<li>Certified Dietary Manager (CDM) course participant, or certification as required by State &amp; County law.</li>
<li>Must successfully achieve the eLuminate Food Service Manager (FSM) and Long-Term Care Food Service (LTC-FSM) certifications within the first 60 days of employment.</li>
<li>Two years of experience in quantity food production/service, dining/nutrition, and personnel supervision experience are desired.</li>
<li>Basic computer skills with the ability to maintain records and complete reports as required, including web-based reporting.</li>

Nexus-PATH is located across North Dakota and offers mental health services, outpatient clinical services, and treatment foster care.
 
We have an opportunity to hire a full-time Case Manager (Treatment &amp; Foster Care) in our Bismarck, ND Nexus-PATH office! 
 
Our Case Manager recruits, orients, and supervises the work of Nexus-PATH resource families while providing intake services and comprehensive case management for children with special needs and families.
 
RESPONSIBILITIES:

<li>Recruit, license, train, supervise, and provide ongoing assessment to resource families to ensure quality care consistent with regulatory agencies (i.e. DHS, COA, FFTA).</li>

<li>Supervise resource families’ interaction with foster child, child’s family, treatment team members and collateral contacts.</li>
<li>Monitor and evaluate resource families’ activities to ensure compliance with agency and state standards. Submit critical incident reports and develop and implement corrective action plans as required.</li>
<li>Facilitate required team/unit meetings and participate in other resource families related activities.</li>
<li>Provide coordination of all tasks required to meet the treatment needs and best interests of service recipients</li>
<li>Complete a systemic assessment of the needs of the child and family.</li>
<li>Establish service parameters with legal custodian as child’s needs dictate.</li>
<li>Provide complete and required documentation, within established time lines relating to case management and treatment planning.</li>
<li>Implement treatment services to meet the needs and goals of the children and families utilizing quality case management practice to provide for the best interest of the child and family.</li>
<li>Provide direction to other team members when applicable and make recommendations for discharge, finalization and permanency.</li>
<li>Advocate for the development and maintenance of funding and resources to meet youth and family needs at local, state and national levels.</li>
<li>Develop and maintain community contacts to promote Nexus-PATH and collaborative efforts.</li>
<li>Provide crisis intervention as needed to maintain safety and provide for the best interest of the child and foster family.</li>
<li>Integrates trauma informed care principles into practice by sharing knowledge and understanding of trauma with the child, foster family and relevant team members. </li>
<li>Routinely screens for trauma exposure and related symptoms and demonstrates competence in using screening tool results to enhance service provision.</li>
<li>Routinely makes resources available and shares knowledge with foster families and providers on trauma exposure, its impact, and trauma-focused treatment.</li>
<li>Engages in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma.</li>
<li>Emphasizes continuity of care and collaboration across child-service systems.</li>
<li>Recognize and value cultural differences in all aspects of work and service delivery</li>
<li>Understand the potential for secondary traumatic stress and practices self-care strategies to mitigate these effects.</li>
<li>Meet standards for productivity and caseload size while maintaining appropriate documentation and files</li>
</ul>
QUALIFICATIONS:
<ul>
<li>Bachelor’s degree in Social Work preferred, Bachelors in Human Service Degree required </li>
<li>Current ND social work licensure required (LBSW) or ability to gain one within 3 months</li>
<li>Experience in foster care, adoptions and/or family-based service preferred</li>
<li>Valid driver’s license required. Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards </li>
<li>Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges. </li>
<li>Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.</li>
