GENERAL STATEMENT OF DUTIES Facilitates intake and eligibility determination for applying individuals; conducts independent functional needs assessments; and plans and carries out case management functions and other related work as required. SUPERVISION RECEIVED Works under supervision of a Clinical Supervisor who assigns work, establishes goals, and reviews the results obtained for overall effectiveness.

Typical Duties - Duties include, but are not limited to the following
All services will be provided either in the office or in the field. Must be able to drive vehicle to and from community meetings and home visits. Must use county issued communication tools including smart phones and computers during work time.
Choice Counseling: Offer choice, annually and as needed, of all available services, provider and setting options.
Service authorization/Individual Support Planning: Facilitate development of Individual Support Plan for all individuals receiving K plan funded services in their own home, family home, or in a licensed or certified residential setting. Authorize services and support as described in Oregon Administrative Rule 411-415-0070. Complete enrollment, termination, and transfer on required forms.
Functional Needs Assessment: Facilitate or participate in an annual functional needs assessment process per current ODDS procedure. 
Plans and Annual Summaries: Complete annual summary for all individuals not enrolled in any K plan funded service (case management only). 
Coordination with school: Participate in service planning by attending Individual Educational Plan meetings for students. It is mandatory to attend IEP meetings for individuals aged 15 and older to complete transition planning.
Documentation: Maintain an accurate client record including Marion County generated documents and documents received from other individuals/agencies. Document all targeted case management activities in a progress note per Marion County policy.
Service Monitoring: Monitor services and support for individuals enrolled in case management as described in Oregon Administrative Rule 411-415-0090 and document per ODDS guidelines. Report Serious Events per published guidelines.
Information and Referral: provide information, timely referral, and assistance to individuals regarding available services to meet assessed needs.
Coordination with other agencies: Coordinate with community partners regarding provision of required support from Community Developmental Disabilities Program to ensure safety and support needs are adequately met.
Staff Meeting: Attend staff meeting, vacancy meeting, and team meetings.
Other job duties as assigned by your supervisor.

Clinical/Administrative Support

<li>Acts as primary support staff to process subpoenas requests, following Marion County Health and Human Services Department subpoena policy.</li>
<li>Oversees and facilitates confidential record request(s) per HIPAA requirements from outside agencies. Performs flow of record requests from receipt, logging, clinical/supervisor oversight and approval (if needed) to the photocopying and preparing confidential record request mailing/invoicing.</li>
<li>Recalls records from archiving. </li>
<li>Uses problem-solving methods to make system improvements and support clinical staff with caseload management.</li>
<li>Runs report to monitor intake enrollments for correct insurance data weekly. Initial billing information has already been faxed by Centralized Intake (CI) staff to the Billing Team for set up of coverage screens. May double check insurance information in the Medicaid Management Information System (MMIS).</li>

<li>Verifies release of information (ROIs) forms and updates information in electronic health record (EHR), as needed.</li>

Fiscal Duties/Payroll
<ul>
<li>Oversees petty cash and related records.</li>
<li>Receives cash/checks for the department, reconciles and submits bank deposits per cash control policy.</li>
<li>Reconciles receipts/log of payments and deposits per cash control policy.</li>
<li>Processes, audits and submits bi-monthly payroll for ABH. (Includes proofing for errors, facilitating corrections on initial timecard, obtaining supervisor signatures, submission to administration. <li>Distributes paychecks/deposit vouchers and maintains confidentiality.</li>

Purchasing Agent/Accounts Payable and Receivable
<ul>
<li>Orders primary office supplies from Office Depot using designated purchasing card using the on-line ordering system. Tracks and maintains needed office supplies. Enters purchase information into Works, submits summary monthly and checks for completeness and supervisor's signature on order request (or approval/authorization form) and monthly report. Submits to direct supervisor for their sign off in Works.</li>
<li>Process requests and reimbursement requests for staff training using Pcard.</li>
<li>Completes purchase requisition form for other department supply orders and submits to accounts payable.</li>
<li>Orders printing orders, business cards, forms, books/magazines/education materials, as requested. Assures that items are priced correctly, charged to the proper cost center/account number, then routed to administration for payment. Signs off and submits bills for payment.</li>
<li>Discusses with clinical/program supervisor any problems with any of the payables.</li>
<li>Serves as contacts for P-cards at Silverton Rd Ste B. Signs in and out P-cards to staff and supervisors.</li>
</ul>
Other Duties
<ul>
<li>Provides secretarial support function for program manager, program supervisor, senior office manager, and office manager.</li>
<li>Runs monthly reports from Electronic Health Record (EHR) and distributes electronically to supervisors and staff as requested for all Adult Behavioral Health Programs/Stepping Stones/Horizon House. Runs reports/queries (i.e. open encounter, annual review and caseload reports) to ensure accuracy and proper billing of data entered by clinical staff in BH service areas of ABH and Stepping Stone/Horizon House.</li>
<li>Performs data entry.</li>
<li>Assists with front desk back up and reminder calls, as needed.</li>
<li>Provides reception and medical records assistance.</li>
<li>Track and maintain equipment list for ABH.</li>
<li>Primary contact that facilitates scheduling of Pre-Admission Screening Resident Reviews (PASRRs) between nursing/medical facilities and evaluators. Facilitates scheduling of PASRRs between nursing/medical facilities and evaluators. </li>
<li>Submits PASRR reports to Office of Mental Health and Addictions Services (OMHAS) according to Oregon Administrative Rules (OARs).</li>
<li>Performs other job duties as assigned by supervisor.</li>

GENERAL STATEMENT OF DUTIES Mental Health Therapist 2: Provide assessments and treatment of individuals and families, including groups; provides crisis assessment and intervention services and related duties as required. <li>Oversee and coordinate daily functioning and activity programs with residents on the unit including outings with Qualified Mental Health Associates (QMHA) and PBNC staff. Facilitate daily huddles with QMHAs and Nursing Staff to identify priorities.</li>

<li>Coordinate discharge planning with NorthWest Seniors and Disability Services (NWSDS) Case Manager (CM) and Providence Benedictine Nursing Care staff. Identify MH needs to be addressed at future placements; relay MH, Neurocognitive, and Physical Health needs to NWSDS CM. (Physical Health and Neurocognitive needs will be identified by PBNC and other appropriate entities.)</li>
<li>Assist in screening residents for discharge. Explore placement options identified by NWSDS CM and review with treatment team, resident, and legal representative.</li>
<li>For all of the above, complete clinical documentation professionally and accurately in a timely manner as directed by Marion County HHS policy. Submit paperwork to OHA/HSD or other entities as required. Maintain accurate and comprehensive MH charts. If necessary, transports charts following HIPAA regulations to support staff at Marion County ABH.</li>
<li>Submit Monthly Census to OHA/DHS.</li>

EXPERIENCE AND TRAINING

Provides service coordination to assist families in accessing formal and informal supports and services; facilitating communication between the family, natural supports, community resources, and involved child-serving providers and agencies; participates in team meetings at which strengths and needs are identified and safety planning occurs; and provides for continuity of care by creating linkages to, and managing transitions between levels of care, and transitions of older youth to the adult service system.</li>

Expectation is that employee will strive for 60% of work hours on average to be billable hours with a minimum of 50% billable hours on average each month.</li>

KNOWLEDGE, SKILLS AND ABILITIES Working knowledge of community mental health concepts and/or chemical dependency case management and treatment services; individual assessment, treatment planning and treatment protocols; mental health disorders and diagnostic classifications.
Ability to establish positive working relationships with other employees, individuals and their families, other agencies, and the public; prepare concise and complete clinical documentation related to individual treatment and progress in services in a timely and professional manner; work within a team and collaborate with team members; knowledge of and ability to act professionally with regard to a code of ethics and confidentiality. 
PHYSICAL REQUIREMENTS Operates a motor vehicle; sees using depth perception; sits; stands; moves about the work area; climbs 1 floor of stairs; lifts, pushes, pulls, and carries 5 lbs.; moves carts weighing up to 5 lbs.; operates a keyboard; speaks with a clear and audible voice; reads a 12-pt. font; hears a normal speech level.

We offer the following retention bonus, prorated based on FTE:

Marion County offers a generous benefits package to regular employees working 50% or more of the regularly scheduled work week and their eligible dependents. Temporary employees and part-time regular employees working less than 50% of the regularly scheduled work week are not eligible for benefits.
