Role Description:
Resource assigned to this position may be requested to be professionally qualified to participate in the coordination, inspection, and evaluation of the therapeutic diet program and maintain adherence to HIPAA standards. Will perform duties in support and development of the goals and objectives of the Dietary Department or Food Services Division and in particular, for the integration of the Dietary service goals with the goals and needs of other clinical and support service departments in the facility. Resource will record and maintain dietary assessments and reassessments. Record progress notes of Medical records according to established standards. Maintain communication with nursing personnel related to residents on tube feedings, diabetics, weight gain or loss or any other nutritional related concerns and document these in the resident medical record. Chart the nutritional progress of each resident every 90 days or as medically needed. Inform physician of any nutritional changes. Ability to plan and modify standard and therapeutic diet menus in accordance with physicians prescriptions and standards of nutrition, cost, variety and quality. Provides guidance and direction to lower level dietetic staff performing work in the nutritional care for residents. Ability to provide special instructions to cooks and food service workers in the preparation of special diet food items. Knowledge of the modern methods, materials, practices and procedures used in a facility therapeutic diet program. Knowledge of facility food service safety and sanitation standards. Knowledge of the clinical and food service tasks performed by food service staff.. Knowledge of the procedures required in special diet tray assembling and distribution to insure the accuracy in quality of the menu items. Ability to integrate the nutritional program into the overall dietetic program. Resource will write nutritional care plans for implementation consistent with physician's orders and dietetic needs. Review care plans to ensure implementation. Resource will plan and modify standard and therapeutic menus for conformance to physician's orders/ diet manual, standards of nutrition, variety and quality. May be required to consult and work with a speech pathologist or other medical professionals. Resource may be required to attend and participate in interdisciplinary care plan meetings. Resource may provide in-service training to dietary and allied health care personnel as needed in applicable areas of nutrition. Resource will complete a nutritional discharge plan and resident instruction prior to discharge on all therapeutic diets. Resource will perform related duties as required by each facility. Resource should be able to provide nutritional assessment, guidance, direction and recommendation to institutional medical and food service staff regarding the nutritional care of the housed individuals in concert with the Agencies Bureau of Healthcare Services Food Service Division. Resource will perform other related duties as they are assigned by an Agency.

We are searching for our next SUPER STAR ! As the Registered Dietitian you will play an integral role in health behavior and interventions resulting in prevention of and/or improved management of chronic disease. IV. Technical Requirement
The dietician must provide evidence of being a Registered Dietician in good standing with the American Dietetic Association’s Commission on Dietetic Registration.
The dietician must provide evidence of being a Licensed Dietitian–Nutrition in good standing with the State of Pennsylvania.
The dietician must provide a copy of liability insurance documentation.
All licensing/certification and malpractice insurance must be maintained throughout the duration of the contract.
The dietician must provide evidence of completion of adequate and required continuing education needed to maintain dietitian registration and licensure.
The dietician must provide an equally qualified substitute dietitian in the event of a prolonged absence.
The dietician must complete all assigned work on time as required by the facility and as specified by the Director of Dietary Services.
The Chief Operating Officer will be notified of all preplanned absences .
 
V. Measures:
At least on a weekly basis, the dietician will communicate work issues and complete a written report that tracks the work completed. This report will be given to the Chief Operating Officer.
At least one week in advance, the dietician will discuss the following week’s schedule with the Chief Operating Officer, then provide the agreed upon written schedule.
 
Role Description:
The dietician must review resident medical records; complete eating dysphagia screens; complete Medicare charting including completion of Minimum Data Set (MDS) work and evaluations; complete nutritional assessments for all residents (initial, quarterly, annual and those required by Medicare); and recommend individualized treatment/care plans for any resident’s nutrition related diagnoses within prescribe time frame.
As needed, the dietician will attend interdisciplinary (team) meetings, residential meetings (e.g. resident care and resident council), and provide educational material to residents, their families and facility staff.
Work involves performance improvement programs and facility committee involvement.
Work includes the development of menus for regular and therapeutic diets as well as addressing the needs for tube feeding orders.
In reference to tube feedings, the dietician must make recommendations that include the name/type of tube feeding, the nutritional analysis of the tube feeding, the amount to be administered, the frequency of feedings as well as the amount and frequency of the water flush.
The dietician must make recommendations to physicians as needed in reference to resident diet orders including tube feedings.
The dietician must chart progress notes monthly on all residents who have physician orders for tube feedings and/or those who are being treated for pressure wounds.
The dietician must write progress notes in the medical records as needed.
If possible, the dietician must respond to all dietary consults on the same day as ordered. If not, the dietitian must consult the Chief Operating Officer who will determine the time line that a consultation will need to be completed.
The dietician is responsible to have current knowledge of dietetic practice, regulatory compliance and department’s policies and procedures.
The dietician must maintain records, reference library and educational materials.
The dietician will maintain computerized data base forms as needed used by the Dietary Department.
The dietician must be present in the dining rooms several times each week during meal service to observe residents at mealtimes to ensure that food preferences are correct and appropriate. Meals observed will include breakfast, lunch and supper.

Required: Certification and maintenance as a Registered Dietitian/Nutritionist (RDN) as issued by the Commission on Dietetic Registration Academy of Nutrition and Dietetics (AND) 
Licensure as a Dietitian/Nutritionist as issued by the North Carolina Board of Dietetics/Nutrition (NCBDN), or in the process of applying. 
Job Summary
The Registered Dietitian/Nutritionist performs a variety of specialized duties related to the nutrition counseling of patients / families for Medical Nutrition Therapy (MNT) diet education. This may include but are not limited to diabetes education, health management, cardiovascular disease, renal disease, malnutrition / weight gain, infants with failure to thrive, renal medical nutrition therapy, gastrointestinal disorders/allergy nutrition therapy, sports nutrition, healthy diet on a budget.
 
Duties
<ol>
<li>Interviews patients / families which may include obtaining a diet history, exercise habits, reviewing emotional/social issues as part of the diet counseling process and documents any pertinent information obtained in the Electronic Health Record (EHR) and documents nutrition notes in the confidential EHR on the Nutrition Services templates.</li>
<li>Provides personalized nutrition counseling and medical nutrition therapy to patients with various health concerns, conducting comprehensive nutritional assessments, developing tailored meal plans, educating individuals on dietary modifications, and monitoring progress to promote optimal health and lifestyle changes and prevention of disease.</li>
<li>Utilizes the principles of the Nutrition Focused Physical Exam to identify malnutrition and/or micro-nutrient deficiencies.</li>
<li>Makes appropriate referrals to other services, such as the Endocrinology, Allergy, Social Services or various Food Assistance Programs as indicated.</li>
<li>Assesses for Eating Disorders or disordered eating and treats or refers as appropriate.</li>
<li>Provides nutrition education to patients before starting the new GLP-1 medications as well as while patients are experiencing the impact of these new medications on their bodies and systems.</li>
<li>Counsels patients / families on medical nutrition / diet therapy principles and restrictions, body image and healthy medical indicators at every size.</li>
<li>Utilizes available medical nutrition therapy education materials or creates them from reputable sources. Uses evidenced-based knowledge to substantiate educational resources.</li>
<li>Creates needed follow-up plan and has staff schedule or reschedule nutrition counseling appointments as needed and time permits.</li>
<li>Collaborates with referred providers to help achieve optimal health goals for patients. Communicates with the needed health professionals and staff to thoroughly assure the best health care for patients of PHC.</li>
<li>Seeks continuing education to keep up to date with scientific nutritional studies pertinent to the types of patients in practice.</li>
<li>Participates in and may conduct Piedmont HealthCare in-services and meets the mandatory annual in-service requirements in utilities management, infection control, safety and blood-borne pathogens.</li>
<li>Conducts or participates in nutrition-related research to advance the knowledge of nutrition in medical care and for the health of patients.</li>

DUTIES:

<li>Assess / monitor the residents' nutritional status, provide recommendations to medical staff, and develop adequate care plans</li>
\n
<li>Evaluate special needs of residents regarding nutrition support, skin breakdown, and significant weight issues</li>
\n
<li>Coordinate procurement of nutrition support supplies and oral supplements with outside vendors as well as involved department heads</li>
\n
<li>Monitor resident meal service to ensure diet modifications are followed</li>
\n
<li>Educate residents, families and staff in concepts of nutrition &amp; diet modification</li>
\n
<li>Routinely evaluate Dietary Care plans for effectiveness to ensure high quality of care</li>
\n
<li>Meet weekly with interdisciplinary team to review and adjust residents’ plan of care</li>
\n
<li>Work closely with Speech-Language Pathologist to ensure appropriate mechanically altered diets are provided for residents with dysphagia</li>
\n
<li>Reviewing all menu changes to ensure they follow facility’s policies/procedures &amp; State/Federal guidelines</li>
\n
<li>Update diet order and menu changes in computer menu software on a daily basis</li>
\n
<li>Supervising &amp; working closely with the dietary team to provide excellent resident care</li>
\n
<li>Conduct audits of areas relevant to providing quality nutrition care on a routine basis</li>
\n
<li>Ensure facility is in compliance with regulations and policies on weight monitoring</li>
\n
<li>Communicate with interdisciplinary team on a daily basis to provide quality care to residents</li>

REQUIREMENTS:
