Department: BH Forensic ACT-Tech 
Forensic Assertive Community Treatment (FACT) is an evidenced based model that provides wrap around mental health services in the community to adults experiencing persistent and severe mental health issues, and criminal justice involvement. The FACT RN is a part of a multidisciplinary team and is primarily responsible for providing medication support and physical health appointment coordination support to clients. FACT RN is also responsible for managing the FACT medication room. Most of the services provided by the FACT RN take place in the community, MDC, and other nontraditional settings which includes clients’ homes and shelters.&nbsp;

Position Summary: Provide professional nursing care for assigned patients in a mental health/psychiatric environment. Design nursing care plans and assist in the development of overall care plans for psychiatric patients, ensuring administration of appropriate overall care in a manner conducive to recovery. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include pediatric, adolescent, adult and geriatric age groups.

Direct Support Professional

Glassport LTSR is a long-term structured residence that serves males with a forensic and mental health history. Our goal is to reintegrate these residents back into the community. We hope to accomplish this goal by working on activities of daily living, such as cooking, cleaning, and budgeting. We also link the residents to service providers who can also help them build relationships in the community. We strive to provide holistic care through group and individual therapy, medication management, psychiatric rehabilitation activities, and evidence-based practices. 
This is a full time position and the shifts are Monday through Friday 3 pm to 11 pm.
Responsibilities:
 Complete daily mental status exams on 6–12 residents. 
 Complete narcotic counts at beginning and end of shift. 
 Administer medication via medication administration record. 
 Identify early signs of psychiatric relapse, medication side effects, or medical issues. 
 Maintain detailed nursing notes in EMR/charting. 
 Document progress toward treatment plan goals. 
 Ensure compliance with state regulations, Joint Commission, and organizational policy. 
 Lead response to psychiatric or medical emergencies. 
 Make patient appointments and assist in transportation when needed. 
Clinical 
 Collaborate on Personal Recovery Plans which emphasize residents’ strengths and needs. 
 Engage with residents regularly to note changes that may impact the recovery plan. 
Educational 
 Participate in program milieu. 
 Attend and/or facilitate educational groups and meetings. 
 Provide education to support residents in understanding medication management. 
 Provide education to support residents in understanding daily health and wellness needs. 
Medication and Health Management 
 Complete assessments/physicals within regulatory time frames. 
 Ensure residents’ prescriptions are current and collaborate with MD as needed. 
 Coordinate medications and health needs with the program Psychiatrist. 
 Maintain electronic medication administration record (eMAR). 
 Monitor the administration of all medications. 
 Assist residents with scheduling medical appointments and follow up on recommendations. 
 Coordinate physical healthcare with Primary Care Physicians and specialists. 
Program 
 Document educational groups and other resident contact. 
 Participate in case consultations, staff meetings, and supervision as assigned. 
 Attend training as required by RHD and licensing bodies. 
 Perform duties as assigned by director to ensure continuity of care. 
Job Qualifications 
• Current PA RN/LPN licensure 
• Current CPR/First Aid certification 
• Experience with Behavioral Health/Criminal Justice preferred 
• Valid Driver’s License 
• Act 33/34 clearances 
• FBI clearance as needed 
• All trainings required by licensure 
 Must complete a Physical, TB test, and Hep B titer
Competencies:

Forensic Nursing
A forensic nurse is a Registered or Advanced Practice Nurse who has advanced education and training in the specialized care of patients who are experiencing violence. Forensic Nurse Examiner-FNE/SANE (PRN-Evenings, Weekends, Holidays)
This is a PRN, on-call paid position for nights, weekends and holidays, when the center is not actively staffed. This position is responsible for ensuring delivery of Forensic Medical Services when requested by law enforcement and the Department of Family and Children Services, for children victimized by crimes of sexual assault/abuse and violence. This includes timely, accurate charting and entry of client information into the Electronic Medical Record (EMR) and Collaborate case tracking system. In addition, the FNE will assist with administrative tasks as requested. Participate and engage in staff meetings and other agency and community functions that represent the agency and promote the agency programs of direct victim-services for children victimized by crimes
