Purpose of the Position: 
The Social Service Supervisor is responsible for ensuring Case Managers working in shelters provide quality services to families in accordance with all the Department of Homeless (DHS) services, the NYS Office of Temporary Disability Assistance (OTDA), and Win requirements. The Social Service Supervisor will provide supervision to Case Managers using Evidence Based Practices (EBP), Motivational Interviewing (MI), Trauma Informed Care (TIC), and boundary setting. 

Responsibilities:

Provide comprehensive case management including individual and group counseling services and other appropriate support services to clients in the absence of the Case Managers.

Supervise Case Managers and provide coaching, training, and staff development as needed.

Job Description

<li>Conduct an admission interview with the resident and family for the purpose of completing a confidential psychosocial history and assessment, identify areas of concern, develop treatment plans/goals, and complete advance directives.</li>
<li>Update residents’ charts/social services records every three months following initial evaluation and/or as social services needs are demonstrated. Re-evaluate resident care plans at least every three months.</li>
<li>Maintain periodic contact with families to keep them informed of residents’ needs.</li>
<li>Assist residents with clothing purchases, obtaining telephone and other items as needed. Facilitate repair of prostheses and arrange for transportation.</li>
<li>Advocate residents’ rights according to Title XXII.</li>
<li>Consult with appropriate staff regarding resident needs and concerns providing resources.</li>
<li>Assist with discharge planning in consultation with Supervisor and Director of Nursing.</li>

POSITION SUMMARY: 
The Social Service Designee plans, develops, organizes, evaluates and directs the overall operation of the Social Services department maintaining compliance with federal, state, and local guidelines and regulations, Masonic Homes policies and procedures, and standards of care for specialty practice. DUTIES AND RESPONSIBILITIES

<li>Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate social service interventions.</li>
<li>Provides guidance with end of life decision making, facilitating communication between patients, families, providers, the IDT and Physicians, supporting a compassionate approach to the process of transitioning to palliative care and hospice.</li>
<li>Work closely with the facility mental health provider to ensure that all resident psychosocial needs are identified, referrals are made, and services are provided.</li>
<li>Ensure or provide therapeutic interventions, support and education to assist residents/family members/significant others in coping with their transition and adjustment to a long-term care facility, including their social, emotional, and psychological needs.</li>
<li>Maintain contact with the resident, legal representative, and other family members approved by the resident or legal representative, involving them in the resident’s total care and providing them with information on the resident’s status as requested or appropriate.</li>
<li>Coordinate resident room changes.</li>
<li>Lead facility behavior management committee. Coordinate care with social worker, psychologist and/or psychiatrist as needed.</li>
