On an average day you will:

<li>Responds to patient telephone inquiries and provider requests regarding medical concerns, lab results, refills, and referrals</li>
<li>Provides provider coverage by determining urgency of pending lab results, diagnostic imaging, and medication refills requested</li>
<li>Documents patient encounters and care plans in electronic health records system</li>
<li>Provides clinical coaching support and indirect supervision to medical assistants</li>
<li>Completes ongoing/annual training for position compliance</li>
<li>Provides shift coverage at other Neighborhood sites, as directed</li>
</ul>
Immunization Program Support/Oversight
<ul>
<li>Reviews and compares needed records and abstract data to California Immunization Registry (CAIR), eClinicalWorks (eCW), and patient’s immunization records; including identifying when patient immunizations are due</li>
<li>Verifies immunization orders for accuracy, such as correct vaccine, dosage, expiration date, and age</li>
<li>Provides vaccine information statements (VIS) and administers vaccines, as ordered</li>
<li>Documents vaccines administered in CAIR, eCW, and patient vaccine records</li>
<li>Schedules recall appointments for upcoming due vaccines</li>
<li>Ensures all forms are kept up to date, such as Vis forms</li>
<li>Completes and submits required reports, including vaccines administered, reminder/recall reports, state flu reports, and other reports, as necessary</li>
<li>Manages vaccine inventory to maintain accurate inventory, manages supply expiration dates, and rotations of stock to avoid waste</li>
<li>Places vaccine orders in a timely manner to prevent running out of vaccines or creating an over supply</li>
<li>Performs routine cleaning standards to maintain refrigerators and freezers for vaccines</li>
<li>Conducts proper handling, storage, and management of vaccines in compliance with Vaccines for Children (VFC)/ Vaccines for Adult (VFA) Guidelines</li>
<li>Provides staff immunization training in conjunction with medical assistant trainings, as needed</li>
<li>Provides periodic vaccine updates, in-services, or information to staff, as needed</li>
<li>Monitors and records refrigerator and freezer temperatures at least twice a day</li>
<li>Assists vaccine excursions in alignment to standard protocols</li>
</ul>
Patient Education
<ul>
<li>Initiates patient education plan and provides patient education according to their individual needs as prescribed by provider and/or clinic policy, including patient and family instructions</li>
<li>Provide on-going support and continuous assessment for international normalized ratio (INR) patients</li>
<li>Educates and instructs patients regarding lab tests, results, medication functions, and expectations to gain cooperation and allay apprehension</li>
</ul>
Qualifications
Education/Experience
<ul>
<li>Associate's degree in nursing required</li>
<li>Bachelor's degree in nursing preferred</li>
<li>CA RN license required</li>
<li>Valid AHA BLS certification required</li>
<li>Two years of clinical, outpatient, or general medicine experience preferred</li>
<li>Bilingual (English/Spanish) preferred</li>
<li>Valid CA driver’s license or out of state driver’s license for active military personnel/spouses required</li>
<li>Proof of auto insurance required</li>
</ul>

Pay range: $52.88 - $60.09 per hour, depending on experience. This role is eligible for a $2,500 signing bonus.
