The Raphael Community Free Clinic (RCFC) in Kerrville TX is seeking a part-time to fulltime (16 to 32 hours per week) Family Nurse Practitioner (FNP) Spanish speaking&nbsp;preferred to join our Primary Care team. In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to our patients. The clinical care team works with the provider daily to drive better outcomes for our patients, especially those with chronic disease. This will include accurate assessment, diagnosis, treatment, management of health problems, health education and counseling, and disposition planning. Seventy percent of our patients speak Spanish as a primary language.
QUALIFICATIONS
EDUCATION – Required: Completion of Master’s Degree level Family Nurse Practitioner program
EXPERIENCE – Required: Minimum 1 Year of related experience; Preferred: Two+ years or more of related experience.

Qualifications:

<li>Education: Master’s degree in Nursing from an accredited program.</li>

Job DescriptionClinic days/hours: 4 days/week The Oasis Center is an innovative clinic focused on providing holistic, trauma-informed care, integrating physical, behavioral, and social support to families affected by substance use disorders. The Oasis Center is a unique work environment that incorporates wide community partnerships, including employment support, transitional housing, peer mentors and Department of Human Services (SNAP, TANF benefits) into the care and stabilization of families. The Oasis Center tagline, More than a clinic, a community encompasses the spirit of support and connection the clinic provides. Family Nurse Practitioner (FNP) will work in a strong team-based environment to provide comprehensive primary and preventative care to patients of all ages, including diagnosing and treating illnesses, prescribing medications, and educating patients on health and wellness. The Oasis Center provides prenatal care and consequently has a large pediatric population, approximately 35% of the panel. Appropriate applicants will have a strong commitment to underserved communities and a desire to be part of an agency that approaches healthcare from a truly bio-psycho-social model. Although experience working with those with substance use disorders is preferred, there is strong physician mentoring and support for the position and a dynamic learning environment that will support the growth of the successful applicant, regardless of prior experience. Responsibilities: 
<ul>
<li>Conduct comprehensive medical assessments for both pediatric and adult patients</li>
<li>Provide evidence-based treatment for SUDs, including Medication-Assisted Treatment (MAT) for opioid and alcohol use disorders, when appropriate</li>
<li>Manage chronic and acute health conditions in both adults and children within families</li>
<li>Collaborate closely with behavioral health therapists, peer recovery mentors, and other wraparound staff to develop integrated care plans</li>
<li>Offer patient and family education on health promotion, harm reduction, relapse prevention, and parenting in recovery</li>
<li>Monitor patient progress and adjust care plans in collaboration with the interdisciplinary team</li>
<li>Participate in case conferences, team huddles, and care coordination efforts</li>
<li>Maintain accurate, timely documentation in the electronic health record</li>
<li>Engage in community outreach and health promotion as needed</li>
<li>Coding/Billing</li>

Qualifications:
