As a Home Health Physical Therapist, you will:

<li>Develop Treatment Plans: Create and implement therapy programs aimed at improving daily living and work skills.</li>
<li>Provide Therapeutic Interventions: Utilize evidence-based techniques to enhance patients' independence and quality of life.</li>
<li>Educate Patients and Families: Offer guidance on adaptive equipment and home modifications to ensure a safe environment.</li>
<li>Collaborate with Healthcare Team: Work closely with physicians, nurses, and other therapists to coordinate comprehensive care.</li>
<li>Document Progress: Maintain accurate records of patient evaluations, treatment plans, and outcomes.</li>

<li>Licensed Professional: Active license and registration New York State.</li>
<li>Experience: experience in physical therapy; home health experience is preferred.</li>
<li>Communication Skills: Excellent interpersonal skills to effectively interact with patients, families, and team members.</li>
<li>Reliability: Strong organizational skills and the ability to work independently in a home setting.</li>
