Qualifications:

<li>Current and valid COTA certification.</li>
<li>Minimum of 1-2 years of experience as a Certified Occupational Therapy Assistant, preferably in home health.</li>

Care manager will conduct assessments to identify member’s and family’s needs. Requirements

<li>Registered Nurse or Licensed Practical Nurse with one year of experience</li>

Additional Requirements

Minimum Qualifications:
