Job Description 
The Health Manager- Ancillary Services is responsible for overseeing the day-to-day operations of the following ancillary departments: Veterinary services, Environmental services, Food Safety, Hyperbaric Chamber, Med log, and Dental services. This role will ensure that our facilities are in line with all contractual requirements; additionally, this role with ensure our clinicians have everything they need to provide the highest level of service to customers.&nbsp;&nbsp;&nbsp;
Key Responsibilities:

Position Summary
The Resident Services Manager is a non-supervisory position that provides case management and outreach, conducts assessments of individuals and households, and makes referrals that address family and/or individual needs. The Resident Services Manager is committed to providing resources, programming, and referrals directly impacting the Social Determinants of Health (SDOH).
 
Essential Job Duties/Responsibilities
<ul>
<li>Establish and maintain relationships with the residents of assigned property(s to be a resource for the property. Coordinate and promote activities that create a sense of community between the residents and the property. </li>
<li>Facilitate intake and assessment for new residents. </li>
<li>Meet with residents to identify barriers to self-sufficiency and assess physical, social, and economic conditions. Encourage participation in or coordinate referrals to programs focused on SDOH needs. </li>
<li>Develop and maintain a comprehensive network of education, training, financial and economic development, health/wellness, and other community resources as determined by program requirements and resident needs.</li>
<li>Conduct Family Needs Assessment surveys per organizational requirements and coordinate referral services to residents needing help.</li>
<li>Provide intensive informal case management as needed, focusing on case coordination and care planning with Managed Care Organizations (MCO).</li>
<li>Electronically document all activities, outreach, contacts, and outcomes in the applicable data management system, as required. </li>
<li>Develop and foster relationships with community service organizations and programs.</li>

 Job Summary:
The Behavioral Health Services (BHS) Case Manager works with eligible families with moderate to minimal case management requirements to assist them in identifying their strengths and needs. The Case Manager works with the family to develop an individualized service plan that includes appropriate services and supports that meet the needs of the child and their family. The Case Manager informs the family of services and supports and assists, when needed, in coordinating services and supports.
 
Primary Responsibilities:
The primary responsibilities of this position include, but are not limited to, the following:

<li>Screen and enroll eligible participants in the BHS Case Management Program and provide linguistically and culturally appropriate service coordination; including assessing clients needs.</li>
<li>Conduct intake services with clients to assess needs and complete all necessary intake paperwork to maintain compliance and conduct a comprehensive needs assessment with participants.</li>
<li>Facilitates in developing an Individual Support Plan, 90 days review and annual plan</li>
