<li>Evaluate the effectiveness of planned interventions in relation to patient responses and the attainment of expected outcomes. Document and communicate actions to maintain continuity among the nursing team.</li>
<li>Communicate effectively with co-workers, physicians, patients, families, and other health care team members. Completes hourly rounding, includes the person/family to actively participate in face-to-face bedside handoff/handover report; communicates in SBAR format for all handoff/handover reports and changes in patients status to the Care Provider/physician, Clinical Coordinator or Charge Nurse. Follows the chain of command.</li>
<li>Documents in a concise, pertinent, and legible manner, on the appropriate form and according to accepted standards. Nursing care is provided in an organized, timely, safe, cost effective manner.</li>
<li>Assesses the patients health literacy level. Plans, implements and evaluates effectiveness of patient education. Instruct patients and families on topics such as health education, disease prevention, and develops individualized health improvement programs helping patients make informed decisions about their health and treatment. Promotes patients' independence by establishing patient care goals and teaching patients and families to understand conditions, medications, and self-care skills.</li>
<li>Participates in Quality Monitoring/Performance Improvement. Incorporates Evidence Based practices into daily care.</li>
<li>Performs similar or related duties as assigned.</li>
<li>Knows fire, disaster and safety procedures and regulations as it pertains to the work area</li>

Requirements: Formal Education:

<li>Graduate of an RN education program (accredited preferred); Associate Degree in Nursing required.</li>

Key Responsibilities:

<li>Patient Evaluation:Assess patients referred for home care to determine their eligibility and appropriateness for services.</li>
<li>Care Planning:Routinely evaluate patients and create or adjust the aide Plan of Care based on their needs.</li>
<li>Supervision &amp; Counseling:Provide regular supervision and guidance to Home Health Aides, offering ongoing instruction and counseling.</li>
<li>Documentation:Complete and submit accurate documentation to the agency office promptly.</li>
<li>Communication:Foster continuous communication with agency personnel to ensure optimal patient care and customer satisfaction.</li>

Qualifications:

<li>Education &amp; Licensure:R.N. diploma from an accredited NLN program with a minimum of an associate degree (BSN preferred). Must have a current, valid, and unencumbered RN license or relevant credentialing.</li>
<li>Skills:Intermediate proficiency with computerized systems such as Microsoft Outlook, Word, Excel, HHA Exchange, and electronic/telephonic assessments.</li>
<li>Travel:Ability to travel by public transportation or private vehicle (travel reimbursement provided).</li>

Required Experience:

<li>Minimum of 1 year of nursing experience.</li>
<li>Home care experience preferred.</li>
<li>Bilingual Spanish skills are a plus but not required.</li>
