The Custom Applicator is responsible for correctly customizing and applying dry fertilizers to our customer's fields and operating all applicator equipment including performing maintenance on the equipment.

Seasonal

Education and experience needed for this position

The nurse in this position provides Tuberculosis Case Management and Public Health Clinic Services. Demonstrates flexibility in adapting as program and/or service needs change.
Tuberculosis (TB) Program Services Works to prevent and control the spread of tuberculosis. Related duties include but are not limited to:

<li>Participates in TB surveillance and service activities including, but not limited to reportable TB and outbreak response.</li>
<li>Participates in weekly case review and documents summary of case review to maintain a basic knowledge of all clients on caseload.</li>

<li>Provision of current and appropriate medication information verbally and in writing.</li>
<li>Initiate a plan with client for follow up and treatment.</li>
<li>Triage walk-in clients and those phoning in with questions or requests for exams.</li>

<li>Reviews lab results and processes appropriately.</li>
<li>Phone in prescriptions as ordered by the Health Officer.</li>

Infection Prevention Services to MCHHS employees:

<li>Conduct Immunity Assessment, TB screening, and provide immunizations to employees per policy.</li>
<li>Draw blood for post vaccine titer per policy.</li>
<li>Provide post occupational exposure follow-up for BBP and other communicable diseases as appropriate.</li>
<li>Provide timely, accurate &amp; complete documentation of services in confidential digital &amp; paper files.</li>
<li>Maintain PPD, fit test solution and vaccine medication inventory for employees. </li>
<li>Ensure OSHA requirements for HBV vaccine are met in concert with Immunization Coordinator.</li>
<li>Ensure employees who fail to comply with needed services are referred to their supervisor and Clinic supervisor within 1 month of being overdue in concert with Immunization Coordinator. </li>
</ul>
Share in General Program Duties with the Clinical Preventive Services (CPS) Team: Participates and/or provides leadership to a variety of activities not limited to the following:

<li>Develop and/or contribute to Team meeting agenda, policies and procedures and other documents.</li>

EXPERIENCE AND TRAINING As defined by OAR 309-019-0105 (81) "Peer Support Specialist" means an individual providing peer delivered services to an individual or family member with similar life experience under the supervision of a qualified clinical supervisor and a qualified peer delivered services supervisor as resources are made available.

Peer Support Specialist - Adult Outpatient Mental Health Services

The Psychiatric Crisis Center has been working to expand by offering bilingual/bicultural services at our Woodburn office. The Mental Health Specialist in this position will provide crisis assessments to individuals in need as well as case management, resource identification, and referral options. This position will require a motivated individual to learn about the available community services to provide mental health well-being. The individual in this position will possess a desire to be creative in matching individuals to available services while educating the community about the various services we provide at the Crisis Center. This position provides services to monolingual Spanish-speaking individuals; therefore, fluent bilingual Spanish/English skills are required.

GENERAL STATEMENT OF DUTIES Crisis Clinician 2: Provide assessments and treatment of individuals and families, including groups; provides crisis assessment and intervention services and related duties as required. Primary responsibility of this class is to provide assessment and treatment services; crisis intervention, and treatment services, including intensive services, to individuals and their families with complex needs and diagnoses and/or to develop and implement specific services in the community.
Crisis Clinician 1: Plans and carries out recreational, social and skills training activities; performs case management and counseling functions with Adults in Custody (AIC); facilitates counseling and/or skills training groups; performs related duties as required. Primary responsibility of this class is case management and counseling, without responsibility for developing and implementing specific programs in the community.
SUPERVISION RECEIVED Crisis Clinician 2 and 1: Works under the supervision of a supervisor who assigns work, establishes goals, and reviews the results obtained for overall effectiveness.
SUPERVISION EXERCISED Crisis Clinician 2 and 1: Supervision of others is not a responsibility of positions in this classification. Crisis Clinician 2 may act as Clinical Supervisor designee when needed. May assist in the supervision of practicum students and/or volunteers.
Typical Duties - Duties include, but are not limited to the following
Counseling to Crisis Clients: Provide counseling by telephone or in-person to individuals and/or their families or concerned others. Intervene with both children and adults. Presenting problems covering the full range of personal needs and include thoughts/plans or violence toward others, and major psychiatric illness. Because of the wide spectrum of presenting problems, it is necessary to have a comprehensive knowledge of psychopathology and to be able to draw upon a wide range of therapeutic strategies in counseling. This work is done at two different locations each day (at the Salem Hospital Emergency Dept. and at PCC), so driving is required.
Assessment of Mental Status: Perform independent assessment of mental status, diagnosis, level of functioning, dangerousness, and specific needs for service. Screen individual for psychiatric hospitalization.
Planning Development: Develop plan with client/family to address immediate needs and plans to obtain services. If client is unwilling to seek services, make decisions regarding involuntary commitment based on dangerousness and ability to care for self.
Community Coordination: Maintain current knowledge of, and contact with, community agencies in order to make referrals appropriately. See that clients gain access to the services they need, and do follow-up work as needed, to make certain that client's needs are being met.

<li>Advocacy for Client's Rights: Provide advocacy for client rights, ethical consideration, needs for service and least restrictive care-intervention.</li>

The Custom Applicator is responsible for correctly customizing and applying dry fertilizers to our customer's fields and operating all applicator equipment including performing maintenance on the equipment.
