equipment and compensatory strategies to enhance independence.

Working Conditions

Schedule limited to 16–20 hours per week, Monday–Friday, during center operating hours (8:00 a.m.–4:00 p.m.).

Work performed in a community-based adult day health care environment.

Frequent standing, bending, lifting, and assistance with participant mobility may be required.
