Job Summary
Under the general direction of the Chief Medical Officer and the Medical Director of Crisis Continuum Services, the Crisis Continuum Services Psychiatrist provides evaluation and treatment to individuals experiencing a behavioral health crisis and leads a multi-disciplinary clinical team providing support to persons served by OCHN’s crisis system, including Crisis Assessment &amp; Triage, Crisis Stabilization Unit (CSU), Crisis Residential Unit (CRU), Sober Support Unit, Mobile Crisis, Crisis Line/Hotline, and Behavioral Health Urgent Care. This physician will play a critical role in overseeing crisis-related determinations, facilitating the operation of an integrated, evidence-based crisis continuum that meets clinical standards and employs best practices while ensuring resources are responsibly allocated. 
Essential Functions
Direct Clinical Care
<ul>
<li>Performs all duties and responsibilities of a psychiatrist; provides high-quality, cost-effective care consistent with licensing, accreditation, and payer standards. </li>
<li>Conducts and/or supervises psychiatric evaluations, medication management, diagnostic study (e.g., labs, imaging, ECG) interpretation, suicide and violence risk assessments, and substance intoxication/overdose/withdrawal assessments (including the use of structured assessment tools and symptom screening questionnaires); within crisis workflows determines level of care (e.g., ED transfer, inpatient, CSU, CRU, partial hospital, outpatient). </li>
<li>Provides clinical disposition decisions and physician certifications as required (e.g., involuntary petitions/clinical certifications under the Michigan Mental Health Code). </li>
<li>Ensures timely documentation in the EHR and adherence to HIPAA and 42 CFR Part 2.</li>
</ul>
Leadership and Program Support
<ul>
<li>Supports the Medical Director of Crisis Continuum Services; implements protocols, care pathways, and escalation algorithms consistent with SAMHSA Crisis Guidelines and MDHHS expectations. </li>
</ul>
<ul>
<li>Leads a multi-disciplinary team consisting of psychiatrists, Advanced Practice Providers (APPs), nurses, bachelor’s-level clinicians, counselors, social workers, and psychologists to deliver rapid, reliable, evidence-based care and disposition determinations. </li>
<li>Interfaces with relevant internal teams, hospitals/EDs, law enforcement, EMS, and community providers to ensure warm handoffs and least-restrictive, recovery-oriented care transitions. </li>
</ul>
Supervision &amp; Collaboration
<ul>
<li>Leads daily/shift huddles and interdisciplinary rounds (nursing, crisis clinicians, peers, behavioral health technicians (BHTs)/milieu staff, discharge planners) to resolve barriers, update risk, and finalize dispositions. </li>
<li>Provides clinical supervision to medical students, residents, and fellows.</li>
<li>May serve as collaborating physician of record for APPs; provides regular chart audits, scope-of-practice oversight, and timely case consultation.</li>
<li>Promotes a culture of person-centered, trauma-informed, and recovery-oriented practice across teams. </li>
</ul>
Quality, Compliance, and Performance
<ul>
<li>Adheres to quality measures and key performance indicators (KPIs) (e.g., time to triage, time to disposition, 23-hour utilization, return-in-crisis rates, warm handoff completion).</li>
<li>Complies with all relevant OCHN policies/procedures, MDHHS/Medicaid requirements, accreditation standards, and the Michigan Mental Health Code; participates in Root Cause Analyses and special reviews as directed by the Chief Medical Officer or Medical Director of Crisis Continuum Services. </li>
<li>Serves on OCHN committees as directed by OCHN Chief Medical Officer.</li>

Job Requirements and Qualifications
Education:

<li>MD or DO from an accredited medical school.</li>
</ul>
Training Requirements (licenses, programs, or certificates):

<li>Board Certified (or eligible) in Psychiatry; Michigan medical license; Michigan Controlled Substance License; DEA registration; BLS.</li>
<li>Preferred additional fellowship training and board certification (e.g., Addiction Psychiatry, Child and Adolescent Psychiatry, Consultation-Liaison Psychiatry, Forensic Psychiatry, or Geriatric Psychiatry).</li>

Experience Requirements:

<li>Completion of accredited psychiatry residency program.</li>

Work environment: This is a State of Florida Psychiatric Hospital which operates 24/7, 7 days a week. It currently has 223 buildings on campus with a capacity of 1,334 patients. Some patients as young as 13 yrs old. The hospital's current Administration Building is on the National Register of Historic Places. 
Types of patients: Categories of patients are treated at the hospital; those civilly committed under Statute 394, who represent a small portion of the hospital's residents; and those forensically committed under Statute 916. The Civil portion of the hospital houses adult and elderly individuals who have been civilly committed to the hospital, and forensic residents who have been "stepped down" to the civil unit. The civil units are also known as Forensic Transition units. Forensic ward entry has security guards and a metal detector. Most patients are ambulatory young adults age 20-25. Facility also has a small geriatric dorm.
Orientation Requirements: No call outs during orientation will be accepted. Restarting NEO or the ability to continue will be at the facility’s discretion. NEFSH will provide all required in-house training (MANDT, CPR, etc.) during the first two weeks of orientation following hire.
Leave request: Once agency staff begin employment, no leave requests will be accepted during the first eight (8) weeks of employment.
Driving requirement: Will be available to assist other staff with transportation on recreational outings, escort individuals to appointments for prosthetic devices or hospital wide celebrations, and transport individuals to the community for discharge readiness appointments or actual discharge.

Responsibilities
Elwyn currently seeks a Mental Health Professional to support our wonderful new Young Adult Residential Treatment program, set to launch in early 2025. The Mental Health Professional will develop, implement, and document individual progress for individuals with behavioral health and forensic issues who are residents of Elwyn's Young Adult Residential Treatment Facility (YA-RTF). Provides supervision and programmatic decision-making. Completes mental status exams utilizing current knowledge of the Diagnostic and Statistical Manual of Mental Disorders, version IV (DSM-IV). Provides supportive counseling and acts as an advocate to represent the individual's needs and rights with all outside support service agencies, families, etc. Assists and supports individuals in all activities of daily living. Designs and facilitates therapeutic groups. Plans and implements community integration plans.
DUTIES AND RESPONSIBILITIES:

<li>Develop and implement individual programs for individuals with behavioral health challenges and provide other individualized case management functions as directed by supervisor or director </li>

We will be a big FAN of yours too if, at a minimum, you have:
<ul>
<li>A High School diploma or GED equivalent and be at least 18 years old,</li>
<li>Relevant experience working with Individuals with intellectual or developmental disabilities, behavioral challenges, dual diagnoses, or forensic involvement, </li>
<li>A current driver’s license and California automobile insurance coverage for driving your personal vehicle to and from various community locations either independently or with other team members and/or Individuals, and if a company vehicle is available for use, one must also be 21 years of age, and </li>
<li>Ability to read, write and speak English. Bilingual in English/Spanish is preferred.</li>
</ul>

But here’s a known family secret, being part of one, does take effort – it’s all about what you do (your responsibilities/contributions) and how you do it (your behaviors/superpowers) -- so here are some of the things you can expect to experience as you build your FAN club in this house.
 
There will be challenges:
<ul>
<li>Just like families, disabilities come in different types. You will see many of them along with the mental health challenges in the Individuals (aka the Program participants) that we serve. You will also learn about the different agencies and resources we work with to help you provide support directly, or indirectly, to the Individuals. </li>
</ul>
 
There will be chores: 
<ul>
<li>Your work will directly improve the quality of life for the Individuals you support. You will learn techniques to help them have positive interactions with others and reach their goals. You will assist Individuals with their personal needs (e.g., hygiene matters like baths/showers, changing clothes/adult briefs, teeth brushing, medication reminders, etc.) and with social needs (e.g., prepare meals, clean, and engage with them through exercise/movement, games and/or transporting them in your, or company, vehicle to community activities, etc.).</li>
<li>You will also keep accurate progress notes and proactively communicate information with your team about the Individuals’ needs – taking care of them like you would with your own family.</li>
</ul>
There will be chats, C.A.R.E., &amp; cheers: 
<ul>
<li>You will participate in all required training, team, and company meetings to build your knowledge, skills, and abilities to grow in the family. We like to promote from within our own house!</li>
<li>You will take ownership of your own behaviors, and help coach others over time, in how to effectively demonstrate the company’s C.A.R.E. values (compassion, adaptability, respect, and empowerment).</li>
<li>You will receive ongoing coaching and performance feedback and be recognized for your personal and team contributions in addition to demonstrating the C.A.R.E. values.</li>
</ul>

Related experience in the following fields a plus: Assisted Living, Behavior Aide, Caregiver, Certified Nursing Assistants, CNAs, Developmentally Disabled, DD, Job Coach, Direct Support Professional, DSP, Child Care, Elderly Care, Senior Care, Hospice Care, ILS, LVN, Autism, Special Education, Special Needs, ILS, SLS, Housekeeper, Driver, Cleaner, Home Health Aides (HHAs), Residential Care Facility Staff, In-home support, Med Tech, Personal Care Attendant, Day Program Staff or Adult Day Program.
