Job Responsibilities
<ul>
<li>Conduct comprehensive Start of Care (SOC) assessments in patients homes</li>
<li>Develop and implement individualized care plans</li>
<li>Provide skilled nursing services and monitor patient outcomes</li>
<li>Coordinate LPN follow-up visits and revisits when SOC is not available</li>
<li>Collaborate with interdisciplinary teams to ensure continuity of care</li>
<li>Educate patients and families about their treatment plans</li>
<li>Maintain timely and accurate documentation in compliance with HIPAA</li>
<li>Assist in discharge planning and coordinate with community resources</li>
<li>Travel locally to patient homes as required</li>
</ul>
 
Qualifications
<ul>
<li>Active RN license in the state of Pennsylvania (Required)</li>
<li>Home health experience required</li>
<li>Valid drivers license and reliable transportation</li>
<li>Strong clinical assessment and communication skills</li>
<li>Ability to work independently and with compassion</li>
</ul>

<li>Be willing to become certified as a home care worker within a set amount of time</li>
<li>Satisfactorily pass a criminal history and reference check</li>
</ul>
Preferred qualifications:
<ul>
<li>High school diploma, GED, or a foreign equivalent</li>
<li>Experience as a caregiver or in a similar role</li>
<li>A compassionate demeanor and excellent communication skills</li>
</ul>
WORK SCHEDULE:
This full-time position offers flexible scheduling, allowing you to work part-time or full-time (based on your preferences). Caregivers are not required to work a minimum number of hours per week and can choose their preferred days, times, and locations.
