Salud’s Ambulatory Care Clinical Pharmacy Specialists (CPS) are the medication-use experts on Family Medicine patient care teams and provide comprehensive medication management. The CPS approaches his/her tasks in a team-based fashion that supports patients and their families in self-management, self-efficacy, and behavior change. Specific Duties:

<li>Operates within the clinical scope and supervision of the clinical pharmacy Collaborative Drug Therapy Management Protocol, as well as all clinical pharmacy Standard Operating Procedures</li>
<li>Engages in providing clinical care services in ambulatory care, both in person and through telehealth models&nbsp;&nbsp;</li>
