Responsibilities:

Qualifications:

<li>Interest in psychiatry, functional medicine, psychedelic medicine, and longevity-focused treatments.</li>
<li>Certification in BLS/ACLS (required).</li>
<li>Passion for patient-centered, holistic care with an emphasis on mental health and wellness.</li>
<li>Strong interpersonal skills and the ability to work in a collaborative, patient-focused environment.</li>

<li>Monitor and treat patients during pregnancy, including prenatal advising and postnatal care.</li>
<li>Prescribe or administer therapy, medication, and other specialized medical care to treat or prevent illness, disease, or injury.</li>
<li>Collect, record, and maintain patient information, such as medical histories, reports, and examination results. Maintain adequate and properly documented records per COMC policy.</li>
<li>Explain procedures and discuss test results or prescribed treatment with patients and/or their family members as designated on patient HIPAA forms.</li>
<li>Conduct follow-ups with patients to monitor and track their condition.</li>
<li>Provide health promotion information and teaching thereof.</li>
<li>Make referral to other health professionals and agencies when necessary.</li>
<li>Assist in the further education of health professionals rotating through the clinics.</li>
<li>Assist with the periodic revision of health maintenance protocols, illness protocols, counseling protocols with other providers.</li>
<li>Continue personal professional education as required to maintain licensure</li>
<li>Participate in all COMC Quality Improvement policies and activities.</li>
<li>Maintain collaboration with a presiding physician</li>
<li>Perform other related duties as assigned.</li>

Qualification Requirements: Must be currently licensed as a Registered Nurse in Missouri and be currently registered as a Certified Registered Nurse Practitioner. 
Education and/or experience: Requires a minimum of a Master of Science inNursing(MSN). Requires experience working in Obstetrics.
Language Skills: Ability to respond to common inquiries or complaints from customers and regulatory agencies. Ability to effectively present information to top management.

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Morehead, KY.

Position Summary 
Qualified Nurse Practitioners interested in this position will have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.  
Nurse Practitioner Responsibilities 
<ul>
<li>Travel to and from patients’ homes. </li>
<li>Perform comprehensive, non-invasive physical assessments. </li>
<li>Review medical history and ensure accurate and timely documentation. </li>
<li>Conduct medical reconciliation to ensure patient safety and compliance. </li>
<li>Provide basic patient education on wellness and preventative healthcare measures. </li>
</ul>
Nurse Practitioner Qualifications and Experience Requirements:  
<ul>
<li>Master’s Degree OR commensurate experience and satisfactory completion of NP license </li>
<li>Active State NP license </li>
<li>Active National board certification </li>
<li>Current BLS certification </li>
<li>12 months of experience as a paid advanced clinical provider is preferred but not required </li>
