Position Summary:
Full-time Social Services Specialist position available for Sea Mar's MSS Department in Seattle, WA.&nbsp;The role of the Social Service Specialist is to conduct an initial screening and provide a comprehensive integrated clinical psychosocial assessment; develop intervention plans based on that assessment; provide brief treatment toward the care plan; provide transition services for long term counseling/treatment and Infant Case Management; and consult with the interdisciplinary Maternity Support Services team on the needs of clients served. The productivity expectation for this position is 20 or more client encounters per week for full-time work. Duties and Responsibilities:

<li>Introduces clients to integrated MSS services (either in home or at clinic) and performs systematic social services screening to determine current status and needs in a variety of areas.</li>

This assessment is shared with the client and team members to assure that all relevant psychosocial issues are addressed in the care planning.</li>
<li>Participate fully in all interdisciplinary team meetings to provide input on client’s assessed psychosocial needs and on interdisciplinary team plans for client. The goal is to build family strengths and resiliency.</li>

Qualifications and Experience:
<ul>
<li>Bachelor’s degree from an accredited university and current Certified Counselor through the State of Washington Department of Health or Chemical Dependency Counselor.&nbsp;</li>
<li>Experience working with underserved populations such as, but not limited to, those who have&nbsp; limited English speaking skills, are homeless, and migrant and seasonal workers. If the patient&nbsp; requires services beyond brief intervention, the Social Service Specialist is responsible for&nbsp; coordinating the timely referral to behavioral health services.</li>
<li>Experience working with patients who have substance use disorders, chronic mental illness, and&nbsp; those who require crisis intervention.</li>
<li>Experience working with safety-net providers within the community and broad knowledge of&nbsp; community resources to facilitate whole person care.&nbsp;</li>
<li>Knowledge of evidence-based screening tools (PHQ 9, GAD 7, AUDIT, DAST, etc.) and the&nbsp; ability to use these tools as part of a framework to make appropriate level of care interventions&nbsp; and referrals for patients as needed.</li>

The CCS Individual Skill Development program uses goal-oriented interventions to support youth who are struggling in various aspects of their life, including social skills, emotional regulation, anger management, independent living skills, and behavior management.
The Safety Services, Mobile Response, and Supervised Parenting Time programs work with families referred to our services by the Outagamie County Department of Health and Human Services to maintain safety in homes where children are at-risk for removal or are in the process of reunification. Position Summary
Under direction of Director of Social Services, assists each resident and family in adjusting to placement, illness, and plan of care to attain the highest practicable level of functioning in accordance with current federal and state regulations, and Company policies and procedures.
 
Duties and Responsibilities:
<ul>
<li>Assists in interviewing and obtaining information from residents and families to assist Social Services Director or Social Services Consultant in development of psychosocial history, assessment and care plan upon admission, quarterly and upon change of condition for social service needs.</li>
<li>Assists resident and family with transition and adjustment to placement in facility.</li>
<li>Assists with determination of legal status of residents upon admission, provides information and obtains copies of advance directives, assures that resident and family wishes regarding treatment (including Do Not Resuscitate (DNR) and withholding treatment) are implemented in accordance with Company policy.</li>
<li>Informs the resident/family of resident's personal and property rights.</li>
<li>Provides input on social, psychological, and emotional needs of residents/family to supervisor.</li>
<li>Ensures residents have required evaluations for visual, audio, mental health and dental needs, and follows up as appropriate.</li>
<li>Arranges residents' appointments including transportation.</li>
<li>Provides assistance to residents and families in finding and utilizing financial, legal, mental health, and other community resources.</li>
<li>Participates in resident and family council meetings as requested.</li>
<li>Develops a working knowledge of federal and state regulations, as related to position, and reimbursement (Medicare, Medicaid and commercial) and provides support with applications and eligibility information requests.</li>
<li>Provides support with resident theft and loss prevention program for the facility.</li>
<li>Works closely with Ombudsperson to resolve resident rights questions or concerns.</li>
<li>May perform documentation duties in accordance with Company charting and documentation policies and procedures and government regulations. Ensures documentation is accurate, clear, informative and descriptive of resident's condition, care provided and residents' response to care.</li>
<li>Assists in discharge planning services including arrangement for follow-up services, transfers to other facilities, and post discharge plan of care, as well as orientation for resident and family regarding discharge.</li>
</ul>
 
Qualifications:

<li>Previous experience with geriatric population or in long-term care setting strongly preferred</li>
<li>Bachelor's degree required</li>

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (<a href="http://www.westhealth.org" target="_blank">www.westhealth.org</a>), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

Position Summary:
Under the direction of the Center Director, is responsible for coordinating and supporting all aspects of the Social Services Department.
Essential Functions:
<ul>
<li>Participate as a supportive member of the interdisciplinary team (IDT), tracking all components and assigned tasks associated with the Social Services Department and assigned IDT MSW.</li>
<li>Attend morning IDT meetings and report status of tasks and assignments pertaining to the coordination of social service activities for all assigned participants.</li>
<li>Coordinate and manage the MSW schedule for in-person assessments including change of conditions, service delivery request/denial assessments, care conferences, and higher level of care assessments and committee meetings.</li>
<li>Prepare the MSW for assessments, conferences, higher level of care meetings.</li>
<li>Prepare the MSW for and attend care planning meetings with IDT.</li>
<li>Present care plans to participants and obtain signatures to be uploaded to the electronic medical record.</li>
<li>Administer psychometric scales as needed for the MSW - including PHQ-9 and GAD-7.</li>
<li>Work closely with participants and families to establish and maintain exemplary rapport and ongoing relationships to provide the highest quality of care in a person-centered, trauma-informed manner.</li>
<li>Coordinate ongoing family meetings, as needed.</li>
<li>Monitor the individual interventions prescribed in the plan of care and update medical record regularly.</li>
<li>Alert SW/BH staff of crises that arise in the day center or in the community.</li>
<li>Maintain current, accurate, professionally written case management records submitted within 72 hours of all encounters pertaining to a participant's care. This includes ongoing documentation of all communication, services provided, participant expressed wishes, and statuses of all.</li>
<li>Act as liaison between the participant and other agencies such as Department of Aging, Social Security Administration, Medicaid, etc.</li>
<li>Alert the MSW to needed referrals to community or specialized programs when indicated.</li>
<li>Assist with obtaining and maintaining current statements of income and other assets.</li>
<li>Participate in participant-related conferences in the community as designated.</li>
<li>Maintain confidentiality of participant information.</li>
<li>Attend monthly meetings with supervisor.</li>
<li>Attend bi-weekly mandatory department meetings.</li>
<li>Attend and participate in all staff meetings, in-services, projects, and committees as assigned.</li>
<li>Adhere to and support the center's policies, practices, and procedures.</li>
<li>Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.</li>
<li>Be flexible in the schedule of hours worked.</li>
<li>May require use of personal vehicle.</li>

Education and Certification:
<ul>
<li>High School Diploma or GED</li>
<li>Current Basic Life Support (BLS) certification required.</li>
<li>Current California Driver's License with proof of auto insurance.</li>

Skills and Experience:

<li>A minimum of one year of experience working with a frail or elderly population required.</li>
<li>A minimum of one year of experience working in a behavioral health or social services setting.</li>
<li>Must maintain excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers as needed for the position.</li>
<li>Skilled in establishing and maintaining effective working relationships with participants, co-workers, vendors, medical staff, and the public.</li>
<li>Must be able to demonstrate proficiency in Microsoft computer applications. Experience working with Electronic Medical Record system (EMR) preferred.</li>
<li>Operates in full compliance with all applicable federal, state, and local fire, health, safety, sanitation, Emergency procedures and other Standards prescribed in law or regulations.</li>
<li>Desire to work in the helping field with an emphasis of the geriatric population.</li>
</ul>
