Position Summary:
The Safe Therapeutic Assault Response Team (START) program is seeking a Forensic Nurse Examiner to join our team. This role is responsible for providing high-quality, comprehensive, medical-forensic exams and care to sexual assault and intimate partner violence patients at HAVEN. This is primarily a night and weekend on-call position. We seek to reflect the diverse community we serve and be a welcoming, inclusive space for all. Under the guidance of the START Program Director and Senior Forensic Nurses, the Forensic Nurse Examiner will interact both in-person and via phone with patients needing urgent medical services and ensure that critical information and communication is provided to the patients in a reassuring, clear, and appropriate manner. 
Essential Duties:

<li>Assists and supports in the management of the HAVEN / START program.</li>
<li>Works as a team with First Response Advocates to implement trauma informed best practices.</li>
<li>Performs comprehensive medical forensic examinations of sexual assault and intimate partner violence patients, including the preparation and reporting of examination results. </li>
<li>Attends at a minimum of 8 monthly START team meetings.</li>
<li>Acts as a subject matter expert and participates in legal proceedings including providing expert witness testimony, as needed.</li>
<li>Promotes greater awareness of services for both patients and a wide variety of professionals.</li>
<li>Advocates for patients of sexual assault and domestic violence. </li>
<li>Assists with patient satisfaction surveys and program evaluations.</li>
<li>Ensures adherence to HAVEN policies and START practice guidelines and compliance requirements.</li>
<li>Works as a team with internal HAVEN programs and external business partners to collaborate on building awareness initiatives.</li>
<li>Maintain equipment and supplies in examination room during and after use.</li>
<li>Maintain records and statistics, as required by state and federal laws.</li>
<li>Participate in ongoing competency and evaluations at the discretion of the Director.</li>

COMHAR’s Long Term Structured Residences (LTSR) are located in Northampton County. This position is in Walnutport, PA. These therapeutic programs are designed to provide 24/7 residential and intensive treatment supports for adults with a variety of psychiatric and health related needs. Individuals requiring this level of care have access to a variety of services including individual and group therapies, psychiatric evaluation, crisis intervention, peer support, psychiatric rehabilitation and medication management. 
 
The multidisciplinary treatment team is comprised of staff that includes a psychiatrist, mental health professionals, licensed nurses, peer specialists, and direct care staff. Services are delivered both on-site and in natural community settings to maximize learning opportunities. Our focus is to assist individuals in developing skills, increasing their independence and stabilizing their psychiatric and physical health symptoms with the goal of moving into lesser structured settings.
 
COMHAR’s Long Term Structured Residences (LTSR) has an immediate opening for an Part-Time Licensed Practical Nurse or Registered Nurse for our Staff Nurse position.
 
Staff Nurse Responsibilities:

<li>The Licensed Practical Nurse works from a recovery framework within the team providing health and wellness care, medication education, group psycho education, therapeutic interventions (evidenced based practices preferred), cultural competence and trauma informed care to help individuals obtain their recovery goals.</li>
<li>Provision of assistance to residents who are unable to self-administer medication. Supervision of self-administered medication when indicated. Injection of medication when prescribed. Participation in education of residents regarding their medications, including assisting residents who are unable to self-administer medication to become able to do so.</li>
<li>Participation in multi-disciplinary treatment team meetings as assigned.</li>
<li>Participation in Quality Improvement Program as assigned.</li>

SUMMARY OF POSITION: 
 
Pine Street Inn provides comprehensive, intensive clinical support to chronically homeless individuals with histories of substance abuse and mental illness using the Housing First model promoting long-term stability in housing with a mission to end of homelessness in Boston. The Lead Case Manager provides intensive case management services to guests in the Emergency Shelters, including but not limited to the following populations: older adults; guests with brain injuries; developmental disabilities; severe mental health symptoms; history of violence, including domestic violence; and pregnant guests. The Lead Case Manager will provide support to homeless individuals to overcome barriers and obtain and retain housing or obtain alternative housing placements when necessary. The Lead Case Manager is responsible for providing support to guests for an array of services under the following categories: medical, emergency, vital records, transportation, housing, immigration, job search and training, mental health, substance use, financial, benefits, guardianship, family consultation, skilled nursing and assisted living placements to help meet daily needs, optimize stability, and enhance upon their functional abilities. In addition, the Lead Case Manager collaborates with leadership in Emergency Shelter and in Housing departments, developing and maintaining positive relationships with partner agencies, stakeholders that contribute to the overall success of the Behavioral Health Department. The Lead Case Manager will need to be a team player, with great organizational, and time management skills, and a communicator/collaborator who can work well with other teams/departments. The Lead Case Manager works directly with homeless guests with the goal of securing housing, and related benefits and services needed to retain housing and empower clients towards greater self-sufficiency. The Lead Case Manager actively manages caseload to facilitate effective engagement with guests utilizing evidence-based practices of Motivational Interviewing, Trauma-Informed Care and Harm Reduction. The Lead Case Manager assesses guests’ vulnerability, chronicity, substance abuse issues, mental health symptoms, legal issues, health care, employment and income, independent living skills, and community engagement to guide frequency and type of case management intervention. The Lead Case Manager maintains accurate written records and documentation in accordance with program standards including, but not limited to ISPs, guest charts, incident reports, detailed case notes and referrals and third-party billing, when applicable. The Lead Case Manager maintains HMIS records in a timely and accurate fashion and contribute to reporting when needed.

ESSENTIAL JOB FUNCTIONS

<li>Administers the operations and activities of the Laurens County Coroner’s Office in accordance with local, state and federal laws, regulations, policies and procedures.</li>
<li>Supervises staff through the chain of command; supervisory duties include instructing, planning and assigning work, reviewing work, maintaining standards, coordinating activities, selecting new employees, acting on employee problems, recommending and approving employee discipline and discharge as appropriate.</li>
<li>Reviews the work of subordinates for completeness and accuracy; evaluates performance and makes recommendations as appropriate; offers advice and assistance as needed.</li>
<li>Provides for adequate staff training and development opportunities.</li>

<li>Documents death scenes by photographing and/or sketching the deceased and the death scene as necessary.</li>
<li>Determines identity of decedent and establishes date and time of death.</li>
<li>Makes personal notification to the next-of-kin as necessary; provides information and assistance to family members of the deceased as appropriate.</li>
<li>Interviews family members, witnesses and attorneys; consults with medical personnel, law enforcement personnel, pathologists, toxicologists, pharmacists, entomologists, anthropologists, toxicologists, dental professionals, fire department personnel, laboratory personnel, photographers and other forensic specialists as necessary to gather information and assistance for the conduct of death investigations.</li>
<li>Issues subpoenas for the collection of medical records or other evidence as necessary.</li>
<li>Determines what type of examination is necessary; coordinates and schedules autopsies, post-mortem examinations, and the collection of body fluids and tissues as needed for analysis.</li>
<li>Collects, transports, stores, submits and interprets evidence.</li>
<li>Occasionally performs post-mortem examinations after a body has been removed from the scene of death; may collect body fluids and other relevant evidence from the body as necessary.</li>
<li>Completes death certificates and a Coroner’s report describing the death.</li>
<li>In cases of death that do not require a response to the scene, obtains all essential information from reporting person to complete an official Coroner’s report.</li>
<li>Collects, secures and returns the decedent’s personal effects to the legal next-of-kin.</li>
<li>Provides court testimony regarding cases as required.</li>
<li>Issues burial, removal, transit and cremation permits as appropriate.</li>
<li>Coordinates and ensures proper implementation of all applicable OSHA safety requirements; conducts annual blood-borne pathogens in-service training for department staff as required.</li>
<li>Procures and maintains inventory of necessary department supplies, materials and equipment.</li>
<li>Maintains assigned vehicles and equipment.</li>
<li>Remains on call 24 hours per day, seven days per week.</li>
<li>Coordinates department activities and functions, including death investigations, with those of other County departments, municipalities and outside agencies as appropriate.</li>

<li>Just like families, disabilities come in different types. You will see many of them along with the mental health challenges in the Individuals (aka the Program participants) that we serve. You will also learn about the different agencies and resources we work with to help you provide support directly, or indirectly, to the Individuals. </li>
</ul>
There will be chores:
<ul>
<li>You will work with Individuals and their “circle of support” (social agencies, health care providers, etc.) and family members to ensure that our behaviorally challenged and/or medically fragile Individuals can develop and/or maintain their own homes and daily routines. You do this by providing first-hand training and developmental support across a broad spectrum of activities, e.g., daily living skills, mobility training, developing coping skills, navigating the community, money management, safe, self-medication management and other customized interventions based on the Individual’s Service Plan. </li>
<li>You will also take care to document and complete all reporting in an accurate and timely manner and engage in proactive and open communications with all key stakeholders, including recommendations for modifications, alternative training methods or need for adaptive equipment, etc. to ensure the personal care and safety of your assigned Individual(s).</li>
</ul>
There will be chats, C.A.R.E., &amp; cheers: 
<ul>
<li>You will participate in all required training, team, and company meetings to build your knowledge, skills, and abilities to grow in the family. We like to promote from within our own house!</li>
<li>You will take ownership of your own behaviors, and help coach others over time, in how to effectively demonstrate the company’s C.A.R.E. values (compassion, adaptability, respect, and empowerment).</li>
<li>You will receive ongoing coaching and performance feedback and be recognized for your personal and team contributions in addition to demonstrating the C.A.R.E. values.</li>
</ul>
Related Experience: in the following fields a plus: Assisted Living, Behavior Aide, Caregiver, Certified Nursing Assistants, CNAs, Developmentally Disabled, DD, Job Coach, Direct Support Professional, DSP, Child Care, Elderly Care, Senior Care, Hospice Care, ILS, LVN, Autism, Special Education, Special Needs, ILS, SLS, Housekeeper, Driver, Cleaner, Home Health Aides (HHAs), Residential Care Facility Staff, In-home support, Med Tech, Personal Care Attendant, Day Program Staff or Adult Day Program.
