Position Summary:
Under the direction of the Behavioral Health Director, is responsible for coordinating and supporting all aspects of the Social Services Department.
Essential Functions:

<li>Participate as a supportive member of the interdisciplinary team (IDT), tracking all components and assigned tasks associated with the Social Services Department and assigned IDT MSW.</li>
<li>Attend morning IDT meetings and report status of tasks and assignments pertaining to the coordination of social service activities for all assigned participants.</li>
<li>Coordinate and manage the MSW schedule for in-person assessments including change of conditions, service delivery request/denial assessments, care conferences, and higher level of care assessments and committee meetings.</li>
<li>Prepare the MSW for assessments, conferences, higher level of care meetings.</li>
<li>Prepare the MSW for and attend care planning meetings with IDT.</li>
<li>Present care plans to participants and obtain signatures to be uploaded to the electronic medical record.</li>
<li>Administer psychometric scales as needed for the MSW - including PHQ-9 and GAD-7.</li>
<li>Work closely with participants and families to establish and maintain exemplary rapport and ongoing relationships to provide the highest quality of care in a person-centered, trauma-informed manner.</li>
<li>Coordinate ongoing family meetings, as needed.</li>
<li>Monitor the individual interventions prescribed in the plan of care and update medical record regularly.</li>
<li>Alert SW/BH staff of crises that arise in the day center or in the community.</li>
<li>Maintain current, accurate, professionally written case management records submitted within 72 hours of all encounters pertaining to a participant's care. This includes ongoing documentation of all communication, services provided, participant expressed wishes, and statuses of all.</li>
<li>Act as liaison between the participant and other agencies such as Department of Aging, Social Security Administration, Medicaid, etc.</li>
<li>Alert the MSW to needed referrals to community or specialized programs when indicated.</li>
<li>Assist with obtaining and maintaining current statements of income and other assets.</li>
<li>Participate in participant-related conferences in the community as designated.</li>
<li>Maintain confidentiality of participant information.</li>
<li>Attend monthly meetings with supervisor.</li>

Position Summary
Under direction of Director of Social Services, assists each resident and family in adjusting to placement, illness, and plan of care to attain the highest practicable level of functioning in accordance with current federal and state regulations, and Company policies and procedures.

The Social Service Manager (SSM) position is responsible for the supervision of Caseworkers and assures that information and assistance needed by participants is available and accessible. The SSM reports to the Executive Director while working closely with leadership, program staff, volunteers and other community organizations.
The SSM oversees the development, implementation, monitoring and evaluation of all program policies and procedures related to the Caseworker department; assures the collection and reporting of program data is reported on a timely basis; builds and maintains an effective resource network to assure program clients receive appropriate referrals that address their nutritional and social needs; and develops and supervise wrap-around services that will enhance the service delivery to program participants (i.e. telephone reassurance program, information and resource directory, etc.).
ESSENTIAL JOB FUNCTIONS:

Work involves assisting clients/families with activities of daily living, advising of available resources, assisting in obtaining services from within the agency and other agencies, and providing transportation. Work may involve accompanying casework staff on visits to homes or institutions, assisting with the completion of forms and assisting with clerical duties. JOB SUMMARY:
The CCS Individual Skill Development program uses goal-oriented interventions to support youth who are struggling in various aspects of their life, including social skills, emotional regulation, anger management, independent living skills, and behavior management.
The Safety Services, Mobile Response, and Supervised Parenting Time programs work with families referred to our services by the Outagamie County Department of Health and Human Services to maintain safety in homes where children are at-risk for removal or are in the process of reunification. Staff within these programs emphasize helping families develop systems of support and resources to promote safety and stability, and enhance the parent-child relationship.
As the Supervisor, it will be your responsibility to direct and monitor all aspects of the SS, MR, CCS, and SV programs.
KEY RESPONSIBILITIES OF THE SUPERVISOR:

<li>Hire, train, and manage program staff.</li>
