POSITION SUMMARY 
\n
Under the direction of a Physical Therapist (PT), the PTA provides client services that have been delegated by the PT.

RESPONSIBILITIES
\n

<li>Assists in the evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.</li>

<li>Participates in the preparation of clinical and daily progress notes, all notes are reviewed by the PT and submitted according to company policy.</li>

<li>As directed, performs routine strengthening and therapeutic exercises and assist the patient with the use of self-help devices.</li>

<li>Together with the PT, instructs the client and family and assists in teaching exercise programs to the patient and family.</li>

<li>Participates with staff, clients, and physicians in discharge activities and assists the Physical Therapist in completing the Physical Therapy Discharge Summary.</li>

<li>Confirms, at least weekly, the scheduling of visits with the Supervisor/Director of Nursing in order to coordinate Agency visits.</li>

QUALIFICATIONS

Current Physical Therapist Assistant (PTA) license within the state of practice.

Physical Therapy Assistant Outpatient Full Time Days Position Summary
At the direction of a Registered Physical Therapist, performs patient treatments as indicated by the treatment plan. Documents treatments given, including frequent and objective measurements. Works with neonatal through geriatric patients with clinical competency. Completes established competencies for the position within the designated introductory period.
Responsibilities
Performs selected tests and measurements as directed by therapist; maintains documentation on all treatments; assures appropriate equipment performance prior to each use.
Education
Required: Associate's degree in Physical Therapist Assistant, Active U.S. military service members or veterans with any military service, training or education verified and credited by an accredited PTA program is acceptable
