Responsibilities:

Requirements:

<li>Minimum of 1 year of recent experience as an RN in a clinical setting.</li>
<li>Active RN license within the State of practice.</li>

Overview
The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. • Respond to and deliver care according to protocols and procedures. • Evaluate and treat patients for acute care and emergency medicine complaints based on standardized procedures and protocols. • Consult with a physician for conditions outside protocol parameters and identify patients who require transfer to an Emergency Department. • Assess patients' medical, traumatic, and psychiatric issues and document all findings using the S.O.A.P method and EMR. • Participate in Quality Management and Peer Review activities, attend administrative meetings, and provide clinical support for other services as needed.
 
Qualifications

Experience

Qualifications and Requirements
Education &amp; Certification

<li>Active, unrestricted CaliforniaPhysician Assistant (PA-C) or Nurse Practitioner (NP) license.</li>
<li>National certification (NCCPA for PA-Cs; AANP or ANCC for NPs) is required.</li>
</ul>
Experience
<ul>
<li>Minimum 2 years of clinical experience in primary care, internal medicine, geriatrics, or chronic disease management.</li>
<li>Experience with CCM, RPM, or value-based care models is strongly preferred.</li>

Technical Skills

<li>Proficiency in electronic health records (EHR).</li>
<li>Familiarity with RPM devices, data platforms, and telehealth technologies.</li>

Position Summary 
Qualified Nurse Practitioners interested in this position will have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.  
Nurse Practitioner Responsibilities

<li>Travel to and from patients’ homes. </li>
<li>Perform comprehensive, non-invasive physical assessments. </li>
<li>Review medical history and ensure accurate and timely documentation. </li>
<li>Conduct medical reconciliation to ensure patient safety and compliance. </li>
<li>Provide basic patient education on wellness and preventative healthcare measures. </li>
</ul>
Nurse Practitioner Qualifications and Experience Requirements:  
<ul>
<li>Master’s Degree OR commensurate experience and satisfactory completion of NP license </li>
<li>Active State NP license </li>
<li>Active National board certification </li>
<li>Current BLS certification </li>
<li>12 months of experience as a paid advanced clinical provider is preferred but not required </li>
