 We are currently seeking a Full Time RN Case Manager for the Bridgeport through East Haven Areas Requirements: 
<ul>
<li> Minimum 1 year Home Care Experience</li>
<li> Comprehensive knowledge and skill in current nursing practice.</li>
<li> Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.</li>
</ul>
 Knowledge of the OASIS process. 
<ul>
<li>Knowledge of Community Resources.</li>
<li>Ability to make meaningful observations, evaluations and reports.</li>
<li>Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.</li>
<li>Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.</li>
<li>Ability to read, write and speak the English language in an understandable manner.</li>
<li>Ability to bend, push, pull, move and/or lift</li>
<li>Must possess a Nursing degree from an accredited College or University and be a Graduate of a CT approved RN program.</li>
<li>Valid driver's license/Car insurance</li>
<li>Active RN License in the State of CT</li>
<li>Proof of Covid 19 Vaccination</li>
