Medical Scribes are individuals trained in medical documentation who assist a provider throughout their shift. The primary goal is to increase the efficiency and productivity of the physician. It will be the responsibility of the Medical Scribe to annotate any dictated or written information for the treatment of patients following all local, state and federal guidelines for documentation into the electronic health record system. The Medical Scribe will be responsible for clearly recording each patient's symptoms, history, physical exam and documenting the diagnosis. This position will have no direct involvement in patient care but has an interdepartmental role in assisting nursing and front office staff.
JOB DUTIES AND RESPONSIBILITIES
<ul>
<li>The primary role of the Medical Scribe is to assist the clinician with documentation of each patient’s medical chart during his or her visit.</li>
<li>Accompanies the clinician into the patient examination area to transcribe a history and physical examination as given by the patient and physician.</li>

<li>Completes the patient’s chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data in order to get the patient’s workup complete so that the clinician is able to make his or her decisions regarding that patient.</li>
<li>Orders in-house testing, Immunizations, lab testing, diagnostic testing along with generating referrals.</li>
<li>Lists all proper diagnoses as well as any follow up instructions and prescriptions, as dictated by the clinician.</li>
<li>Other duties as assigned.</li>

JOB DUTIES AND RESPONSIBILITIES:

The front desk clerk will answer the M&amp;FRC telephone in a professional/timely manner

Ensure all customers utilize sign in log to document usage of Resource Center, other services and inputs data in AFFIRST.
If needed, assist in preparation of classroom for M&amp;FRC classes weekly.
Ensure copier room is neat, orderly and supplies are readily available at least once a week.
Input and track statistical data using the AFFIRST data tracking system daily (Quick Tracker).
Review M&amp;FRC web page and office calendar during the first week of each month for information accuracy.

Base Salary: $ 21.00- $31.50/hourJOB SUMMARYMedical Scribes are individuals trained in medical documentation who assist a provider throughout their shift. The primary goal is to increase the efficiency and productivity of the physician. It will be the responsibility of the Medical Scribe to annotate any dictated or written information for the treatment of patients following all local, state and federal guidelines for documentation into the electronic health record system. The Medical Scribe will be responsible for clearly recording each patient's symptoms, history, physical exam and documenting the diagnosis. This position will have no direct involvement in patient care but has an interdepartmental role in assisting nursing and front office staff.JOB DUTIES AND RESPONSIBILITIES
<ul>
<li>The primary role of the Medical Scribe is to assist the clinician with documentation of each patient’s medical chart during his or her visit.</li>
<li>Accompanies the clinician into the patient examination area in order to transcribe a history and physical examination as given by the patient and physician.</li>
<li>Documents any procedures performed by the clinician or nurses, on the electronic medical chart.</li>
<li>Is an expert in the use of various templates and documentation techniques available in the EHR system, and use these tools efficiently so individual encounters can be well documented, as well as accurately congregate data collected for statistical reports.</li>
<li>Transcribes any consultations or discussions with family members and/or the clinician.</li>
<li>Completes the patient’s chart by transcribing results of any labs, x-rays, or other evaluations. In doing so, the scribe continuously checks on the progress of this data in order to get the patient’s workup complete so that the clinician is able to make his or her decisions regarding that patient.</li>
<li>Lists all proper diagnoses as well as any follow up instructions and prescriptions, as dictated by the clinician.</li>

<li>A Scribe can be trained to perform vitals, room patients, handle Patient Portal tasks for your assigned provider, chart abstraction, and assisting in other departments such as HIS, or Radiology.</li>

KNOWLEDGE, SKILLS, AND ABILITIES
<ul>
<li>Recognition of physical exam process and ability to record exam details quickly and accurately.</li>
<li>Must be able to listen to complex medical information and summarize in a clear, complete and concise fashion.</li>
<li>Strong attention to detail.</li>
<li>Computer proficiency in Windows environment and ability to quickly learn new applications.</li>
<li>Ability to communicate effectively in the English language in person, by phone and in writing.</li>
<li>Typing 45 WPM or greater.</li>

EDUCATION &amp; EXPERIENCE
<ul>
<li>High school diploma or equivalent.</li>
<li>One year experience in healthcare field or minimum 2 years of college education.</li>

PREFERRED Qualifications

<li>2-4 years of college education desired with Medical Terminology, Anatomy and Physiology coursework/degree.</li>
<li>Experience working in an outpatient clinic or doctor’s office.</li>
<li>Medical Transcriptionist experience.</li>

Position Objective
The Medical Scribe is responsible for ensuring an accurate and complete medical record for each patient. The Medical Scribe assists the healthcare provider by accompanying them during the medical history-taking and exam, recording details of both into the patient’s medical record. 
Essential Responsibilities
 
EMR Documentation &amp; Utilization 
<ul>
<li>Accurately and thoroughly document medical visits and procedures, including but not limited to: </li>
</ul>
Patient Medical history and physical exams, procedures, treatments, diagnoses, prescriptions and instructions for patient or family members for self-care and follow-up
<ul>
<li>Identify discrepancies or inconsistencies in medical documentation and check to correct the information in order to reduce errors
<ol>
<li>All addenda must be signed off by a healthcare professional</li>
<li>Ensure that clinical data, lab and/or other test results, including the interpretation of the results by the healthcare professional are recorded accurately in the medical record</li>
<li>Alert healthcare professional when chart is incomplete </li>
</ol>
</li>
<li>Proofread and edit all the healthcare professional’s medical documents for accuracy, spelling, punctuation, and grammar</li>
</ul>
Compliance
<ul>
<li>Comply with specific standards that apply to the style of medical records and to the legal and ethical requirements for preparing medical documents and keeping patient information confidential </li>
<li>Collect, organize, and catalog data for preparing professional quality
<ol>
<li>Assist in developing and maintaining systems to track patient follow up and compliance </li>
</ol>
</li>
<li>Attend trainings on diverse subjects such as information technology, legal and regulatory compliance, billing and coding</li>

Required Qualifications
<ul>
<li>High School Diploma or equivalent </li>
<li>Medical Scribe, transcriptionist or related healthcare experience</li>

<li>This role is well-suited for a student currently pursuing a degree in Radiography.</li>

Duties and Key Responsibilities
<ul>
<li>Works under the supervision of the Radiologist, Radiologic Technologist and Radiology Transcriptionist.</li>
<li>Operates ionizing radiation-generating equipment in the performance of standard radiography exams under the scope of the GXMO license. </li>
<li>Assists the licensed Radiologic Technologist in performing radiological/ancillary exams.</li>
<li>Performs clerical duties, which may include but is not limited to, patient reception, scheduling and screening. Data entry, filing answering telephones taking and delivering messages.</li>
<li>Transports patients safely to and from Medical Imaging and other work areas. Utilizing appropriate mode of transportation for each patient and using assistive devices as necessary such as gait belt, smooth mover, hover jack and Hoyer lift.</li>
<li>Screens MRI patients and ensures patients and other safety in the MRI environment.</li>
<li>Respects patients’ rights to privacy, dignity and confidentiality.</li>
<li>Maintains established hospital and work area policies, procedures, objectives, quality assessment, guest relations, safety, environmental, and infection control. </li>
<li>Communicates appropriately with interdisciplinary team and other customers.</li>

Requirements

Education/Certifications

<li>Licensed by the State of Ohio in the category of General X-Ray Machine Operator (GXMO)</li>
