As a Nurse Practitioner Traveler is responsible for providing continuing and comprehensive medical care and health maintenance to senior-age patients. IMA’s Nurse Practitioner traveler exercises outstanding diagnostic services, cares for the patients, and provides them with appropriate healthcare services that meet state and national standards of care. Additionally, this person is also responsible for conducting comprehensive annual health assessments. These health assessments will involve closing quality care gaps, identification and documentation of chronic medical conditions, and education and counseling of patients regarding their conditions and lifestyle improvement. 
This position will require the practitioner to travel to multiple locations and spend at least 50% of the time functioning as a primary care provider, with the remaining amount of time spent conducting annual health assessments.

Core Responsibilities
<ul>
<li> Evaluates veterans for mental health disorders and appropriately diagnoses and treats mental health conditions.</li>
<li> Orders and performs diagnostic tests and therapeutic procedures as related to mental health diagnoses and treatment and interprets test results for deviations from normal and provides follow up care.</li>
<li> Prescribes medication and recommends dietary and activity programs.</li>
<li> Performs appropriate interval follow up visits for necessary treatment of mental health diagnosis.</li>
<li> Refers patient to the VA medical specialist when indicated.• Counsel patients on the use of prescription medications and educates patients in relation to treatment plan.</li>
<li> Develops and implements patient management plans, assists in provision of continuity of care through the PCMM (Primary Care Management Module).</li>
<li> Evaluates patients records from medical providers outside the VA and works with the veterans utilizing rules set forth by the VA for co-managed care.</li>
<li> Conducts individual counseling/therapy.</li>
<li> Contributes to group therapy (i.e., PTSD groups; weight management; smoking cessation; grieving; and personal interactions).</li>
<li> Contributes to pain management plan as needed.</li>
<li> Involves veterans in planning and assuming responsibility for activities.</li>
<li> Refers veterans, when indicated, to community resources and other organizations.</li>
<li> Addresses issues such as advance directives and organ donation in accordance with acceptable standards.</li>
<li> Inputs all patients’ information into VISTA/CPRS.</li>
<li> Assists in co-managed care collaboration with non-VA providers, and facilities.</li>
<li> Maintains confidentiality of veterans’ data and information.</li>
<li> Complies with all VA and company training requirements.</li>
<li> Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission(JC), Environment of Care (EOC) oversight, lab compliance and other related items.</li>
<li> Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.</li>
<li> Must provide excellent customer service to each veteran and his/her family, both in person and over the phone, as well as to fellow colleagues and clinic visitors.</li>
<li> Participate in the clinic’s outreach events to help support the clinic’s enrollment initiatives.</li>
<li> Support patient enrollment and retention in appointments, follow-up visits or nurse visits.</li>

Requirements:
Qualifications
<ul>
<li>Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioner (MSN required), including preceptorship</li>
<li>Certification by applicable professional organization</li>
<li>Minimum three-year clinical experience as an adult mental health CRNP. (Specific requirements could vary based on individual VA contract).</li>
<li>Must be credentialed and remain in good standing through the Veterans Health Administration (VA).</li>
<li>Must comply with and maintain all requirements for a valid, unrestricted license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.</li>
<li>Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract</li>
<li>Current, unrestricted Drug Enforcement Administration (DEA) registration</li>
<li>Proficiency in written and spoken English.</li>
<li>Strong computer skills</li>
<li>Energetic and optimistic demeanor</li>
<li>Strong service mentality and a focus on achieving all aspects of defined service standards</li>

About the Position: 
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities
What to Expect:
<ul>
<li>Conduct Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up</li>
<li>Provide services in a variety of venues to include: Home Visits, Skilled Facility Visits</li>

Type: PRN - Flexible - Nurse Practitioner
PAY: Earn from $2,000 up to $10,000 per month (Based on an average of 20-100 completed visits per month, and pay of $100 per completed assessment). Compensation range is based on availability and assessment completion.
Location: Home, SNF, and other community environments in Austin, Texas
Hours: Days, Evenings, Nights, Weekends, Flexible Scheduling

Qualifications
What is Required:
<ul>
<li>Master’s Degree OR commensurate experience and satisfactory completion of NP licensure</li>
<li>Current RN and NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority</li>
<li>Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty</li>
<li>Current BLS, ACLS or CPR certification</li>
<li>Minimum of 1 year experience as a Nurse Practitioner</li>
<li>Strong computer skills and familiarity with employee health/medical record software</li>
<li>Excellent verbal and written communication skills with patient, clients, and colleagues</li>
