Position Overview:
As a Mobile Wound Care Nurse Practitioner, you will be responsible for providing expert wound care management in patients' homes and/or care facilities. You will assess, diagnose, treat, and educate patients with chronic and complex wounds, including but not limited to diabetic ulcers, pressure ulcers, surgical wounds, and other skin-related conditions. You will work collaboratively with physicians, nurses, and administrative leadership to ensure the best possible care for our patients.
Key Responsibilities:
- Conduct comprehensive assessments of patients with acute and chronic wounds, including diabetic ulcers, pressure ulcers, venous ulcers, and surgical wounds.
- Develop, implement, and evaluate individualized wound care treatment plans based on patient needs.
- Perform wound debridement, dressings, and other advanced modality wound care procedures.
- Collaborate with interdisciplinary teams, including physicians, nurses, physical therapists, and dietitians, to ensure optimal patient outcomes.
- Educate patients and their families on wound care, prevention, and self-management strategies.
- Monitor and document patient progress, including wound healing and response to treatment.
- Prescribe medications, such as antibiotics or topical treatments, as appropriate for wound management.
- Stay updated on the latest evidence-based practices and wound care technologies.
- Provide leadership and mentorship to nursing staff and other healthcare providers.
Qualifications:
- Master’s degree in Nursing (MSN) or Doctorate in Nursing Practice (DNP) from an accredited program.
- Active Nurse Practitioner (NP)/Physician Assistant (PA) license in KS and MO (preferred).
- Wound Care Certification (WCC) or board certification in wound care (preferred).
- Minimum of 2 years of clinical experience in wound care or a related field.
- Strong knowledge of wound healing, wound care products, and advanced wound therapies.
- Excellent interpersonal, communication, and organizational skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Strong critical thinking and problem-solving abilities.
- Current BLS (Basic Life Support) certification; ACLS (Advanced Cardiovascular Life Support) is a plus.
