Job Description 
This role is considered the minimum role for all nurses at UMMC who have greater than 12 months experience. This nurse will be competent in the skills/knowledge required and fully participate in unit governance. He/she will practice and adhere to the guidelines specified in the UMMC Professional Nurse Job Charter. Independently provides and coordinates care provided to patients within a clinical specialty. Serves as a fully effective and participative member of the multidisciplinary patient care services team. Demonstrates and models the full range of nursing professional practice in both care provided and in participation in development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness at the unit level. Job Summary
As a Transfer Coordinator RN, you help patients move within Baylor Scott &amp; White Health System. Your main job is to make it easier for doctors and patients to access our facilities and providers through the Transfer Center. You will focus on providing excellent customer service and interaction, leading to higher customer satisfaction.
Essential Functions of the Role
<ul>
<li>You will ensure smooth patient transfers by working closely with physicians, nurses, and other departments while following laws. Plans will promote good outcomes and efficient hospital stays.</li>
<li>You will provide excellent customer service within and outside the organization, striving to be helpful and responsive.</li>
<li>You'll follow protocols to gather information accurately and on time from other institutions and physicians.</li>
<li>You'll enhance services provided to all facilities within our System with this information and our resources.</li>
<li>You will strive to ensure that all documentation is clear, concise and meets standards by using appropriate forms.</li>
<li>Your role includes managing customer relationships by driving satisfaction among referring physicians and expanding BSWH market share. This involves follow-up calls, clear communication, and prompt issue resolution. You will oversee and assist the Transfer Center Room Control Coordinator.</li>

RN Transfer Center Coordinator PRN

JOB SUMMARY The Operating Room (OR) Registered Nurse (RN) is a licensed professional who uses the BSWH nursing professional practice model to coordinate patient care delivery by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN promotes safe passage for their patients by using knowledge of patient needs and the healthcare environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition and the competencies of the employee. ESSENTIAL FUNCTIONS OF THE ROLE Clinical Judgment: Using clinical reasoning, conducts accurate clinical assessments according to practice standards. Identifies and prioritizes patient and family needs. Develops, implements and evaluates the nursing plan of care. Modifies plan to meet clinical outcomes. Clinical Inquiry: Systematically evaluates the quality and effectiveness of nursing practice, including, but not limited to, nursing sensitive indicators.
