MAJOR FUNCTIONS:
An Office of Mental Health (OMH) Housing Case Manager plays a crucial role in assisting individuals with mental health challenges to secure and maintain stable housing. The primary responsibilities include conducting assessments, developing personalized service plans, connecting clients with necessary services, and ensuring compliance with housing regulations. The role requires a combination of direct client support, coordination with service providers, and administrative tasks to promote clients' independence and well-being.
SPECIFIC DUTIES &amp; RESPONSIBILITIES:

<li>Client Assessment and Service Planning: Conduct comprehensive assessments to identify clients' housing and support needs. Develop individualized service plans with short-term and long-term goals, regularly reviewing and updating them to ensure they meet clients' evolving needs. </li>
<li>Service Coordination: Connect clients to essential services, including medical care, mental health support, substance use treatment, legal assistance, and entitlement programs. Act as a liaison between clients and external organizations to advocate for their needs. </li>
<li>Direct Support and Monitoring: Provide ongoing support through regular home visits (at least one per month) and telephone contacts. Monitor clients' progress toward their goals, document interactions, and adjust service plans as necessary. </li>
<li>Crisis Intervention: Respond to crisis situations promptly, employing de-escalation techniques and coordinating with appropriate services to ensure clients' safety and stability. </li>
<li>Documentation and Compliance: Maintain accurate and up-to-date case records, including service plans, progress notes, and other required documentation. Ensure all activities comply with OMH guidelines and regulations, preparing for audits as necessary. </li>
<li>Client Empowerment: Assist clients in developing life skills necessary for independent living, such as budgeting, cooking, and community navigation. Provide education on tenants' rights and responsibilities to promote self-sufficiency</li>

REQUIREMENTS: EDUCATION AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES:

POSITION SUMMARY: The Case Manager is responsible for working with youth to meet their housing, medical, mental health, employment and educational goals as they move towards becoming housed and having permanent connections. Essential functions of the position include intake and assessment, case planning, documentation, and basic individual and group counseling. The position is also responsible for providing these services within the framework of the Mission Statement, Five Principles, and the Policies and Procedures of Covenant House California.
Classification: Full-time non exempt, 40 hours per week
Pay Rate: $24.00 / hour
ESSENTIAL FUNCTIONS:
1) Engage youth in developing their Individual Service Plan.
2) Create an ISP for each youth that addresses areas all their critical needs, through a trauma-informed and positive-youth-development model.
3) Partner with the youth to create a clear structure of addressing their needs, complete with due dates, calendars, structure, guidance, and support.
4) The case manager links clients to needed services, including making referrals, and as needed, facilitating the youth’s accessing of those services.
5) Monitor client progress and aide youth in meeting their identified goals, and coaching youth to plan, implement, and follow-through.
6) Motivate clients to improve wellness along health, mental health, substance use/abuse/dependence, and other areas that will improve their overall functioning.
7) Coordinate between other departments (clinical services, outreach, residential) and work as a team to meet client needs.
8) Utilize innovative approaches to engaging youth in case management: such as conducting case management outside the building, on walks, in parks, and/or planning learning outing visits to the bank, grocery store, job-site, etc.
9) Partner with the youth to find full-time employment. This work involves resume building, cover letter writing, job applications, interview preparedness, professionalism, etc.
10) Partner with youth to identify and meet their educational goals, but not in lieu of employment which is the fastest track to self-sufficiency and to becoming housed.
11) For youth who have developmental, mental health, health, or social challenges, work with the youth to modify their case plan according, while at the same time balancing the responsibility to the youth to move them towards progress and becoming independent.
12) Provide unconditional positive regard to clients regardless of behavior, race, class, culture, country of origin, disability, gender, gay, lesbian, trans, bi, language capacity, and ability to complete case plan requirements.
13) Collaborate closely with team members to ensure that activities are focused on and in sync with the needs of youth.
14) Maintain statistics and complete reports as needed.
15) Attend weekly supervision with supervisor and all weekly meetings as assigned.

JOB REQUIREMENTS:
EDUCATION: Bachelor’s Degree in Psychology, Sociology, Social Work or a related field preferred, but will consider additional relevant experience.
EXPERIENCE: 2-3 years of professional experience working with at-risk/homeless youth, homeless services, child welfare or serving other at-risk groups.
SKILLS: Prior Case Management Experience and an ability to build positive rapport with youth. Knowledge of the full range of public and social services in Alameda County and the Greater Bay Area for low-income, homeless, and foster youth. Excellent communication skills, as demonstrated by on-point, timely, and professional correspondence, both written and verbal inter-agency coordination skills (scheduling appointments, and maintaining collaboration with multiple stakeholders and providers). Communication with and about youth must reflect a “strength based” and “trauma informed” approach. MUST be bilingual - English/Spanish.
TRANSPORTATION: Must have reliable transportation, a valid California driver’s license and good driving record.
WORKING CONDITIONS: General business/office environment. Must be able to lift up to 40 lbs. on occasion, bend, sit and stand for prolonged periods of time. Some weekend/holiday work may be required.

Food Management: · Shop for and store healthy, nutritional food for the program participants. Assure there is an ample supply of food and menu items can be made daily. · Manage petty cash account for the residence, including program participants personal funds; · Monitor staff menu planning and reduce waste; Ex: leftovers used first, timely use of produce, etc. Assures that guidelines for food storage and disposal are followed. · Manages out to eat following approved restaurants and menu items to be purchased, assures staff are purchasing the same type of items as program participants. Scheduling and On-Call: House Leads are required to complete schedules and make revisions in the time keeping system before payroll Mondays. House Leads are responsible for finding coverage for call-ins, leave for vacations/absences, etc. and must stay in contact with the Program Director/Lead Supervisor regarding coverage and keeping overtime at a minimum. House Leads are responsible for working the shift if coverage cannot be found. House Leads are responsible for attending Special Olympics and other company and/or program participant related events. Must be able to cover urgent care, crisis, and emergency situations. House Leads are required to have a cell phone at all times. Must be available to receive calls during shift, after hours, and on weekends for emergency, non-emergency situations, and staffing needs/questions (including staff coverage/crisis). House Leads must return emergency/crisis calls within 20 minutes, all other calls within 24 hours. Crisis Intervention Technique: · Follow all behavior support plans for program participant. · Utilize the Company’s approved crisis intervention technique if approved in the behavior support plan. · Employees must step back when a program participant has a challenging event and/or behavior. Restraint is used as a last resort. Utilize restraint only if the program participant is trying to physically hurt themselves, someone else or put themselves in immediate danger. Services are based on motivational interviewing, stages of change, strengths-based case management, housing first, and trauma-informed care. The position will provide outreach services, referral and linkage with mainstream services and housing, and crisis intervention for program clients. NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date.&nbsp; The credentialing and&nbsp;privileging&nbsp;timeline is dependent on the selected candidate's submission of documents necessary for clearance.&nbsp; Please note that a start date will be scheduled once credentialing and privileging is complete.Additional Requirement

<li>Provide strengths-based case management, specifically housing placement and support, to homeless individuals with disabilities including substance use and significant primary and mental health care needs.</li>
<li>Assists with connection to mainstream resources including health, mental health, and substance use treatment.</li>
<li>Advocates for the needs of individual clients as well as for systemic change, whenever needed for the overall benefit of homeless individuals.</li>
<li>Accepts referrals internally from the Housing Intake and Placement Team as well as externally from the Denver Street Outreach Collaborative, and the statewide Coordinated Entry process and works collaboratively with staff of all agencies to provide high-quality service. Reports back to referral source in a timely fashion to inform them of client disposition and additional services needed.</li>
<li>Communicates clinical assessments, interventions, and ongoing client contact through timely, thorough recordkeeping, route slips, contact forms, and verbal communication.</li>
<li>Works with primary clients, as well as team clients, related to service planning, documentation of progress, service tracking, and outcome evaluation.</li>
<li>Monitors client progress by regular, ongoing contact with individuals receiving services and contact with collaborative service providers both external and internal to CCH.</li>
<li>Transports clients using a CCH vehicle or an approved personal vehicle.</li>
<li>Serves as a liaison with collaborating agencies and other teams within the organization, establishing and maintaining effective working relationships with organizations and groups providing services to our target population.&nbsp; </li>
<li>Assists others with work that needs to be done at moments of urgency or crisis.</li>
<li>Navigates the Social Security, Medicaid, and Medicare systems on behalf of or with clients. </li>
<li>Completes Medicaid authorizations, HMIS data entry, as well as any other reporting requirements necessary to obtain, and sustain funding and provide services for individuals served.&nbsp; </li>
<li>Meets formally with the Program Manager for ongoing individual and group supervision.</li>
<li>Performs additional duties deemed necessary and appropriate by the Program Manager.&nbsp;</li>

<li>Bachelor’s degree in human services, social work, psychology, or related field preferred or equivalent experience. </li>
<li>Previous direct experience with homeless population preferred, or previous experience in a related direct human service field working with underserved populations preferred.</li>

Responsibilities:
