<li>Provide direct patient care through office visits, telehealth, and occasional same-day appointments</li>
<li>Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions</li>
<li>Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions</li>
<li>Educate and empower patients on preventive health and functional medicine principles</li>
<li>Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff</li>
<li>Document visits in the EHR (NextGen) in a timely and compliant manner</li>
<li>Participate in team meetings, quality improvement initiatives, and training sessions</li>
<li>Support clinic operations by contributing to a positive, team-oriented environment</li>
</ul>
REQUIRED QUALIFICATIONS
<ul>
<li>Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO (or eligibility to obtain)</li>
<li>Board certification in Family Medicine, Internal Medicine, or applicable specialty</li>
<li>Minimum 2 years of clinical experience preferred (functional/integrative medicine experience is a plus)</li>
<li>Strong interest in preventive, functional, and lifestyle medicine</li>
<li>Excellent communication and patient relationship skills</li>
<li>Comfortable working in a fast-paced, collaborative environment</li>
<li>Proficient in using electronic health records (NextGen experience is a plus)</li>
