You will: 
<ul>
<li>
Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.

Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment.

Required Qualifications:

A minimum of one year of experience as a Nurse Practitioner is required.&nbsp; Experience in Family medicine or primary care practice preferred.

Active Family Nurse practitioner license in good standing

Provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. This includes education and treatment for pregnancy prevention, sexually transmitted infection (STI) Prevention and safer sex practices.

<li>
Order and administer immunizations and injections (to include but not limited to COVID19, influenza, TB, Depo) and provides evidence-based patient/parent vaccine counseling and education.
</li>
<li>
Minimum of two years of medically relevant experience or equivalent
</li>
<li>
Complete the DOT certification exam within 30 days of employment
</li>
<li>
Depending on the market, the ability to be proficient in both speaking and writing in additional languages not limited to but including Spanish
</li>
<li>
Basic Life Support (BLS) certification, including but not limited to performing CPR and operating an Automated External Defibrillator (AED).

Pass a FIT test and wear the provided respirator mask when protocol requires.

Education:

Pay Range
The typical pay range for this role is:
$87,035.00 - $187,460.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. &nbsp;The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. &nbsp;This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.&nbsp; &nbsp;

<li>Does not prescribe for insomnia but first refers patients to a psychologist, social worker, or mental health counselor to utilize cognitive-behavioral interventions for the treatment of insomnia when appropriate.</li>

HopeHealth is seeking an experienced Palliative Nurse Practitioner for its affiliate HopeHealth Hospice &amp; Palliative Care. 
This is a part-time 24 hours per week position that is eligible for benefits, and may have occasional weekend and holiday rotations, no more than once every quarter. 
The Palliative Care Nurse Practitioner works in collaboration with the referring provider and appropriate consultants to provide relief of suffering for patients and families facing serious illness. They will provide assessment and recommendations to relieve burdensome symptoms like pain, dyspnea, anxiety, fatigue. They will be able to communicate serious news and help patients and families establish goals compassionately and clearly for care. 
The palliative Nurse Practitioner needs to be highly collaborative, working cooperatively with patients, families, referring entities, serving as an expert in hospice and palliative care. This position serves patients in homes, nursing homes, and assisted living in southern RI. 
Duties / Responsibilities
Direct Patient Care
<ul>
<li>New consults and follow up consults for seriously ill adults in a variety of inpatient and outpatient settings including but not limited to homes, assisted livings, nursing facility, hospitals, clinics.</li>
<li>Complete pain assessments and pain management recommendations for seriously ill adults.</li>
<li>Assesses and makes recommendations to manage other non-pain symptoms including nausea, fatigue, dyspnea, anxiety.</li>
<li>Assesses patient’s disease trajectory and co-morbidities and, in conjunction with treating physicians, formulates prognosis</li>

Job Description:
We are looking for an experienced, dedicated, and Registered Nurse Practitioner (NP) to join our growing team of medical professionals. You will provide direct patient care, discussing patient symptoms and concerns, and perform thorough examinations to diagnose their medical problem and prescribe medication or create a treatment plan if necessary. You will also administer treatment and observe patients' functioning over time.
 
Furthermore, your duties as a licensed nurse practitioner will include recording the medical histories of patients, perform physical exams, administer diagnostic tests, and create patient care plans. When required, you will collaborate with our physicians to treat for more advanced illnesses or injuries.&nbsp;
 
To be successful as a nurse practitioner, you should be empathic but resolute in your decision-making, and attentive to detail. Ultimately, a top-notch nurse practitioner will exhibit cultural competence and exceptional teamwork skills.
Responsibilities:
 
<ul>
<li>Perform physical examinations and screenings of patients.</li>
<li>Record the patient's medical histories and symptoms.</li>
<li>Facilitate new patient consultation and health history review.</li>
<li>Create patient care plans and contributing to existing ones.</li>
<li>Order, administer, and analyze diagnostic tests, including bloodwork.</li>
<li>Review and interpret test results to diagnose health problems.</li>
<li>Recommend further treatment options or future appointments if needed.</li>
<li>Monitor and operate medical equipment.</li>
<li>Administer medicine and other treatments.</li>
<li>Provide referrals to other practitioners when necessary.</li>
<li>Detect changes in the patient's health and modify treatment plans, as needed.</li>
<li>Provide after-care/ongoing care information and resources to patients and their families.</li>
<li>Consult with healthcare professionals and families.</li>
<li>Train patients and their families to manage and prevent illness and injury.</li>
<li>Ensure patient records are kept up to date including test results, immunizations, condition changes, etc.</li>

Requirements:
 
<ul>
<li>Minimum of 1 year of recent experience as an RN in a clinical setting.</li>
<li>Active RN license within the State of practice.</li>
<li>Current certifications (variable depending on the facility).</li>
<li>Prescriptive authorization.</li>
<li>Experience providing primary care as a family nurse practitioner.</li>
<li>Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology.</li>
<li>Able to operate as part of a team, alongside physicians and other nurses.</li>
<li>Proficient in EHR medical charting system and computer-based data management programs and information systems.</li>
<li>A sound understanding of all pertinent federal and state regulations, including HIPAA and OSHA.</li>
<li>Excellent verbal and written communication skills.</li>
<li>Knowledge of disease prevention.</li>
</ul>
