JOB SUMMARY:
As a licensed therapist specializing in mental health and/or substance abuse services in a school based setting, skill in performing work through observation, interviews, and tests, to get information about patients is required. Based on his/her observation, this professional must be able to determine the right type of treatment for the patient. Provide counsel with emphasis on prevention. Work with individuals and groups to promote optimum mental health. May help individuals deal with addictions and substance abuse; family, parenting, and marital problems; suicide; stress management; problems with self-esteem; and issues associated with aging and mental and emotional health.
Essential Job Duties and Responsibilities:
<ul>
<li>Provide individual, group, and/or family counseling to clients referred to you by SEIC intake and CCA team only.</li>
<li>Comply with Clinical Coverage Policy No. 8C NCAC and Clinical Coverage Policy No. 8-I.</li>
<li>Assure all progress notes are completed within 24 hours of services delivered.</li>
<li>Assure PCP/Treatment Plan is completed within 15 days of first service.</li>
<li>Provider will not conduct independent CCA, but provide care initiate PCP based on recommendations from CCA conducted by SEIC staff.</li>
<li>Comply with SEIC EMR requirements.</li>
<li>Comply with SEIC training requirements.</li>
<li>Assure all clients referred by SEIC with recommendations for Outpatient Therapy is supported and documented in your PCP/Individualized Plan of Care.</li>
<li>Assure all UM/medical necessity and authorization information is documented to substantiate medical necessity for outpatient behavioral health services, including appropriate ICD-10-CM diagnosis codes.</li>
<li>Clients will be scheduled through SEIC EMR scheduling system.</li>
<li>Clients will be referred to higher level of care as indicated and when medically necessary.</li>
<li>School based Therapist will see children within the scope of their licenses for substance use and behavioral health disorders.</li>
<li>Coordinate with primary care provider and SEIC case management team as required; Abide by all State and Federal regulations, legislation, or requirements that the Facility be subject to including HIPAA and 42 CFR Part 2.</li>
<li>Verify each Medicaid beneficiary’s eligibility each time a service is rendered.</li>
<li>Provide services under the overall direction and supervision of a physician licensed to practice medicine or osteopathy, or other approved individuals (nurse practitioners, certified nurse midwives, physician assistants), who must be immediately available by phone or pager.</li>
<li>Ensure assessments include the child or adolescent and, as appropriate, the parent or caregiver.</li>
<li>Prepare and maintain all required treatment records and reports, including referral reason, assessment results, diagnosis, signed treatment plan, intervention details (date, duration, purpose, nature, outcome), provider signature and credentials, consults, follow-up, and releases of information.</li>

<li>Modify treatment activities and approaches as needed in order to comply with changes in clients' status.</li>
<li>Learn about new developments in the field by reading professional literature, attending courses and seminars, and establishing and maintaining contact with other community agencies.</li>
<li>Discuss with individual patients their plans for life after leaving therapy.</li>
<li>Gather information about community mental health needs and resources that could be used in conjunction with therapy.</li>
<li>Coordinate with prescribers regarding clients' use of medications.</li>
<li>Supervise other counselors, including provisionally licensed staff, professionals, and paraprofessionals.</li>

Primary Purpose of the Position:
Carolinas Creative Counseling, PA provides Outpatient Behavioral Healthcare services to children, adolescents and adults who are in need of clinical treatment. This service is to be provided by licensed professionals. Outpatient Therapy provides intensive clinical therapeutic services to children adolescents, and adults with identified mental health or substance abuse diagnosis. This service is designed to meet the specific mental health needs of the identified client as they relate to their diagnosis and can be provided in-home, in-office, in-school or other approved/appropriate community setting. Outpatient therapy includes psycho-educational activities/interventions designed to assist the client in the reduction of dysfunction. Services are provided with client-centered and family-centered approaches and are delivered through intensive, time-limited, goal specific interventions.
 
Description of Duties and Responsibilities:

<li>To deliver in-home or in-office services to clients as requested by the area mental health including the specific hours requested. These services may be provided in the school setting by therapists identified as members of the school based therapy unit if the clients presenting issues would be better addressed in that setting;</li>
<li>To provide therapeutic interventions to the identified client in order to enhance and foster the clients optimal functioning and interactions in the home and community;</li>
<li>To continually assess client needs, service availability and appropriateness;</li>
<li>To work cooperatively with treatment team members and the family in the provision of services to meet the goals developed in the Treatment Plan;</li>
<li>To provide the appropriate documentation for service delivery including treatment goals, progress notes and billing and submit said documentation as specified by local, state and federal standards.</li>

CORE FUNCTIONS 1. Provides comprehensive nutrition assessment of patients' nutritional needs, to include nutrition-focused physical exams as appropriate, with at least 50% of patient case load in designated specialty area. Obtains timely and appropriate data and analyzes/interprets data based on evidence-based standards and established practice guidelines. 2. Demonstrates and maintains competency in enteral and parenteral clinical practice protocols and oral nutrition management standard of practice as demonstrated by successful completion of initial competency exams and ongoing evaluation. 3. Writes orders to initiate and manage the nutrition care and of all assigned patients including those receiving nutrition support (parenteral and enteral nutrition) per protocols. 4. Evaluates effectiveness of the care plan and communicates effectively to the other members of the healthcare team to provide optimal nutrition outcomes. Provides pertinent information in the medical records, patient plans of care and/or clinical pathways while meeting requirements of regulatory agencies (i.e. Joint Commission, CMS, State and Local Agencies). 5. Provides nutrition education to patients, families, staff and other health care team professionals through the development of effective teaching materials and in-service programs; Evaluates patient and or family response to education. 6. Provides leadership in area of practice, establishing proper patient care procedures and acts as liaison with nursing areas. Establishes and monitors policies and procedures in area of expertise and communicates areas of concern that impact patient care in this area. Assists with program marketing through the development of effective work relationships with internal and external customers to enhance the use of services provided by clinical program. Serves as mentor to new staff and interns where assigned. 7. Participates in and identifies continuous quality improvement activities in clinical areas to ensure optimal nutrition care outcomes are achieved; Participates in clinical nutrition Continuous Quality Improvement (CQI) activities and completes one outcome study per year to improve on quality of nutrition service or to improve nutrition outcomes. Demonstrates proficiency in area of practice; Responsible for developing training and educational materials to assist colleagues in developing skills in area of specialty and mentors staff members in area of specialty. 9. This position interacts with patients, families, health care providers and nutrition and dietary staff in support of providing high quality nutrition assessment, care planning, delivery and measurement of outcomes. MINIMUM QUALIFICATIONS Must possess the knowledge of nutrition and dietetics demonstrated through the completion of a Bachelor's Degree in Nutrition and/or Dietetics. Must be a Registered Dietitian based on academic and professional requirements as defined by the Commission on Dietetic Registration (CDR) of The Academy of Nutrition and Dietetics. Effective January 1, 2024, CDR requires a minimum of Master's degree to be eligible to take the registration examination. Must hold current registration through the Commission on Dietetic Registration of The Academy of Nutrition and Dietetics and be able to maintain ongoing registration while providing verification of registration annually. Maintains current licensure as required by State law. Must possess a level of expertise typically achieved through three to five years clinical experience in a specialty area. Must be able to function independently and be effective at prioritizing assigned patient case load as well as the ability to lead and serve as a resource for issues. To be classified as a Senior Registered Dietitian, at least 50% of dietitian's case load must be in the specialty area.
