Qualifications

Position Summary
Under the supervision of the Assistant Principal of Student Support, the Social Worker works directly with stakeholders, including students, families, MWA personnel, and external providers, to address student-related social and emotional challenges and behavioral health needs.  These challenges may derive from both school-based and/or external factors.  The Social Worker will work in various ways with internal and external stakeholders and is pivotal in connecting services that will support a successful learning experience.  The social worker will also lead the development of relationships and networks that allow MWA to provide and refer many services for MWA students and families. In addition to direct work with students and families, the Social Worker will play a role in compiling and sharing data with parents, faculty, and staff to further train and educate the MWA community on various non-cognitive developmental challenges, possible solutions, and overall progress that students are making in developing the adequate non-cognitive skills necessary to effectively navigate college and other post-secondary life skills.

Title: Social Worker (SW) 
Supervisor: Program Director
Summary of Job Description:
Provide social work and case management services for Members in collaboration with families and other stakeholders to ensure coordination of care.
Specific Responsibilities:
<ul>
<li>Collaborate with admissions to support the admission and acclimation of new Members into the program setting.</li>
<li>Complete Incident Reports in accordance with processes outlined by the Managed Care Organization (MCO), State and Project Transition Policy and Process.</li>
<li>Collaborate with members of the multidisciplinary team to develop treatment plans for identified Members.</li>
<li>Complete monthly individual sessions with Members and document in DAP format within 24 hours of the session.</li>
<li>Provide documented case management services to Members including (but not limited to):
<ol>
<li>Coordination of financial eligibility and benefits</li>
<li>Coordination of family contact and support</li>
<li>Conduct utilization and member care reviews with MCOs and other payers.</li>
<li>Track member authorizations and input into Electronic Health Record (her) timely.</li>
<li>Run authorization reports and other reports in the EHR as assigned.</li>

<li>Provide coverage for staff callouts.</li>

OBJECTIVE SUMMARY: Serves as a member of the health care team, assisting individuals/families and the treatment team through the provision of psychosocial assessments; advocacy; education; treatment and discharge planning for all patients admitted to the inpatient acute medicine and swing beds; patients admitted to the Emergency Department and through patient self-referral, social work, case-finding and/or referral from medical and nursing staff.  Social Workers will screen all patients to determine extent of social work services needed by each patient.
ESSENTIAL FUNCTIONS:

<li> Provides a psychosocial assessment of patient, family or appropriate support systems in order to implement crisis or support interventions and to maximize coordination of services</li>
<li> Conducts standardized mental health screenings for patients admitted to the Swing Bed Program</li>
<li> Initiates discharge planning with patients and their families to develop a plan that meets the patient’s needs in both short and long term planning. Examples:  coordinates residential placement, transportation, homecare, mental health follow-up</li>
<li> Provides counseling/education for patient, family, support systems regarding adjustment to illness and hospitalization, end of life issues, decision-making. Re: substance abuse, domestic violence, elder abuse, and child abuse</li>
<li> Resolution or identification of other problems related to the patient’s transition within the health care setting</li>
<li> Coordinates, negotiates and advocates for patient’s needs with payers and external case managers</li>
