POSITION PURPOSE: To participate in the planning and delivery of patient care services under either minimal or direct supervision of an RN and directs patient care activities in the absence of an RN. The LPN may function as a Shift Charge Nurse, directing the delivery of patient care on the Skilled, Intermediate and Residential Care Units for an assigned shift (* Sign-on bonus: certain terms and conditions apply). Multiple shifts available including NOC shift.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
 
Functions within the scope of practice for a Licensed Practical Nurse in the State of Oregon. Maintains the confidential nature of the work environment. Participates in the planning and organization of patient care. Coordinates with other departments and care providers, the delivery of patient care services. Performs detailed assignments, works under pressure and relates positively to patients, staff and visitors. Is familiar with the current condition of each patient. Routinely monitors resident care to observe patient condition and monitor delivery of care. Participates in the establishment of the care plan. Initiates emergency measures as necessary. Reports changes in patient condition to Physician, family and as appropriate, to other care givers. Maintains complete and accurate patient records, providing timely, accurate, current documentation. Maintains a safe, clean and comfortable environment for patients. Maintains and updates own clinical skills and learns new skills as necessary to perform unfamiliar assignments. When functioning as Shift Charge Nurse, adjusts daily Nursing Department staffing in both the current shift as well as up-coming shifts by calling in or dropping personnel as needed. Assures timely acquisition of patient observations made by other staff members. Assures adherence to new and established policies and procedures by instruction, supervision and example. Collaborates with the DNS in formal performance evaluations. Able and willing to work beyond normal working hours and in other positions, temporarily, if necessary. When functioning as Shift Charge Nurse, initiates employee counseling and/or disciplinary action as indicated by infraction. 
 
The Charge Nurse may:
<ul>
<li>A) Administer first formal warning for policy infractions.</li>
<li>B) Administer suspension from work of current shift.</li>
</ul>
 
Administers IV Therapy according to established policies and procedures, if certified in IV Therapy. Maintains harmonious shift/inter-shift and departmental/interdepartmental working relationships. Represents the interests and concerns of co-workers to Shift Charge Nurse or DNS. Provides leadership and positive direction in accordance with, and in support of, established philosophy, objectives, policies and procedures.
 
Daily:
A) Does dressing changes, treatment’s, ostomy care, catheter care, catheter changes assigned.
B) Gives all G-tube medications during hours assigned.
C) Completes all accuchecks and insulin injections during work hours.
D) Records all vital signs taken during the night.
E) Completes other tasks assigned by the charge nurse or RNCM.
 
NON ESSENTIAL JOB FUNCTIONS include the following:
Participates in committees as assigned. Provides input to Shift Charge Nurse or DNS regarding program proposals and policy/procedure review. Assures that supply levels and equipment condition maintained so as to be readily available.

Job DescriptionOur Company ResCare Community Living Overview Plans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients. Responsibilities 
<ul>
<li>Provide 1:1 care for client always following client's plan of care while maintaining client's dignity and independence</li>
<li>Monitoring clients' vital statistics and reporting abnormal findings to the case manager and physician assessments</li>
<li>Performs medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results</li>
<li>Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist</li>
<li>Ensures infection control policies are maintained at all times, reports possible communicable diseases or infections, and ensures the overall implementation and practice of infection control procedures are implemented</li>
<li>Accurately documents nursing actions of all care given and communicates with family, case manager, and physicians as well as documents the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needs</li>
<li>Orders supplies and medications, as needed, to ensure adequate amounts are on hand at all times</li>
<li>Ensures all patient needs are being met in accordance with the instructions of the physician.</li>
<li>Monitors incidents involving individuals. Ensures incident reports are completed timely, accurately, and case managers are informed; examines the individual following the incident</li>
<li>Ensures all medical equipment is maintained and reports faulty equipment to Durable Medical Equipment for repair</li>
<li>Other duties as assigned</li>
</ul>
 Qualifications 
<ul>
<li>Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse</li>
<li>Knowledge of individual service plans, including understanding of client rights and health needs</li>
<li>Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice</li>
<li>CPR Certified</li>
<li>Excellent interpersonal skills, including customer service, patience, empathy, and compassion</li>
<li>In-depth understanding of patient care policies and regulations</li>
<li>Familiarity with symptoms and signs of improvement for several medical conditions</li>
<li>Moderate amount of travel between facilities may be required </li>

We are currently seeking a dedicated professional to fill the &nbsp;Licensed Practical Nurse position.&nbsp;To provide support and assist Philadelphia County psychiatrists in coordinating the behavioral and physical health care needs of program participants. Assist psychiatrists with: insurance prior authorization process from completing the forms to notifying the participant when insurance approves; developing and implementing a system to ensure participants have prescribed laboratory tests including how to ensure psychiatrist receives and reviews the results; assist in the Clozaril clinic and work with other nurses in the clinic to facilitate an efficient flow of information from and to the psychiatrists; special projects to improve efficiency in psychiatric services.&nbsp;Network with the community to address the healthcare needs of participants and provide psychiatric nursing services in accordance with physician orders and standard nursing practices. Function as a consultant and provide education to participants, non-medical team members, and the community.
Supervision Received: · Receives general direction from the Medical Director or the Philadelphia BHS Regional Director. Receives medical direction from the psychiatrist and/or the participants’ treating psychiatrists and/or doctor, when applicable. Qualifications: · Board Certified Licensed LPN in the State of PA. · Prior experience in servicing community mental health facilities and their needs. · Comprehensive knowledge of psychiatric clinical and psychosocial rehabilitation practices. · Excellent communication skills, both oral and written; Detail oriented. · Strong interpersonal skills; Team oriented; Very strong organization and analytical skills · Must be available for scheduled emergency on-call service.
Scope: · Frequent talking and listening. · May require verbal crisis intervention assistance in emergency situations. · Must be available for local travel and possess a valid driver’s license with an acceptable driving record. · Scheduling flexibility and possible on-call work are necessary. · May be required to report to work during emergencies including inclement weather.
