**Join Our Dynamic Team as a Physical Therapist!**\n\n**Job Description:**\n\n- Are you a passionate and skilled Physical Therapist looking to make a real difference in the lives of others? \n- Join our interdisciplinary team in a vibrant Skilled Nursing Facility (SNF) where your expertise will shine! \n- Your role will include:\n\n - Performing thorough assessments of patients’ physical abilities, movement limitations, and pain levels.\n \n - Developing and implementing personalized treatment plans based on evaluation findings, medical history, and patient goals.\n \n - Delivering evidence-based physical therapy interventions to enhance strength, coordination, balance, and mobility.\n \n - Collaborating with physicians, nursing staff, occupational and speech therapists, and family members to deliver comprehensive patient care.\n \n - Monitoring patient progress, adjusting therapy plans as needed, and documenting all care provided in accordance with state and federal guidelines.\n \n - Educating patients and caregivers on safe mobility techniques, home exercises, and equipment use.\n \n - Ensuring compliance with all regulatory requirements, including Medicare/Medicaid documentation and facility policies.\n \n - Participating in care planning meetings and interdisciplinary team conferences.\n \n - Supervising physical therapist assistants (PTAs) and aides as necessary.\n \n - Maintaining a safe and clean work environment.\n\n**Qualifications:**\n\n- Graduate of an accredited Physical Therapy program.\n \n- Current state license to practice as a Physical Therapist (PT).\n \n- CPR certification (preferred or required depending on the facility).\n \n- Experience in a Skilled Nursing Facility or geriatric rehab setting preferred.\n \n- Excellent communication, documentation, and interpersonal skills.\n \n- Ability to work independently and collaboratively within a multidisciplinary team.\n\n**Work Environment:**\n\n- Skilled Nursing Facility (SNF) focused on inpatient, long-term care, and short-term rehabilitation.\n \n- Involves lifting, standing, walking, and assisting patients with transfers and mobility.\n\n**Shift Information:**\n\n- 9 AM to 5 PM\n\n**Minimum Years of Experience:**\n\n- Less than 1 year\n\n**Why Catalytic Solutions (CatSol):** \n\nAt CatSol, we connect dedicated therapists with rewarding assignments across the country.
