The Department of the Air Force (DAF) Family Advocacy Program (FAP) is designed to identify, prevent, provide treatment to families, couples, and/or intimate partners impacted by domestic abuse (DA). FAP offers the support of Domestic Abuse Victim Advocates (DAVA) to provide DAF personnel, their family members, or intimate partners who are victims of DA (sometimes referred to as domestic violence) non-clinical emergent and urgent service whenever requested. With the exception of mandatory state, federal, and military reporting requirements (i.e., domestic violence, child abuse, and duty to warn situations) the DAVA provides a private and confidential service to encourage victims in seeking assistance.
 Responsibilities 
<ul>
<li>Provides 24 hour/7 days a week response to victims alleging DA by publishing and maintaining an emergency contact number via cell phone provided by vendor.</li>
<li>Establishes a Victim’s Safety Plan on a case-by-case basis.</li>
<li>Partners with the Family Advocacy Treatment Manager (FATM) and Family Advocacy Intervention Specialist (FAIS) or mental health on-call provider if the treatment or case manager is unavailable, to establish safety plans. The victims’ safety is the DAVA’s top responsibility and priority.</li>
<li>Review the safety plan during each victim contact.</li>
<li>Develops safety plans based on initial and ongoing risk assessments. If imminent risk of serious harm or death is established, notify the FAO, law enforcement, and Command. If children are involved, the process also includes child physical safety and emotional well-being; refer involved children to the treatment or case manager for follow-on care.</li>
<li>Immediately reports any changes in the victim’s circumstances that changes or impacts the safety plan to the FAP treatment/case manager or mental health on-call provider if the treatment/case manager is unavailable. When determining whether a victim is at imminent risk of serious harm or death, the DAVA will assess the following risk factors:
<ul>
<li>Victimization patterns have increased in severity or frequency.</li>
<li>Alleged offender threatened or attempted to kill the victim or his/her children.</li>
<li>Alleged offender threatened or attempted suicide.</li>
<li>Alleged offender strangled the victim.</li>
<li>Alleged offender used or threatened to use a weapon against the victim.</li>
<li>Victim sustained serious injury during the abusive incidents.</li>
<li>Prior police contact with the alleged offender regarding DA.</li>
<li>Victim has a restraining or protection order against the alleged offender.</li>
<li>Victim is estranged, separated or attempting to separate from the alleged offender.</li>
<li>Alleged offender stalked the victim.</li>
<li>Alleged offender abuses alcohol or drugs.</li>
<li>Alleged offender forced sex on the victim.</li>
<li>Alleged offender abused victim during current pregnancy.</li>
<li>Victim expresses fear of imminent serious harm or death.</li>
<li>Alleged offender has active psychosis or mania.</li>
<li>Alleged offender is using psychoactive drugs, such as amphetamines or cocaine.</li>
<li>Alleged offender exhibits obsessive behavior, extreme jealousy, or extreme dominance.</li>
</ul>
</li>
<li>Advises each victim at the initial contact that:
<ul>
<li>DAVA services are voluntary.</li>
<li>DAVA have limited confidentiality IAW AF policy.</li>
<li>Medical examination and documentation of victim’s injuries is highly recommended.</li>
<li>Victims may choose to make a restricted or unrestricted report of DA (those options are fully explained by the DAVA)</li>
</ul>
</li>
<li>Advises victims of the military or civil actions available to promote safety (e.g., military order of protection, restraining order, and injunction).</li>
<li>Offers victims information regarding their identified needs (e.g., emergency shelter, housing, childcare, legal services, clinical resources, medical services, transitional compensation).</li>
<li>Offers follow-up DAVA services to each victim:
<ul>
<li>Empower the victim to advocate for the needs of self and children.</li>
<li>Support the victim in decision-making by exploring options.</li>
<li>Assist the victim with prioritizing actions and establishing short/long-term goals.</li>
<li>Provide information and referral on military and civilian resources.</li>
</ul>
</li>
<li>Advises victims of the impact of domestic violence on children and supports victim’s efforts to have children assessed and treated, as needed</li>
<li>Accompanies the victim to appointments or court proceedings when requested by the victim.</li>
<li>Assistance with transportation is typically not provided to victims, however, after consultation with the FAO and when no other reasonable means exist the DAVA may transport the victim to important appointments (i.e.: medical, court, protective actions). The DAVA cannot transport minor children unless accompanied by the parent or legal guardian and has the appropriate safety restraints in the vehicle. The government will not be responsible for any costs or liabilities if the DAVA elects to provide transportation for victims except as identified in the contract.</li>
<li>Collaborates with the FAP treatment/case manager to support the victim and promote safety for the victim and children in the home.</li>
<li>Briefs the treatment/case manager prior to the Clinical Case Staffing (CCS) so that the CCS team has the most current information on the victim and children for staffing. (Note: The DAVA does not have access to information containing Personal Health Information (PHI) and does not attend the CCS).</li>
<li>Establishes a contact file for each victim served that contains minimal information about the allegations or nature of the incident. The primary purpose of the DAVA contact file is to maintain victim’s name and contact information as well as a log of the victim contacts and nature of the contacts or service provided by the DAVA. Information in the DAVA contact file will assist the DAVA in maintaining contact with the victim and will provide continuity of care in the event of DAVA position turnover.</li>
<li>Place the safety plan, Victim Impact Statement and Victim Preference Statement in the DAVA contact file. Provide a copy of items to the treatment/case manager for the FAP Record.</li>
<li>Maintain DAVA contact files in the FAP office under a double lock system and/or DAVA database.</li>
<li>The contact file will be clearly marked as either “Open” or “Closed” indicating whether the victim is currently receiving DAVA services.</li>
<li>Submits Victim Impact Statement when the victim asks the DAVA to inform the alleged offender’s commander, the FAP treatment/case manager, or the FAO (for presentation to the Central Registry Board) of the impact the maltreatment has had on the victim and/or children. (Note: This form is only required when the victim requests the DAVA share the impact information on the victim’s behalf</li>
<li>Enters victim contact information and perform other activities monthly IAW DAF FAP guidance.</li>
<li>Develops process to keep the FAO and designated POC (e.g., on-call mental health provider, emergency room.) informed of DAVA’s location and timelines when providing DAVA victim services. This process will include purpose, location, arrival, and departure notification.</li>
<li>Supports the Family Advocacy Intervention Specialist (FAIS) to develop System Advocacy, Education and Public Awareness, promoting a coordinated community response to DA. As a system advocate, the DAVA shall:
<ul>
<li>Continually evaluate the quality of the installation’s coordinated community response and collaborate with base agencies to improve the system response to victims.</li>
<li>Empower victims to be involved in plans or decisions about the safety of self and children.</li>
<li>Collaborate and establish protocols with Security Forces Squadron (SFS) and Office of Special Investigations (OSI) confirming:

<li>24-hour notification of the DAVA in all incidents of suspected DA</li>
<li>Collaboration on safety planning</li>

<li>Partners with the Family Advocacy Nurse (FAN) when the FAN is involved in open partner maltreatment cases.</li>
<li>Promptly communicates with the treatment/case manager and the FAN any information that may impact the victim’s current safety plan.</li>
<li>Refers victim for direct service from the FAN through the FAO.</li>
<li>Shares victim information with the FAN at least monthly to ensure continuity of care between the FAN and DAVA services.</li>
<li>Partners with the FAIS to promote education about and prevention of DA. Such activities may include outreach programs designed to increase awareness, educational briefings (e.g., restricted reporting option), skill-building classes, and the development of marketing tools and strategies.</li>
<li>An invited member of the Outreach Prevention Management (OPMT), the DAVA shall make every effort to attend OPMT meetings to promote the DAVA-FAIS partnership and the DAVA involvement in DA prevention activities.</li>

Greater Norwalk Head Start / Early Head Start a program of the Connecticut Institute For Communities, Inc. seeks full-time Social Services and ERSEA Professional. Position is responsible for assuring that programs meet 100% of the related Federal Performance Standards, including services to families and children, working closely with Management team and Teaching staff, and Parent Education as a whole or on a case-by-case basis. This position includes supervising Family Advocates and participant enrollment. This position is chiefly responsible for the following core functions of the Head Start program and performs other tasks as needed: 
 
Summary:
(1) Social Services Manager: This position has responsibility for fulfillment of the following Head Start Performance Standards: Family Partnerships; Community Partnerships, and Parent Involvement; Program Planning and the related Federal Monitoring Protocols. Directly supervises the Family Services Team (3–4-person team). 
 
(2) ERSEA Manager: This position is responsible for fulfillment of the following Head Start Performance Standard: Eligibility, Recruitment, Selection, Enrollment, and Attendance (ERSEA)
 
This position is chiefly responsible for the following functions and performs other tasks as needed:

Requirements:

<li>2-5 years of relevant employment experience.</li>
<li>Experience with social services and/or education in mental health services. </li>
<li>Bilingual fluency (English/Spanish) strongly preferred.</li>
<li>Ability to complete physical examination, background check, and fingerprinting screen requirements</li>
<li>Able to lift a child weighing 40 pounds, 20 times a day if needed</li>
<li>Driver’s license required</li>
<li>Basic computer literacy in email, word processing and internet navigation </li>

Position Summary:
Full-time Social Services Specialist position available for Sea Mar's MSS Department in Federal Way, WA.&nbsp;The role of the Social Service Specialist is to conduct an initial screening and provide a comprehensive integrated clinical psychosocial assessment; develop intervention plans based on that assessment; provide brief treatment toward the care plan; provide transition services for long term counseling/treatment and Infant Case Management; and consult with the interdisciplinary Maternity Support Services team on the needs of clients served. The productivity expectation for this position is 20 or more client encounters per week for full-time work. Duties and Responsibilities:
<ul>
<li>Introduces clients to integrated MSS services (either in home or at clinic) and performs systematic social services screening to determine current status and needs in a variety of areas.</li>
<li>Based on identified client needs, makes referrals to appropriate agencies/services or provides information and assists clients for self-referral.</li>
<li>Provides advocacy as needed to encourage participation in implementing the care plan or to resolve problems which are interfering with active participation in the care plan (such as transportation or language barrier).</li>
<li>Charts notes of the services provided as required by the state and agency in Sea Mar’s Electronic Health Records.</li>
<li>Assists client in accomplishing tasks necessary to access services such as filling out appropriate forms, obtaining necessary documentation or authorization.</li>
<li>Maintains professional standards and follows the treatment policies and procedures of the agency, as well as applicable federal, state and local regulations.&nbsp;</li>
<li>Complete a thorough, culturally sensitive, psychosocial assessment which includes but is not limited to: Personal history (cultural background, health status, coping strategies, mental health status, substance use, abuse and violence); current family functioning; review of support systems (formal and informal); and basic physical needs. This assessment is shared with the client and team members to assure that all relevant psychosocial issues are addressed in the care planning.</li>
<li>Participate fully in all interdisciplinary team meetings to provide input on client’s assessed psychosocial needs and on interdisciplinary team plans for client. The goal is to build family strengths and resiliency.</li>
</ul>
Qualifications and Experience:
<ul>
<li>Bachelor’s degree from an accredited university and current Certified Counselor through the State of Washington Department of Health or Chemical Dependency Counselor.&nbsp;</li>
<li>Experience working with underserved populations such as, but not limited to, those who have&nbsp; limited English speaking skills, are homeless, and migrant and seasonal workers. If the patient&nbsp; requires services beyond brief intervention, the Social Service Specialist is responsible for&nbsp; coordinating the timely referral to behavioral health services.</li>
<li>Experience working with patients who have substance use disorders, chronic mental illness, and&nbsp; those who require crisis intervention.</li>
<li>Experience working with safety-net providers within the community and broad knowledge of&nbsp; community resources to facilitate whole person care.&nbsp;</li>
<li>Knowledge of evidence-based screening tools (PHQ 9, GAD 7, AUDIT, DAST, etc.) and the&nbsp; ability to use these tools as part of a framework to make appropriate level of care interventions&nbsp; and referrals for patients as needed.</li>
<li>Good working knowledge and skills in using electronic health record platforms, external&nbsp; registries, as well as the Microsoft suite of programs in order to document patient services,&nbsp; develop and run reports, etc.&nbsp;</li>
<li>Bilingual in English/Spanish is preferred, but not required. Must be open to using an interpreter. &nbsp;</li>

PRIMARY FUNCTION/PURPOSE

The Social Services Supervisor is responsible for overseeing a team of Case Managers to ensure the support and resources necessary to promote self-sufficiency and independent living skills for families’ w/children residing in temporary shelter are being provided in accordance with CRF, DHS and OTDA standards.
PRIMARY RESPONSIBILITIES

<ul>
<li>Ensure document folder in CARES (VDP, Client rules and regulations, Crib acknowledgment, ACS Video, etc.) is completed and documented immediately upon client arrival</li>
<li>Provide field orientation and on the job training to Case Managers</li>
<li>Create and execute plans for meeting contracted ILP completion and placement goals</li>
<li>Ensure all Case Managers complete mandated CRF, DHS, and OTDA tasks in accordance with prescribed timeframes and mandates</li>
<li>Ensure Case Managers are providing consistent services in accordance CRF, DHS and OTDA policies and procedures</li>
<li>Monitor Case Managers work activities via CARES reviews, physical chart reviews, and spreadsheets</li>
<li>Conduct individual and group supervisions with Case Managers. Provide staff with documented feedback of performance, including areas of strength and areas in need of development</li>
<li>Conduct performance evaluations of Case Managers as per CRF, DHS, and OTDA policies and professional standards</li>
<li>Collaborate with Social Services team to develop service plans to address clients’ needs related to housing, substance abuse treatment, mental health and/or employment, etc</li>
<li>Meet with Senior Management (PD and APD) for individual and group supervision</li>
<li>Assist families with identifying appropriate housing resources and assist application process as needed</li>
<li>Monitor demographic report to better guide Case Managers and ensure program compliance with CRF, DHS, and OTDA expectations</li>
<li>Attend and moderate Case Conferences with Case Managers and challenging, noncompliant and/or housing ready clients</li>
<li>Establish linkage with community service providers to obtain housing, health, and social services for clients</li>
<li>Assist in facilitation of intake, discharge, transfers, and placement into permanency as needed</li>
<li>Complete ILP’s and other Case Management related tasks as needed</li>
<li>Generate and distribute reports (i.e., exit projection, quarterly projection, case summaries)</li>
<li>Support in the acquisition of housing vouchers and placements</li>
<li>Deliver crisis management and intervention services to clients</li>
<li>Support efforts to place families into permanent housing</li>
</ul>

MINIMUM QUALIFICATIONS

<ul>
<li>Bachelor’s degree in Social Work required</li>
<li>At least two (2) years of experience in a related field. Experience with multi-problem families desirable</li>
<li>Bilingual in Spanish</li>
<li>Demonstrated knowledge of and proficiency in CARES</li>
<li>Demonstrated experience working in shelter setting and/or with high-risk families, including families with documented histories mental health, substance abuse, Domestic Violence, child abuse/neglect</li>
<li>Ability to work effectively with a wide range of community-based providers</li>
<li>Working knowledge of Case Management theories and approaches</li>
<li>Knowledge of community health services, social service support agencies and networks</li>
<li>Familiarity with DHS/OTDA social service documentation policies and procedures</li>
<li>Critical thinking, analytical and problem-solving skills as well as resourcefulness</li>
<li>Demonstrated ability to be flexible, work independently as well as with a team</li>
<li>Familiarity with housing vouchers (i.e., Section 8, FEPS, SEPS, LINC, FHEPS, SOTA, EOSD, City FHEPS)</li>
<li>Familiarity with SOI (Source of Income Discrimination) and Human Rights Law regulations</li>
<li>Ability to assess needs and assist residents in developing housing plans for promotion of self-sufficiency and permanent housing</li>
<li>Excellent analytical, evaluative, and human service management skills</li>
<li>Strong organizational and leadership skills</li>
<li>Supervisory experience with a team-oriented approach</li>
<li>Must have incredible initiative, the ability to work well under pressure</li>
