Job status: Full Time (8:00-5:00pm) Some evenings or weekends based on client availability. 
 Job Summary:
The Behavioral Health Services (BHS) Case Manager works with eligible families with moderate to minimal case management requirements to assist them in identifying their strengths and needs. The Case Manager works with the family to develop an individualized service plan that includes appropriate services and supports that meet the needs of the child and their family. The Case Manager informs the family of services and supports and assists, when needed, in coordinating services and supports.
 
Primary Responsibilities:
The primary responsibilities of this position include, but are not limited to, the following:
<ul>
<li>Screen and enroll eligible participants in the BHS Case Management Program and provide linguistically and culturally appropriate service coordination; including assessing clients needs.</li>
<li>Conduct intake services with clients to assess needs and complete all necessary intake paperwork to maintain compliance and conduct a comprehensive needs assessment with participants.</li>
<li>Facilitates in developing an Individual Support Plan, 90 days review and annual plan</li>
<li>Assess the clients strengths and needs and develop a holistic individualized family plan for each client and</li>
<li>Provide wrap-around linguistically and culturally appropriate case management for clients.</li>
<li>Assist clients and their families in applying to external resources.</li>
<li>Conduct home visits to client homes as necessary.</li>
<li>Develop self-sufficiency plans and timelines in partnership with each client.</li>
<li>Provide individualized support through direct services, referrals, and advocacy.</li>
<li>Provide cross-cultural information, orient individuals and families to their community.</li>
<li>Assess and monitor client progress to support their attainment of established goals, address challenges, and to ensure that available resources are utilized.</li>
<li>Understand and adhere to the guidelines and requirements of BHS case management services</li>
<li>Maintains a minimum caseload of clients required by the agency.</li>
<li>Must perform billable hours within the first 60 days of employment.</li>
<li>Must maintain 25 hours billable per week</li>
<li>Renegotiation of salary will be triggered if below 25 hour/week minimum for 4 weeks in a row or for 2 weeks a month for 3 consecutive months</li>

Job Title: Mental Health Services Program Manager

REPORTS TO: Director of Strategic Development
 
SUPERVISES: Mental Health Clinicians, Program Coordinator, Administrative Support, Volunteer Facilitators, and Interns.
 
POSITION SUMMARY: The Mental Health Services Program Manager is responsible for the overall planning, development, implementation, management and evaluation of the Hope Counseling and Children’s Grief Support Program.
 
RESPONSIBILITIES:
<ul>
<li>Provides leadership and oversight of Hope Counseling and Children’s Grief Support Program which includes budget preparation, adherence to policies and procedures, service delivery, quality improvement, and compliance with state and federal regulations including provider credentialing. Manages the training, supervising and evaluating of program personnel. Ensures appropriate service delivery, engages in continuous quality improvement and proposes new efficiencies which best meet the needs of CHHS clients. Participates in CHHS policy and procedure development, attends monthly staff meetings. Presents a monthly program statistical report to Director of Strategic Development.</li>
<li>Carries a partial case load of Hope Counseling clients to support productivity that is billable up to 50% of total work time.</li>
<li>Responsible for the Children’s Grief Support Program growth into additional schools and surrounding communities. Assist with contract management, including negotiations with school district personnel. Develops new or revised services to best meet the needs of CHHS clients. Ensures appropriate volunteer facilitator training for the provision of quality services.</li>
<li>Reviews Hope Counseling documentation of assessments, treatment plans, progress notes, contracts and grant reports. Works in collaboration with leadership to develop a plan for evolving Hope Counseling program.</li>
<li>Provides information to individuals and community groups about Hope Counseling and the Children’s Grief Support Program and opportunities. Serves as a CHHS public resource as a speaker, trainer and presenter.</li>
<li>Performs other duties as assigned by the Director of Strategic Development.</li>
<li>Observes all CHHS policies and procedures and safety requirements as outlined in adopted policies and procedures as well as the Injury and Illness Policy.</li>
<li>Maintains a professional appearance of self and work area; exhibits appropriate behavior with clients and staff, i.e. respectful, courteous, supportive, and professional at all times.</li>

<li>Computer data entry and database maintenance skills required.</li>
<li>At least one year of supervisory experience.</li>
<li>Bi-lingual in Spanish/English is preferred.</li>
<li>Experience working with volunteers is preferred.</li>
<li>Possess a philosophical stance compatible with hospice and palliative care ideals.</li>
<li>Possess public speaking skills.</li>
<li>Demonstrates good communication and interpersonal skills.</li>
<li>Ability to work in a supportive manner in a team setting.</li>
<li>Self-motivated and able to work independently.</li>
<li>Possesses a valid California driver’s license, reliable transportation, and current auto insurance.</li>

WORK ENVIRONMENT: This position works in an office environment using a computer and general office equipment as well as on site on school grounds.
 
WORK SCHEDULE: This position is full-time regular exempt position required to work 40-hours per week. Work hours are scheduled during regular business operation periods to manage the program appropriately. When needed, extra work time may be needed depending on organization needs.
