Marion County Health and Human Services is looking to add an organized, motivated, person-centered, and flexible Service Coordinator to our Intellectual and Developmental Disabilities (I/DD) team. This position will provide case management services to children and/or adults who experience Intellectual or Developmental Disabilities. Service Coordinators work collaboratively with individuals, family members, service providers and community partners to coordinate disability related services in all Home and Community-Based settings.

GENERAL STATEMENT OF DUTIES Facilitates intake and eligibility determination for applying individuals; conducts independent functional needs assessments; and plans and carries out case management functions and other related work as required. Primary responsibility of this class is case management, facilitating intake and eligibility determination and administration of functional needs assessments.
SUPERVISION RECEIVED Works under supervision of a Clinical Supervisor who assigns work, establishes goals, and reviews the results obtained for overall effectiveness.

Typical Duties - Duties include, but are not limited to the following
All services will be provided either in the office or in the field. Must be able to drive vehicle to and from community meetings and home visits. Must use county issued communication tools including smart phones and computers during work time.
Choice Counseling: Offer choice, annually and as needed, of all available services, provider and setting options.
Service authorization/Individual Support Planning: Facilitate development of Individual Support Plan for all individuals receiving K plan funded services in their own home, family home, or in a licensed or certified residential setting. Authorize services and support as described in Oregon Administrative Rule 411-415-0070. Complete enrollment, termination, and transfer on required forms.
Functional Needs Assessment: Facilitate or participate in an annual functional needs assessment process per current ODDS procedure. 
Plans and Annual Summaries: Complete annual summary for all individuals not enrolled in any K plan funded service (case management only). 
Coordination with school: Participate in service planning by attending Individual Educational Plan meetings for students. It is mandatory to attend IEP meetings for individuals aged 15 and older to complete transition planning.
Documentation: Maintain an accurate client record including Marion County generated documents and documents received from other individuals/agencies. Document all targeted case management activities in a progress note per Marion County policy.
Service Monitoring: Monitor services and support for individuals enrolled in case management as described in Oregon Administrative Rule 411-415-0090 and document per ODDS guidelines. Report Serious Events per published guidelines.
Information and Referral: provide information, timely referral, and assistance to individuals regarding available services to meet assessed needs.
Coordination with other agencies: Coordinate with community partners regarding provision of required support from Community Developmental Disabilities Program to ensure safety and support needs are adequately met.
Staff Meeting: Attend staff meeting, vacancy meeting, and team meetings.
Other job duties as assigned by your supervisor.
Response Level 2: In the event of an emergency, employee makes a reasonable effort to report to work. Employee may be required to perform duties outside of their regularly assigned duties. Employee may be requested to work an extended or flexible schedule. Staff at this response level must complete Incident Command System 100 and 700 on-line or in-person training.

EXPERIENCE AND TRAINING
<li>Five years of education in a related field and/or paraprofessional experience/training consisting of serving individuals with a primary diagnosis of developmental disabilities within a residential, employment or case management service at a management or case management level; AND</li>

Position Summary:
Administrative supportive services are an integral part of the overall operation of the SUNY North Bronx Career Counseling &amp; Outreach Center's (NBCCOC) mission, goals, and objectives. The Administrative Support Specialist performs all crucial administrative support services which includes intake, screening, follow-up for applicants as well as reviewing/validating financial income documents and academic guideline requirements for program eligibility. They will be responsible for the timely data entry of student applications into the Banner system to reflect the Center's progress towards meeting annual target goals.
Responsibilities:
<ul>
<li>Responsible for Center administrative support: answering/screening phone calls and routing/assigning them to the appropriate staff member for follow up, correspondence, reporting, mailings, and document management.</li>
<li>Assist with student applicant data collection for NBCCOC program students and the Advanced Technology Training And Information Networking (ATTAIN) Lab participants.</li>
<li>Responsible for data entry into the Banner system: contact information, demographics, eligibility documentation, programs of interest, test scores, etc.</li>
<li>Assist with staff calendar appointments and scheduling student TABE Assessment Exam appointments.</li>

The Master directs the ferry's activities and operates the vessel safely and efficiently while supervising onboard employees. This position serves as the primary contact onboard the vessel with the USCG concerning compliance with laws and regulations. The Master administers training and drills aboard the vessel, and this position ensures a high level of customer service is provided.
Essential Duties &amp; Responsibilities:

<li>Oversee and coordinate daily functioning and activity programs with residents on the unit including outings with Qualified Mental Health Associates (QMHA) and PBNC staff. Facilitate daily huddles with QMHAs and Nursing Staff to identify priorities.</li>

<li>Determine, with consultation with Supervisor and Prescriber (Psychiatric Mental Health Nurse Practitioner (PMHNP) or Psychiatrist), appropriateness of continued enrollment in ECS.</li>
<li>Coordinate discharge planning with NorthWest Seniors and Disability Services (NWSDS) Case Manager (CM) and Providence Benedictine Nursing Care staff. Identify MH needs to be addressed at future placements; relay MH, Neurocognitive, and Physical Health needs to NWSDS CM. (Physical Health and Neurocognitive needs will be identified by PBNC and other appropriate entities.)</li>
<li>Assist in screening residents for discharge. Explore placement options identified by NWSDS CM and review with treatment team, resident, and legal representative.</li>
<li>For all of the above, complete clinical documentation professionally and accurately in a timely manner as directed by Marion County HHS policy. Submit paperwork to OHA/HSD or other entities as required. Maintain accurate and comprehensive MH charts. If necessary, transports charts following HIPAA regulations to support staff at Marion County ABH.</li>
<li>Submit Monthly Census to OHA/DHS.</li>

Enhanced Care Outreach Services
<ul>

Job Summary: The Training Supervisor is responsible for the initial and ongoing training of all new hires and current staff. The Berkeley Institute for Data Science (BIDS - <a href="https://apptrkr.com/get_redirect.php?id=5184681&amp;targetURL=https://bids.berkeley.edu" target="_blank">https://bids.berkeley.edu</a>) was founded in 2013 to build on Berkeley's strength in data science with a multidisciplinary emphasis. Marion County Adult Behavioral Health hiring four mental health therapists for their expanding Assertive Community Treatment (ACT) team. In this position, you would provide mental health therapy to individuals diagnosed with serious and persistent mental illnesses (SPMI). You would get to work closely with other providers on the adult outpatient team, including other therapists, skills trainers, case managers, peer support specialists, prescribers, and supported employment and education specialists. GENERAL STATEMENT OF DUTIES 
Provide assessments and treatment of individuals and families, including groups; provide crisis assessment and intervention services and related duties as required. The primary responsibility of this class is to provide assessment and treatment services, crisis intervention, and treatment services, including intensive services, to individuals and their families with complex needs and diagnoses or to develop and implement specific services in the community.

Typical Duties - Duties include, but are not limited to the following
Provide behavioral health services to individuals with serious and persistent mental health conditions and co-occurring substance use disorders who are enrolled in Assertive Community Treatment (ACT), with the majority of services provided in the community (i.e., individual's homes, hospitals, jails, licensed care, shelters, etc). Services include:

<li>Provide intake and annual comprehensive mental health assessments, including assessing mental health and/or behavioral symptoms using a variety of standard assessment tools, individual functional skills, determining treatment needs, developing individualized treatment plans, directing provision of treatment services, and complete other related required paperwork per Office of Mental Health and Addiction Services Oregon Administrative Rules.</li>
<li>Provide individual and group therapy services to individuals using a variety of evidence-based practices as clinically indicated, which may include Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Collaborative Assessment and Management of Suicidality (CAMS), Collaborative Problem-Solving (CPI), and Motivational Interviewing. </li>
<li>Provide case management to include determining individual service needs, eligibility, and appropriateness of services; making referrals and coordinating with other agencies to obtain services and resources for individuals; and advocacy and brokerage on behalf of client to access community resources, benefits, and entitlements. </li>
<li>Provide crisis assessment and specific intervention services, which may include development of safety/crisis plans and suicide and self-harm risk assessments with supervisor oversight. </li>
<li>Provide consultation to family members and other responsible parties involved in individuals' care and well-being. </li>
<li>Work on development of individuals' natural supports.</li>
<li>May provide skills training focused on daily living and community integration. </li>
<li>Employee must be able to drive county vehicles. </li>
<li>Participate in after-hours on call rotation to provide coverage to support to individuals in crisis and to provide consultation and coordination with other agencies and providers.</li>
<li>Maintain accurate and timely clinical documentation in accordance with Federal, State, County, and treatment program requirements under the direction of the clinical supervisor. Meet performance expectations for time and billing standards.</li>
<li>Participate fully as a member of the Marion County Assertive Community Treatment (ACT) team as a supportive, reliable, positive, collaborative, and professional team member during the development of individual treatment planning, daily clinical team meetings, and client crisis intervention. Work with a team of clinicians and supervisor for clinical supervision, treatment planning, staff and departmental meetings, and trainings.</li>

Requirements for the Position
EXPERIENCE AND TRAINING
<li>Master's degree in psychology, social work, recreational therapy, music therapy, art therapy or a behavioral science field; OR</li>
<li>Bachelor's degree in nursing, or occupational therapy and licensed by the State of Oregon; AND</li>
<li>One year of postgraduate degree experience in the behavioral health field.</li>

GENERAL STATEMENT OF DUTIES Provide assessment and treatment services; crisis intervention, including intensive services, to individuals and their families with complex needs and diagnoses; or develop and implement specific services in the community. Act as a task supervisor for interns and staff, provide clinical supervision to mental health professionals, to perform various advanced, specialized clinical and treatment services for individuals receiving services and their families. 
SUPERVISION RECEIVED Work under the general supervision of a manager or supervisor, who assigns work, establishes goals, and reviews the results for overall effectiveness. Per Oregon Health Authority, Behavioral Health Services Rule 309-019-0130: Program staff providing direct services shall receive clinical supervision by a qualified clinical supervisor related to the development, implementation, and outcome of services, documentation of two hours per month of supervision for each individual supervised.
SUPERVISION EXERCISED Primary supervision of employees is not a responsibility of this classification; however, positions may act as lead workers, as defined in the Marion County Personnel Rules. May assist in the supervision of interns and volunteers.
Typical Duties - Duties include, but are not limited to the following

<li>Provides individual and family therapy to children, adolescents and Transition age youth in the Woodburn and surrounding communities. </li>
<li>Provides intake mental health assessments including screening for A/D issues.</li>
<li>Documents services to meet contract, administrative rule and billing requirements.</li>
<li>Expectation is that employee will strive for 30 hours per week to be scheduled with therapy, client support, committee work and approved training. </li>
<li>Provides consultation, training and orientation regarding mental health and co-occurring disorders treatment to team members, student interns and other professionals.</li>
<li>Provides case management services to clients. </li>
<li>Assists clients and families with accessing community supports/resources. </li>
<li>Assists with making referrals for services. </li>
<li>Other job duties as assigned by your supervisor.</li>
</ul>
Response Level 2: In the event of an emergency, employee makes a reasonable effort to report to work. Employee may be required to perform duties outside of their regularly assigned duties. Employee may be requested to work an extended or flexible schedule. Staff at this response level must complete Incident Command System 100 and 700 on-line or in-person training.
Requirements for the Position
EXPERIENCE AND TRAINING
<li>State of Oregon licensure, Licensed Clinical Social Worker (LCSW), Professional Counselor (LPC) or Marriage and Family Therapists (LMFT); ANDMaster's degree in psychology, social work, recreational therapy, music therapy, art therapy, or a behavioral science field; OR</li>
<li>State of Oregon licensure, Licensed Clinical Social Worker (LCSW), Professional Counselor (LPC) OR Marriage and Family Therapists (LMFT); ANDBachelor's degree in nursing or occupational therapy and licensed by the State of Oregon; ANDOne year of postgraduate degree experience in the behavioral health field.</li>
SPECIAL REQUIREMENTS

<li>The finalist for this position will be required to pass a criminal history background check, including finger printing; however, conviction of a crime may not necessarily disqualify an individual for this position. </li>
<li>Must possess a current driver's license in the applicant's state of residence and an acceptable driving history.
