Job Description 
The Sexual Assault Nurse Examiner (SANE) reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
<ul>
<li>The&nbsp;SANE (Sexual Assault Nurse Examiner)&nbsp;is a registered professional nurse who provides exemplary specialized, trauma-informed medical forensic services to all individuals who presents with a complaint or concern of sexual assault. These services will be prompt, non-judgmental and compassionate while encouraging autonomy and reducing psychological trauma. The medical forensic services may include a medical forensic exam and evidence collection, counseling for emergency contraception, prophylactic treatment for sexually transmitted infections, photo documentation, and referrals for appropriate medical and psychological follow-up, support and participation in legal proceedings.</li>

Responsibilities:
<ul>
<li>Acts autonomously following the professional standards as outlined by the Sexual Assault Survivors Emergency Treatment Act (SASETA), the Illinois Nurse Practice Act, the International Association of Forensic Nurses Scope and Standards of Practice, and unit specific protocols appropriate to the patient population served.</li>
<li>Works collaboratively with the interdisciplinary team, including SANE coordinator, providers, imaging and therapeutic services to facilitate care of sexual assault survivor.</li>
<li>Maintains documentation standards with accurate, timely and complete recording and reporting to ensure compliance with SASETA.</li>
<li>Maintains appropriate survivor privacy and chain of custody for all evidence collected, including Illinois State Police Evidence Collection Kit (ISPECK).</li>
<li>Remains up to date on current regulatory and legislative guidelines related to treatment and documentation standards for sexual assault survivors.</li>
<li>Educates survivors and their families to meet their emotional and healthcare needs to aid in the recovery process.</li>
<li>Performs assigned work safely, adhering to established departmental safety rules and practices, reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and/or visitors.</li>
<li>Testifies in court as a fact witness and/or expert and assists other SANEs in preparing for court proceedings.</li>
<li>Provides a learning atmosphere by sharing knowledge with coworkers; participates in orientation of new staff by providing content relevant to caring for sexual assault survivors.</li>
<li>Supports continuous quality improvement efforts designed to enhance patient outcomes, increase patient satisfaction, and improve the overall care of sexual assault survivors</li>
<li>Ensures professional growth and development by participating in and utilizing current evidence-based information and keeping current with standards in Forensic and SANE nursing practice.</li>
<li>Maintains required medical and forensic continuing education requirements per current regulatory or legislative guidelines.</li>
<li>Participates in goal setting, program development and evaluation of the regional and system-wide SANE programs.</li>
<li>Attends monthly SANE meetings and participates in peer review.</li>

Qualifications 
Required:

<li>Bachelor of Science in Nursing (BSN) or Associate Degree Nursing (ADN) with commitment to obtain a BSN from an accredited program within three years of first day of employment in clinical nurse role OR Master’s degree from an accredited school/college of nursing and Advance Practice Registered Nurse (IL) license</li>
<li>Current Illinois Licensure as a Registered Nurse in good standing</li>
<li>BLS certification upon hire</li>
<li>Completion of the IL SANE Clinical Training Program within 1 year of hire

Position Summary:
CTT is mandated to provide service to those individuals within the mental health system who present the greatest degree of severity of symptomatology as evidenced by their multiple physical, psychiatric and legal conditions, extensive use of services and lack of follow through with treatment. This population requires the highest level of medical necessity as defined by the state Adult Environmental Matrix. The Teams that provide multidisciplinary clinical review and assessment of individuals who may be young adults, heavy users of service, or actively involved in the criminal justice system and may also have ccoccurring behavioral health diagnoses are generic and specialized. The Case Management Specialist (CM Specialist) provides case management supports based on the Community Support Program of Philadelphia with assisting individuals in developing skills required to enhance his/her comfort and abilities to function as a productive member of the community. The CM Specialist takes the lead in one of five (5) areas of specialty: vocational, housing, D&amp;A, forensic or Benefits/Entitlements; and works in collaboration with the primary case manager to identify goals, develop, implement and monitor the service plan. The CM Specialist will ensure that the mission, goals and philosophy of the CTT are operationalized within the team.
The incumbent works with individuals in their community environment assisting them in understanding, acquiring and maintaining independent living skills in the areas of: (a) their daily living situation; (b) interpersonal skills and social support/network building; (c) leisure and recreational support/skill development; (d) maintenance and enhancement of physical and mental needs; (e) obtainment of benefit entitlements and the skills to manage same; (f) vocational and educational needs.
Duties and Responsibilities:
<ul>
<li>Assesses individual’s strengths, needs and wants utilizing instruments to operationalize data on the consumer’s behalf. This includes completion of Strengths Assessment and client-centered Unified Personal Plan for each assigned individual, involving all treatment team principles for comprehensiveness.</li>
<li>Works collaboratively with multidisciplinary team internally on daily basis and external, as needed, to ensure coordination between systems; provides linkage with primary care physician to ensure integration of medical and psychiatric service needs; maintains linkage with CBH to minimize person’s reliance on acute services.</li>
<li>Periodically completes Environmental Matrix to determine level of care needed and delivers services according to individual’s needs; ensuring appropriate frequency of contact.</li>
<li>Monitors individual’s progress toward attainment of identified goals through monthly meetings to review same. Goals should be short term, measurable and obtainable. Plans should be updated according to regulatory standards and all related activities documented on appropriate agency forms.</li>
<li>Liaises, communicates with and represents individuals via telephone contact and face to-face meetings. This includes regular contact with individual and community service providers in order to respond to individual’s changing needs, assist in problem resolution and provide advocacy mechanisms to ensure that needs are met.</li>
<li>Participates in in-patient treatment team and discharge planning meetings; monitors individual’s treatment and progress during hospitalizations.</li>
<li>Maintains and updates community resource file on housing, vocational, D&amp;A, forensic and benefits resources; assists individual in accessing same and provides necessary training around use in order to facilitate individual’s ability to use resources independently; attends DBH or Community sponsored training within their specialty area; provides periodic training to and shares information with team; participates in internal specialist meetings, as scheduled.</li>

<li>Participates in daily team meetings and Clinical Care Meetings to problem solve around persons needing extensive services for specific times; attends Open Forum and other required agency meetings.</li>

Bachelor’s Degree in (including but not limited to) sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling or education with two (2) years related experience preferred; Bachelor’s Degree in nursing with 3 years mental health direct care experience; be a Registered Nurse (RN). Expertise in behavioral health with one or more of the following required: criminal justice system, substance abuse, vocational, geriatric, MR, housing or entitlement issues.
Physical Demands/Essential Functions:
Able to sit for at least one (1) hour at a time. Able to climb stairs when visiting participant residences. Able to lift up to 20 pounds when assisting participants with residential moves. Able to work in the community, some part of every work day.
