Bear River Hospital is a 16-bed facility that offers a broad range of wellness, diagnostic, and treatment services, features a Level IV Trauma Center, and is supported by Intermountains virtual hospital and Life Flight air medical transport.
The Hospitalist Advanced Practice Provider (APP) plays a critical role in the management of adult and pediatric hospitalized patients, with a primary focus on those on general medicine floors. In this role you will work with a multidisciplinary team of doctors, case managers, nurses, dietitians, and pharmacists to provide optimal patient care and outcomes.
 Key Responsibilities:
 Advanced Practice Providers (APPs) are accountable for the provision of patient care at the top of licensure, within their scope of practice. Licensed, credentialed and privileged to perform such duties as:

Conduct comprehensive assessments, including history and physical examinations.

<li>Manageable and Sustainable Caseload: Adult inpatient caseload consisting of 15-18 total encounters per day.</li>
<li>Flexible Schedule: Generally, Monday-Friday, day shifts.</li>
<li>Primary Duties: Initial psychiatric evaluations, assessments, treatment team leadership, and patient discharges.</li>
<li>Work-Life Balance: Occasional once monthly weekend rounding and once per week evening call coverage by phone only.</li>
</ul>
Please note: This position requires 3+ years of previous PMHNP/APRN work experience (clinical rotations and preceptor hours are not included).

<li>Salary range: $145,000-$185,000 depending on experience and job fit. </li>

<h3>Job Description</h3>
Saint Joseph PACE, a member of Trinity Health, is seeking a Nurse Practitioner for its location in Mishawaka, Indiana.
Position Purpose:
Collaborate with PO physicians to deliver primary medical care, assessments, and care plans for participants. Serve as a vital member of the Interdisciplinary Team (IDT), ensuring effective communication and documentation of participant health.
What you will do:
<ul>
<li>Provide primary medical coverage, assessments, and care planning in conjunction with PO physicians.</li>
<li>Assist in discharge planning and manage acute illness, prescribing medications and therapies as needed.</li>
<li>Conduct nursing home or home visits as necessary.</li>
<li>Collaborate on clinical policies and standards of care, offering guidance and training to staff.</li>
<li>Advise on improving accountability and participant services, serving as a resource during program operations.</li>
<li>Participate in provider on-call rotation.</li>
</ul>
Minimal Qualifications:
<ul>
<li>Master's degree in Nursing.</li>
<li>5-7 years of experience, preferably in long-term care.</li>
<li>Current, active license to practice in IN, along with CDS Certification and DEA Registration.</li>
<li>Board Certification by ANCC or AANP, plus certification as a Nurse Practitioner with prescriptive authority.</li>
<li>Current BLS certification and proficiency with Microsoft Office suite.</li>
<li>Valid drivers license and ability to travel within the region.</li>
<li>Experience working with the elderly population preferred.</li>
