As a Hospice Registered Nurse Case Managers (RN), here’s a big-picture view of what you’ll do:
<ul>
<li>Conduct comprehensive assessments of patients’ physical, emotional, psychosocial, and spiritual needs to develop individualized care plans.</li>
<li>Coordinate interdisciplinary care team meetings to ensure collaboration among healthcare professionals involved in the patient’s care, including physicians, social workers, chaplains, and volunteers.</li>
<li>Educate patients and their families about disease progression, symptom management, medication management, and end-of-life care options to empower them to make informed decisions.</li>
<li>Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes in compliance with regulatory requirements and organizational policies.</li>
<li>Provide compassionate end-of-life care, including symptom management, comfort measures, and support for patients and their families during the dying process.</li>
<li>Participate in quality improvement initiatives aimed at enhancing the quality of care provided to hospice patients and their families.</li>

A few must-haves for Hospice Registered Nurses Case Managers (RNs):

<li>Graduate of an accredited nursing program and active RN license in California</li>
<li>Minimum of 1 year of nursing experience in hospice</li>
<li>CPR certification</li>
<li>Knowledge of state and federal home health regulations</li>

We are looking for the right Nurse Practitioner to perform medical assessments on United States Veterans as part of the VA's disability benefits process. 
In performing Disability Assessments, you will review medical records electronically through a web portal and perform a focused, in-person physical exam on each Veteran.
You will electronically document the exam findings required by the VA through the web portal.
The Nurse Practitioner DOES NOT make disability determinations. The United States Veterans Administration makes all disability determinations.
This role ONLY performs Disability Assessments on Veterans. You will NOT treat, and you will NOT prescribe medication in this position.
This position performs Disability Assessments on 3-5 Veterans per day, on average.
Schedule Full-Time Monday-Friday 8am-4pm 
This is an IN-PERSON position.
