Primary Role: 
Community Health Workers use their unique position as trained community members to provide trust-based information and focused education, help clients (patients) cope with and overcome fear, walk clients through the often-complex healthcare system, and help them strategize to manage the logistics of caring for oneself in the context of a complicated life.
 
CHWs are defined as role model peers who share the same experiences and community membership and who are trained to provide effective services that increase the retention, and medication adherence/viral suppression of the people they serve. In fact, reminder phone calls, phone check-ins to ascertain reasons for missing an appointment, and appointment accompaniment (e.g. received a letter of lapsed health insurance coverage, for medical care, legal services, social services, and Supplemental Security Income (SSI)-related appointments) are among the most common CHW tasks. Peers can also be used to perform community outreach and home visits—something that may not be possible for case managers, LCCs or front desk staff due to caseloads and required office-based tasks.
 
Essential Job Duties: 
The following are primary roles and responsibilities of the Community Health Workers:

<li>CHW model integrated as part of the multidisciplinary medical team;</li>
<li>Identify individuals who know they are HIV-positive but are not in HIV medical care by receiving referrals, utilizing organizational information about missed appointments. Case finding and community outreach to bring out-of-care clients or clients who have never access care to services.</li>
<li>Work to build the trust and confidence of clients;</li>
<li>Provide accurate information to clients about living with HIV, preventing further transmission, the benefits and challenges of HIV care and treatment, and available services and how to access them;</li>
<li>Provide personalized assistance to clients to help them enroll or re-engage in medical care. Accompanying clients to medical and social service appointments, coordinating and assisting with transportation; referrals and follow-up with mental health providers; treatment adherence education and support; educating and assisting clients in knowing what materials to bring or questions to facilitate access to needed services, such as obtaining an identification (ID) card for a housing application, health insurance/health literacy education or application for other social support services.</li>
<li>Together with case managers and clinical staff, support clients throughout the early part of their care so they participate fully, become educated, and can advocate for themselves;</li>
<li>Help clients to develop confidence about their participation in HIV treatment;</li>
<li>Work with case managers to assist clients in identifying services to address other aspects of their life that affect their health, such as mental health and substance abuse treatment, domestic violence services, housing programs, and education and employment services;</li>
<li>Ensure all information related to patient interactions is documented properly and made available to the care team;</li>
<li>Weekly/regular calls to check-in, for encouragement, appointment reminders, and to reschedule missed appointments</li>
<li>Comply with all program protocols, including those regarding confidentiality of client information;</li>
<li>Complete employee all mandatory program trainings to guarantee appropriate competency in program requirements;</li>
<li>Participate in all staff and program meetings, site visits with funders, required network events, and mandatory trainings;</li>
<li>Keep supervisor informed of all problems, challenges and conflicts related to job; and</li>
<li>Perform other duties as assigned.</li>

Knowledge of regional community resources/services is important. Ability to assess clients for needs related to treatment education, risk reduction and prevention.</li>
<li>Experience: Must demonstrate experience with issues related to HIV care and treatment. Familiarity with issues that put people at risk for poorer health is also highly helpful, such as homelessness, incarceration, domestic violence, and/or substance abuse.</li>
<li>Skills: Strong communication skills are essential. Demonstrated ability to conduct sensitive, empathetic interviews and conversations that highly respect the dignity and diversity of clients. Ability to engage staff and clients in a supportive and empowering way. Ability to help manage conflict. Ability to work effectively within a team. Job Summary
The Behavioral Health (BH) Patient Services Coordinator plays a vital role in supporting outpatient behavioral health care for established and incoming patients who need psychiatric or counseling services. This individual ensures a welcoming, organized, and compassion environment and links patients to his/her BH provider, assists with administrative and clerical duties, and supports patient behavioral health needs. The coordinator helps connect patients to services and resources while always maintaining confidentiality and professionalism. This is a position of trust and one of frontline support within our care team.
Communication
Good oral and written communication skills are required for this position. Information must be exchanged professionally and appropriately as well as documented in the patient care record. The person in this position must be able to communicate with the patients, staff, and others and to contribute to a positive BH healthcare environment. The ability to communicate in Spanish, Creole, or both is a plus but not required.
Key Responsibilities
<ul>
<li>Act as the first point of contact for patients, providing a warm and welcoming environment in person and over the phone and providing quality customer service.</li>
<li>Perform daily clerical functions including answering multi-line phones, scheduling appointments, and handling any inquiries for WFHC’s Behavioral Health services.</li>
<li>Assist in keeping the office running efficiently by performing clerical duties, such as answering telephones, photocopying, faxing, documenting in patient care records, and communicating needs to the BH Program Manager and BH clinical care providers as needed. </li>
<li>Ensure all patient information is captured into the current system being used for Electronic Medical Record during the check-in process.</li>
<li>Obtains patient demographic and insurance information, verifies insurance eligibility in advance, and enters accurate information in the electronic health record.</li>
<li>Collect and process payment due for visit and complete daily reconciliation process.</li>
<li>Scan and upload necessary documents such as insurance cards, identification, referrals, and consent forms needed for care.</li>
<li>Work with insurance plans and clinicians to obtain preauthorization of medications and/or services in accordance with payer guidelines.</li>
<li>Assist BH psychiatric treatment services when scheduled including measuring and recording patient vitals (blood pressure, pulse, temperature, weight, etc.) per clinic protocols and ensure all vitals are entered accurately into the EMR and communicated to the care team in a timely manner.</li>
<li>During treatment days, observe patients for any signs of distress or immediate behavioral health concerns and report findings to the BH psychiatric provider promptly.</li>
<li>Protects patients' rights by maintaining confidentiality of personal and financial information.</li>
<li>Maintains professional behavior and attitude towards patients, visitors, and coworkers.</li>
<li>Ensure reception, waiting, and common areas are clean, organized, and stocked with necessary supplies.</li>
<li>Perform other duties and responsibilities as assigned.</li>
<li>Provide coverage for the other WFHC Behavioral Health location in Fort Pierce if assigned (mileage reimbursement provided). </li>
