 Job Summary:
The Behavioral Health Services (BHS) Case Manager works with eligible families with moderate to minimal case management requirements to assist them in identifying their strengths and needs. The Case Manager works with the family to develop an individualized service plan that includes appropriate services and supports that meet the needs of the child and their family. The Case Manager informs the family of services and supports and assists, when needed, in coordinating services and supports.
 
Primary Responsibilities:
The primary responsibilities of this position include, but are not limited to, the following:

Educare Lincoln is a collaborative partnership between the Lincoln Public Schools (LPS), University of Nebraska – Lincoln (UNL), Early Head Start (EHS), and the Buffett Early Childhood Fund. The mission of Educare Lincoln is to help young children thrive socially, emotionally, and academically through high-quality early childhood education, and in partnership with their families and the community. The EHS program (ages 6 weeks to 3 years) and the preschool program (ages 3 years to 5 years) promote school readiness by enhancing the social and cognitive development of children through the provision of evidence-based education, health, nutritional, social, and other services to enrolled children and their families.
The Health Services Coordinator works directly with families and teachers to coordinate health services to children enrolled in the Educare program. Duties include the implementation and monitoring of health &amp; safety practices and the food services program. Involvement in Family and Community Partnerships

Under the supervision of the Senior Director of Integrated Services and/or Clinical Administrator, the Manager of Peer Recovery Support Services provides guidance, supervision, and operational oversight to Eva’s Village Recovery Community Center (RCC) and Opioid Overdose Recovery Program (OORP). The Manager of Peer Recovery Support Services also functions as the Patient Navigator / Trainer within the OORP program.
Job Duties
1. Embodies Eva’s Village’s core values, voice, and tone to provide an exceptional and effective experience for our clients, funders, employees, and volunteers.
2. Oversee, supervise, and monitor the daily operations of the RCC and OORP program, services, and staff.
3. Collaborates with the Senior and Associate Peer Services Coordinators to develop and coordinate activities and services within the Recovery Community Center, ensuring alignment with related programs, departments, and staff to maximize efficiency and maintain compliance with relevant policies, procedures, and standards.
4. Shall attend any training and/or meetings in person or via conference call that are scheduled by DMHAS to support the delivery, sustainability, and effectiveness of OORP services.
5. Maintains effective intra-office and inter-office communication within departments.
6. Shall ensure cooperation and collaboration with the DMHAS statewide evaluator for the OORP, which may include focus group participation; key informant interviews; and individual data reporting (i.e., select social indicators; substance use, overdose and mental health history; patient outreach; services provided, and recovery and treatment follow-up).
7. In collaboration with the Sr. Director of Integrated Services shall engage in communication regularly with the DMHAS Project Director as well as the State Opioid Treatment Authority pertaining to any identified issues and/or barriers in linking individuals to treatment.
8. Shall engage in the data collection, which is to be reviewed with the Sr. Director of Integrated Services before its submission to the DMHAS Project Director monthly (by the 1st of each month) in a format to be provided by DMHAS. Data shall consist of a minimum of the following:
· Number of reported opioid overdose reversals by Emergency Department.
· Basic demographics on the individual reversed from an opioid overdose, including a minimum of the following: Age, Race, Hispanic origin, Gender
· Number of visits per month by the Recovery Specialists, Number of successful referrals to treatment by Level of Care (LOC)
· Number and types of referrals to recovery supports other than substance abuse treatment (i.e. mental health services, housing, employment, etc.)
· Number of follow-up phone calls by Recovery Specialists per month
9. Shall submit a Narrative Contract Progress Report to the Senior Director of Integrated Services and/or Clinical Administrator for review before submission to the DMHAS Project Director within 5 working days of the end of each quarter.
10. Maintain client information and ensure up-to-date documentation in the OORP Teams Channel.
11. Providing an evidence-based comprehensive case management assessment that includes life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs.
12. Communicating with the DMHAS Interim Managing Entity (IME) for substance use disorder assessment authorization and subsequent treatment, as needed.
13. Assist participants in connecting with vital community resources, including but not limited to self-help groups, vocational training, educational opportunities, and social support systems that align with their interests and recovery.
14. Empower participants to advocate for their own needs, ensuring their voices are heard and their goals drive the direction of their recovery services.
15. Providing a warm hand-off to appropriate treatment and recovery services, to include referrals to DHS licensed programs that offer Medication Assisted Treatment (MAT). Developing an integrated Recovery Management Plan (RPM) that is consumer-centered and includes strategies for recovery. The plan shall identify priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment
16. Developing linkages and linking individuals to resources that address specialized needs, such as services related to HIV/AIDS, mental health disorders, chronic and acute health problems, pregnant and parenting women, veterans and problems stemming from involvement with the criminal justice system.
17. Advocating on behalf of the patient.
18. Training Hospital Emergency Department staff (when applicable) on the Opioid Overdose Recovery Program.
19. Shall assist the Recovery Specialists when necessary.
20. As the Patient Navigator shall also engage in, but not limited to, the following:
a. Will be on-call at all times for the OORP program after the scheduled employment shift.
b. Document the integrated case plan in the individual’s case file, as well as document how the warm hand-off was conducted.
21. Performs other relevant duties as assigned by the Director of Integrated Services and/or Clinical Administrator.
Qualifications
1. Must possess a master’s degree in health, psychology, counseling, social work, education or other behavioral health profession.
2. Must possess a Certified Peer Recovery Specialist Certification.
3. Experience working within evidence-based practices.
4. Ability to speak, read, and write English.
5. Must be able to work independently and complete assignments with minimal supervision.
6. Good interpersonal skills are required.
7. Must maintain a professional demeanor and perform duties effectively in stressful/emergencies.
 
EXPERIENCE:
1. Must have a minimum of three (3) years of demonstrated experience working with high-need individuals with substance use and mental health disorders. Must possess the knowledge, skills, and experience necessary to competently perform case management activities.
2. Possess the ability to work with individuals affected by substance use disorder and mental health issues to support and strengthen their capacity to achieve and maintain recovery and engage in health practices.
3. Knowledge of all aspects of addiction treatment and how to access it, including the Stages of Change Theory, Motivational Interviewing, and behavioral problem-solving techniques.
4. Knowledge of the principles of recovery for either substance use or mental health disorders.
5. Job Description
This position will support the Pathway Home 6 grant. This grant is expected to run for 42 months and provides funding for workforce development programs that support incarcerated individuals. The program goal is to improve employment outcomes and reduce recidivism by offering job training and other supportive services during incarceration and upon reentry into the community.

The Offender Services Team is comprised of competent and caring resource professionals who are innovative, resourceful, collaborative, and helpful in facilitating the reentry process for justice involved individuals who access resources at Kinexus Group.  This responsibility requires that we be service oriented relative to the needs of all referred participants, while still assuring that all services adhere to federal, state and local policies and procedures, financial requirements as established through funding sources.

OUR DESIRED OUTCOMES:
The Case Manager will play a pivotal role in the success of the Pathway Home 6 (PH6) reentry initiative. Each Case Manager will be assigned to one of the three county jails and will support job seekers pre- and post-release. The primary goal is to help job seekers develop and follow Individualized Development Plans (IDPs), overcome reentry barriers, gain employment, and reduce recidivism through consistent, trauma-informed, and strengths-based support.
WHAT WE EXPECT FROM YOU:
As a Case Manager for the Pathway Home 6 Program, you will be the primary support for justice-involved individuals transitioning from incarceration to the workforce. You will lead the intake and assessment process, develop and manage individualized reentry plans (IDPs), and provide direct services both pre- and post-release. This includes coordinating training and employment opportunities, connecting job seekers to supportive services, and tracking their progress toward self-sufficiency and reduced recidivism. Your role requires strong communication, organizational, and interpersonal skills to collaborate with jail staff, service partners, and employers while maintaining accurate documentation and compliance with federal performance standards. Ultimately, you are expected to be a proactive, compassionate advocate who helps job seekers overcome barriers and achieve lasting success in the community.
Case Management &amp; Participant Engagement
<ul>
<li>Manage participant documentation, enrollment files, and intake logistics</li>
<li>Provide one-on-one case management to incarcerated and recently released job seekers.</li>
<li>Facilitate program intake, risk and needs assessments (e.g., LS/CMI), and career evaluations (e.g., JOFI).</li>
<li>Collaborate with jail staff and service providers to conduct in-jail visits twice weekly.</li>
<li>Develop and maintain Individualized Development Plans (IDPs) linked to training, employment, and support services.</li>
<li>Connect job seekers with supportive services (housing, legal aid, mental health, substance abuse treatment, etc.).</li>
</ul>
Service Delivery &amp; Support
<ul>
<li>Assist job seekers in obtaining vital records such as ID, Social Security card, and birth certificate.</li>
<li>Provide job readiness training (resume building, interview skills, digital literacy, etc.).</li>
<li>Guide job seekers through job placement and post-release occupational training pathways.</li>
<li>Follow up with job seekers weekly for at least 12 months post-release to support stability and job retention.</li>
</ul>
Collaboration &amp; Communication
<ul>
<li>Act as liaison between job seekers, jail staff, probation officers, public defenders, and external service providers.</li>
<li>Attend and contribute to weekly case coordination and staff performance meetings.</li>
<li>Work closely with the Program Coordinator to ensure timely, accurate entry of participant data into LS/CMI system.</li>
</ul>
Documentation &amp; Performance Tracking
<ul>
<li>Maintain thorough and confidential participant records.</li>
<li>Collect employment verification, pay stubs, and training completion documents.</li>
<li>Monitor WIOA indicators and recidivism metrics, including participant rearrest and reconviction status.</li>
<li>Support data collection for quarterly and annual grant reporting.</li>
<li>Follow all policies and procedures related to case management.</li>
</ul>
MINUMUM REQUIREMENTS:
<ul>
<li>Associate's degree in human services, criminal justice, social work, or a related field (Bachelor's preferred).</li>
<li>At least two years of experience in case management or direct services.</li>
<li>Strong interpersonal and motivational interviewing skills.</li>
<li>Comfort working in correctional and community-based settings.</li>
<li>Proficient in Microsoft Office and familiar with electronic case management systems.</li>
</ul>
PREFERRED EXPERIENCE:
<ul>
<li>Experience working with justice-involved individuals or in reentry programs.</li>
<li>Familiarity with WIOA and LS/CMI case management platform.</li>
<li>Understanding of trauma-informed care and wraparound service coordination.</li>
</ul>
WORK ENVIRONMENT:
<ul>
<li>Office-based with regular travel to jails, employer sites, and partner meetings.</li>
<li>Must pass background checks as required by jail facilities.</li>
<li>Flexibility in scheduling to accommodate access to correctional settings and participant needs.</li>
