The Social Services Coordinator provides post-hospital care services to guest experiencing homelessness who are transitioning out of an acute care hospital. Responsibilities and Initiatives

Position Summary: The Social Services Coordinator plays a critical role in supporting patients across our telehealth platform by connecting them to essential community resources, behavioral health services, financial assistance, and care navigation.
