About the Role:

Social Services Coordinator 
The Social Services Coordinator provides post-hospital care services to guest experiencing homelessness who are transitioning out of an acute care hospital. The Social Services Coordinator applies professional experience and independent judgment to complex tasks and supports organizational goals; performs a wide variety of administrative functions to assess needs and ensure program objectives are met; serves as a community and organizational resource on issues of healthcare and housing. 
Responsibilities and Initiatives 
To help NHF meet its growth goals, the Social Services Coordinator will: 
<ul>
<li>Complete Needs Assessment and Care Plans.</li>
<li>Facilitate discharge process and locating housing for individuals.</li>
<li>Complete discharge notes and disseminate to hospitals.</li>
<li>Complete referrals and coordinate services for mental health, housing, etc.</li>
<li>Maintain up-to-date CRM database and hospital contacts.</li>
<li>Complete Coordinated Entry System ("CES") Documents.</li>
<li>Attend care coordination meetings.</li>
<li>Conduct random searches and maintain harm reduction procedures for contrabands.</li>
<li>Support program team with appropriate guest interventions.</li>
<li>Conduct daily check in with guest.</li>
<li>Collaborate with nursing team to execute weekly case conferencing for guest.</li>
<li>Conduct warm hand off for guest when appropriate.</li>
<li>Ability to manage a caseload of 15-30 guest.</li>
<li>Complete additional tasks as assigned to meet the needs of the facility, notwithstanding primary responsibilities.</li>

Requirements 
<ul>
<li>Bachelor’s Degree Required; master’s in social work preferred.</li>
<li>Minimum of three (3) years relevant experience in healthcare delivery, hospital education, social services, or similar field.</li>
<li>Ability to effectively present information and respond to questions from directors, case managers and discharge planners in hospitals.</li>
<li>Excellent communication skills both oral and written.</li>
<li>Positive and professional attitude.</li>
<li>Time management and strong organizational skills.</li>
<li>Computer proficiency in MS Word, Excel, Outlook, PowerPoint, and proficient typing skills.</li>

Physical Requirements 
The duties as described will required frequent standing and walking, as well as extended sitting. The employee will frequently bend/stoop, squat, reach above the shoulders, twist, and turn, kneel, and push/pull. The employee will frequently be required to lift/pull/push/move up to 50 pounds. Must utilize conversational speech for effective verbal communication. Noise level is high in some client care areas. Must be able to hear equipment alarms, overhead pages, and direct verbal communication.

<li>Respond to telephone inquiries for emergency assistance in a timely manner to determine those who qualify for assistance.</li>
<li>Provide appropriate referrals to other agencies that may assist client either during telephone prescreen or in-office interview.</li>
<li>Interview clients to complete initial files and assistance application.</li>
<li>Based on completed documentation determine assistance eligibility, review prescreen, interview, and after reviewing client information</li>
<li>Complete financial assistance in the form of vouchers, guarantees, or other documentation required to pay bill.</li>
<li>Facilitate the receipt of financial assistance through available grants and other funds.</li>
<li>Provide food assistance in the form of food boxes and/or USDA food.</li>
<li>Complete all aspects of USDA and food box documentation and reports.</li>
<li>Provide accurate and complete food invoices.</li>
<li>Recruit and supervise volunteers for the food pantry.</li>
<li>Keep inventory and order food items for food pantry and/or holiday food boxes.</li>
<li>Travel to the outside agency food pantries and/or grocery rescue as necessary to pick up available foods for the TSA food pantry.</li>
<li>Assess a client while in the office for any available ancillary or referral services.</li>
<li>Provide advocacy and assistance with applications for Identification, Social Security, AHCCS, Food Stamps or other social or health services when appropriate and advisable.</li>
<li>Follow up as necessary with other agency staff co-managing client cases for planned outcome.</li>
<li>Complete of required forms and reports for client financial assistance and in a manner that agrees with the requirements of grants or other funding sources.</li>
<li>Maintain accurate on-going case records for all interactions with client and agencies.</li>
<li>All client files are to be entered into the Wellsky database within 3 days, or sooner if necessary for time limited reports.</li>
<li>Be familiar with and contribute to agency compliance with expectation and grant outcome mandates.</li>
<li>Regularly review program policy and procedure manual to remain up to date on all aspects of our Emergency Assistance Program, and other standards.</li>
<li>Maintenance of necessary documentation for services provided.</li>
